No elaborate setup here. Bounced a flood light off the counter to reduce harsh shadows on the bible. Used a T-Mobile poster to block the light from spilling into the background. To create the background lights, I used a Christmas wreath and a few candles.
To create custom bokeh, all you have to do is use a large aperture setting and a piece of paper with a symbol cut out. My symbol cutout is about 15mm high and 7mm wide. Next, focus in on your subject, then place the paper filter over your lens and shoot away. You may want to set your lens to Manual Focus once you have the focus. Silvertone elec guitar run through a Mesa Boogie Combo (Mark II prototype formerly owned by Lou Reed)

Pair of lightstands in background with a manfrotto crossbar holding up a roll of white seamless with a pair of 'A' clamps and three strips of gaffer's tape holding the paper in place.
2x 580EXII strobes into background (next time I'll pull the subject further away…just didn't have the patience/energy for it tonight).

Setup to make this photograph: Merry Christmas
A plant pot lined with layers of kitchen roll to regulate the water drops through the hole in the botom was balanced on pint glasses with a sponge beneath. A Christmas card was placed upside down in the background.

Setup for a few shots I took of some cupcakes.
Sunpack 383 on 1/4 power camera left with grid. Cupcakes are sitting on thin plexiglass with black foamcore underneath. Black reflector to side to eat the light.
Setup for portrait of Disa. Ambient is contributing pretty much nothing to the exposure.
Now that I have an umbrella I can steal Hobby's full setup. Today I also had access to a third flash, so I added a rim/chin light too. It was a bit too hot and probably should've been placed more behind subject as well.
