NYPD Officer Reggie Hillaire, here in an examination room at Memorial Sloan-Kettering Cancer Center, has cancer related to post 9/11 toxic exposures. Officer Hillaire had his thyroid removed due to thyroid cancer and now has multiple myeloma.
Vigorous palpation of the thyroid may result in rupture of follicles with extravasation of colloid into adjacent tissue. The extrafollicular presence of colloid incites a granulomatous reaction which has been called “palpation thyroiditis”. This is encountered as an incidental finding in resection specimens of thyroid. It has been suggested that this lesion may also follow pressure exerted on the thyroid during sonography.

This was an incidental 0.5cm nodule in a thyroid removed for multinodular goiter. The tumor stained with synaptophysin, chromogranin, and calcitonin. Thyrogloblulin was negative.

Meet Willa; A prime example of one of those animals who has suffered in silence. Willa is a darling, spayed, two year old Shepherd/Shar Pei mix with the most expressive, comical ears you have ever seen and a set of goofy webbed feet to match. And to those who meet her for the first time, Willa is a submissive, loving little girl who rolls over for belly rubs and can't wait to meet new people.
Other than some hair loss due to a low thyroid which is now on the mend with a very inexpensive daily medication, you wouldn't see the scars of Willa's abuse until you get to know her and until you meet with her trainer who has helped us to unravel the mystery of Willa's horrific past.
Willa came to us when one of our volunteer members was introduced to her at a county shelter, just days before she was to be euthanized. She caught the attention of our volunteer when the shelter workers tried to take her out for a walk and Willa flattened herself to the ground, with no clue what it meant to be on the end of a leash. Willa's behavior, combined with the fact that she had virtually no hair, compelled those of at Take Me Home to spare her life and get to the bottom of her needs.
Medically, Willa's needs were simple: complete hair loss due to low thyroid which is 100% curable with daily medication. But psychologically, Willa's needs were still a mystery. If she had been abused (hit, beaten, inappropriately scolded), then why was she so happy to meet new people and showed no outward signs of aggression? And if she hadn't been abused, then why the strange behavior at the shelter which would randomly reoccur in the middle of our walks with her? And why did Willa always refuse to go potty on her walks no matter how many miles we walked; only to immediately urinate and defecate as soon as she returned to her dog run?
Willa's behavior didn't add up. That is, not until we determined that we needed a professional to address her needs and invested in her training with Linn Boyke of Zen4K9s (www.Zen4K9s.com).
Linn is one of only three apprentices to Cesar Millan and is highly skilled at understanding dog behavior. Once Willa moved in to Linn's facility, it did not take him long to determine the cause of her strange behaviors. We now understand that Willa has what is called "Dirty Dog Syndrome." In short, prior to being turned into the county shelter, Willa likely spent the first two years of her life confined to a crate so small that it was impossible for her to remove herself from her own waste. Forced to lay in in her own urine and feces, Willa learned that without a choice to go potty anywhere but right underneath herself, it must be OK to lay in your own filth. This is a common syndrome of puppy mill dogs that are forced to live, breed and give birth in tiny cages; cages that are often times not even large enough for the dog to stand up and turn around; which is likely the cause of Willa's flat feet. "Dirty Dog Syndrome" is a learned behavior and it can be broken, but it is a matter of reversing months, and in Willa's case, years, of unthinkable abuse. For Willa to CHOOSE to urinate and defecate on herself, even after being given the option of doing so out on her walks, we now understand the degree to which she suffered; alone, hairless, and covered in her own urine and feces.
Take Me Home is searching for someone who understands that the dogs with the greatest needs are often the dogs that appear to be perfectly healthy. We are looking for someone who will realize the blessing of adopting a dog that has received months worth of professional training, along with the opportunity to work with Willa's trainer so that she can take all the skills she has learned at her training facility into her new home. We are looking for someone who doesn't feel sorry for Willa and will just "forgive" her for accepting a life of living in her own waste; and instead will challenge her on a daily basis to demand better for herself and to not give in to the submission that comes from years of abuse.
We are looking for someone who realizes that as far as special needs dogs go, Willa has a lot of positives; she's not an aggressive dog that can't be around new people, she doesn't require costly medical care, and she's still young with her whole life ahead of her.

