When the Yarn is in the neutral position and there are equal loops on the top and bottom needle you are ready to move to the next step. It is recommended that you cast on a few extra stitches to keep things tidier. Those extras will disappear in a couple steps.
For this demo we are actually casting on 6 total stitches.

From the neutral position after casting a stitch onto the top needle, pass the working yarn down and around the bottom needle.
The working yarn will always move down and around the bottom needle from the neutral position.

Take the working yarn and pull it up and over the TOP needle. This is one stitch cast-on.
The working yarn will always move up and over the top needle from the neutral position.
Using two circular needles allows you to have a bit more play than using DPNs. This becomes obvious here. Pull the bottom needle to your right and let those loops rest on the cable. This gives you more room to maneuver the other needle.
Here the extra loops have been dropped. Extract the tail from being wrapped around any cables, but be sure you maintain three knit stitches on the right hand needle.
We'll clean up the tail area in a bit.
Knit the stitches on the top needle once more. This is to give you a bit more room for making any increases needed to form a toe or the top of a hat or anything else where a seamless start is required.
Sew the center square to the rectangle above it. Start the seam where the two ends are aligned and sew ONLY 1/2-to-2/3 across the small square. Press the partially sewn seam. Complete instructions for this block blogged here: Quartered Star Quilt Block
