PATIENT PROTECTION AND AFFORDABLE CARE ACT: CMS Should Act to Strengthen Enrollment Controls and Manage Fraud Risk

Notes: A single application may represent more than one person, and different people on an application may have different number or types of inconsistencies. Because subsidy information is at the application level, subsidy amounts are not mutually exclusive by category.

a) Other inconsistency types are American Indian status, and presence of qualifying employer-sponsored coverage or other minimum essential coverage.

b) Resolved status indicates inconsistencies resolved by consumer action, such as document submission, or removed due to events such as life change, application deletion, or consumer cancellation.

c) Open status indicates applications with inconsistencies that had no reported resolution as of April 2015. Figures by category of inconsistency do not sum to total because the categories are not mutually exclusive.

d) Terminated/adjusted status indicates the federal Health Insurance Marketplace has terminated policies or adjusted subsidies based on failure to submit documentation to resolve inconsistencies.

A few small uploads of things I've had forever.

The Terminator CIWS is an incredibly powerful ship defense weapon used by the US Navy on only the largest ships. It features twin GAU-8 30mm gatling guns each with 1100 rounds, as well as 8 RIM-162 Evolved SeaSparrow Missiles. The system can track up to 25 targets and engage at up to 30nm. Terminators also feature a mid power laser for engaging aircraft launcher anti-ship missiles. They are generally used in compliment to Phalanx II CIWS and stand-alone RIM-162 units. Plans exist to mount Terminators on HEMTT trucks or even Krieger IFV chassises to serve as a mobile, mid-range missile defense system.

Heavily inspired by the Russian Kashtan CIWS.

Love the composition between an actual housewife from the fifties and me.

The Easter Parade terminated outside my apartment block but as my windows face away from Bolton Street I was afraid that I might not be able to get any usable photographs of the parade but things did work out too bad even though members of the public did frequently block my view of the event.

Since Ireland joined the United Nations in 1955, the Army has been deployed on many peacekeeping missions. The first of these took place in 1958, when a small number of observers were sent to Lebanon. A total of 86 Irish soldiers have died in the service of the United Nations since 1960

As of 1 December 2015, 493 Defence Force personnel are serving in 12 different missions throughout the world including Lebanon (UNIFIL), Syria (UNDOF), Middle East (UNTSO), Kosovo (KFOR), German-led Battle Group 2016 and other observer and staff appointments to UN, EU, OSCE and PfP posts. The largest deployments include:

Lebanon (UNIFIL) 51 Infantry Group, Syria (UNDOF) 50 Infantry Group.

The Army has historically purchased and used weapons and equipment from other western countries, mainly from European nations. Ireland has a very limited arms industry and rarely produces its own armaments.

From its establishment the Army used the British-made Lee–Enfield .303 rifle, which would be the mainstay for many decades. In the 1960s some modernisation came with the introduction of the Belgian-made FN FAL 7.62 mm assault rifle. Since 1989 the service rifle for the Army is the Austrian-made Steyr AUG 5.56 mm assault rifle (used by all branches of the Defence Forces).

Other weapons in use by the Army include the USP 9mm pistol, FN MAG machine gun, M2 Browning machine gun, Accuracy International Arctic Warfare sniper rifles, AT4 SRAAW, FGM-148 Javelin Anti-tank guided missile, L118 105mm Howitzer, RBS 70 Surface to Air Missile system.

The Army has purchased 80 Swiss made Mowag Piranha Armoured personnel carriers which have become the Army's primary vehicle in the Mechanised infantry role. These are equipped with 12.7 mm HMGs, or the Oto Melara 30 mm Autocannon. The army also has 27 RG Outrider light tactical armoured vehicles. The Army has no tanks, but does have a variant of the FV101 Scorpion light armoured reconnaissance vehicle, with a 76.2 mm main gun.

The full Terminator Squad. all ready for combat. Has been a long time since I completed a squad fully and it is definitely motivational to see them all painted and finished. Shall now begin to finish my vanguard veteran squad!

A rather glorious and beautifully produced (it is bound in slik tape) handbook describing the industrial and municipal services of the Lancashire borough of Salford and lavishly illustrated to show scenes of the borough and various industrial activities. It is obviously aimed at VIP visitors to the Civic Hall at the 1924 Briitsh Empire Exhibition at Wembley and as well as Salford's claims to fame and importance it includes descriptions of the various colonies and Dominions and their trade with Lancashire.

Salford was an important industrial centre in its own right, albeit often overshadowed by its neighbour Manchester, and indeed in 1926, two years after this publication, the County Borough was raised to City status matching that of its neighbour. Salford also shared the the spoils of the Manchester Ship Canal, that incredible engineering feat that had made landlocked Manchester one of the largest port facilities in the UK - if only because a large acreage of the docks themselves was administratively in Salford. It meant that the borough was well placed as an entrepot - handling imports and exports via rail and road links across the south and south east Lancashire conurbation. Needless to say cotton, raw in and finished goods out, made up a major part of this trade.

The book also describes Salford's municipal services such as transport, gas and electricity - seen as vital in 'selling' the borough to potential investors and traders. This advert is for Salford's Tramways Department and rather nicely shows an outline route map illustrating the tramways within the borough and the first few motor bus routes that where being operated. Salford, like many other Manchester area operators, ran an extensive system of linke and jointly operated services so for example both Manchester and Bury Corporation vehicles could be seen on Salford routes. The map shows the adjacent local authorities who, although they owned the actual tram tracks, leased operational rights to Salford Corporation. The system's tracks were also connected to those of a private operator, Lancashire United Tramways.

Salford was also famous in that due to the close proximity of the two city centres, divided by the River Irwell, many of its tram routes effectively terminated over the boundary in Manchester. The decision to scrap the trams was therefore bound up rather intimately with, in particular, Manchester's early decision to convert their system to buses, a process that strated in the 1930s but that was delayed by the outbreak of war in 1939. The last remaining trams soldiered on until 1947, two years before Manchester finally abandoned its last route and the motor bus reigned supreme. In 1969 Salford City Transport passed to SELNEC PTE although, in 1999, light rail returned to the City when the Metrolink system was extended to Eccles returning street operation to Eccles New Road.

A final touch is the coat of arms in the central cartouche of the map compass.

Putting the other half of that Calth characters sprue to use. This was a pretty simple kitbash, even if making room for the Terminator Lord head took a bit of creative cutting.

Really happy with the end result of this conversion, and the base - a mini diorama of sorts, I suppose - was quite the striking success, in my opinion. Certainly not bad for a spare bloodletter head, some hot glue and a paperclip. :)

If I could choose to improve on one thing, it would be the glow of the staff head - I've tried blue glows a few times, without any great success. Might be that I simply don't have the right colours - I may have to pick up some electric blue layers if I keep making psykers.

All in all though, proud of this project and somewhat self-satisfied to be able to say I now have a fully converted representation of each Chaos Marine HQ. I may have to add alternative versions as time goes on - a Jump Pack Lord, a Sorcerer on a Disc of Tzeentch, etc - but for now, I'd say this is quite an achievement in of itself. :)

C&C appreciated as always!