Tc99m, a synthetic radionuclide, is taken up by the thyroid just as iod, and thus, you can record the activity with a gamma camera - making the thyroid and its function visible.
other parts of the body take up Tc99m as well, but only in minor amounts - if you take a close look, you can see my neck and where my torso starts though, in lighter blue.

Depends on your perspective really, on whether you see this image as two-dimensional, two-dimensional but representing three-dimensional, or a part of a map. Which is fitting, because this is also a classic perspective shot. So what's my perspective today? I suppose I quite like the idea that it could be a representation of all three really; that things can only get better after yesterday's failure to launch, that one must always look to what's ahead and keep moving and living, without dwelling on time that could have been better-invested, and that the solution to these objectives is to go to Brighton Town Centre. Of course, this road also leads straight to the station, which could be symbolic in its own way, of interstices and choices and open options.
Good old world being so filled with symbolism of living in the world!
I started a Very Low Calorie Diet (VLCD) on Monday, which is a really great way to keep my thyroid activity on a more even keel regardless of how I medicate, as well as a theoretically great way to lose some of the Implant weight, but past experience and wide acknowledgement of online communities tells me that the first week or so often mess up your moods quite a lot. The body has to make a lot of adjustments to cope with radical alteration to fuel supplies and it all makes sense, but it always feels a bit frustrating to me that I start these eating regimes filled with optimism and positivity and then am immediately punished for it! But experience has also taught me that the long game pays off. So there's one path along which I am slowly edging forwards. I apologise if I am bad-tempered this week - this too shall pass!
Having said that, I just feel tired today, but fairly equanimous. I think the brilliance of the sunshine today helps a lot!
There are a lot of things about 2012 that really felt like standing still; I left my PGCE course after a year and a half, due to recurrent illness (and ideological conflicts); I took on a different role but essentially returned to my old job (which nevertheless is well-paid and enjoyable); I spent the vast majority of it in pain or bordering on insane, and thus didn't accomplish very much in terms of visiting friends or being proactive. On the vast plus side, I spent the year living in our insanely beautiful spacious flat with Tom and Puppy, so it's still an easy contender for best year ever - but it does mean I'm starting this year with a feeling that I want to move life forwards.
It doesn't really need to involve changing my job or studying more or anything like that - I have never really measured progress or achievement in terms of income or acquisitions or career; I suspect this is because I spent so much of my formative years just trying to put everything I had into appearing sane and balanced, since my hypothyroidism went untreated for so many years. Earlier still I was such a social reject, all I could imagine desiring from life was that I have friends and be the sort of person who people wanted to be around instead of all running away from across the school field because it was funny to leave me out.
So for as long as I can remember the only things I've wanted were to belong and be liked, and to feel happy. Those, for me, have been my formative goals - and, as it turns out, relatively straightforward to accomplish! I felt like I'd pretty much got on top of them several years ago now, yet every year that passes feels like a new step on that path. The good thing about having abstract goals like that as my ultimate life goals, is that even though I feel like I've achieved them, I don't feel like I've reached the limit of achieving them - unlike, say, completing a marathon. The goal isn't over; it's as much a goal to sustain that way of life.
But I am frequently surprised by how many people I encounter who don't feel happy and don't feel like they are accepted and have friends; it makes me feel like perhaps it was worth concentrating my efforts on those as goals, that perhaps they don't come as naturally to everyone as it looked when I was standing so very much on the outside. It's so easy to assume when you're 6 that everyone else just 'gets' it and that your role in society is really different to theirs because you don't get it. more than 20 years later, I have to remind myself sometimes that I'm not 6 anymore, and realistically don't even need to recall much at all of what I believed as a 6 year old.
That was a really convoluted way of saying that I intend to work at making this a year that I feel like I'm moving forward in those goals, but maybe try to think up some short term goals that give me a sense of developing in some tangible way, to compensate for a year of relative static.

for his blood test. He has a thyroid condition and has to be monitored to make sure he has the correct dosage. He has also been limping on his paw so we need to see what is going on.
