Ruscus aculeatus is a low evergreen Eurasian shrub, with flat shoots known as cladodes that give the appearance of stiff, spine-tipped leaves. Small greenish flowers appear in spring, and are borne singly in the centre of the cladodes. It also showed reduction in venous insufficiency in two other studies. It was approved by the German Commission E guidelines for hemorrhoids treatment. It is occasionally prescribed for varicose veins which can be a complication of pregnancy. However, since it is classified as a natural product, there is no evidence or trials to suggest complete safety for the fetus. A qualified healthcare practitioner should be consulted prior to using this compound during pregnancy.

Contraindication

Like full agonist opiates, buprenorphine can cause drowsiness, vomiting and respiratory depression. Taking buprenorphine in conjunction with central nervous system (CNS) depressants in people who are not tolerant to either agent can cause fatal respiratory depression. Sedatives, hypnotics, and tranquilizers can be dangerous if ingested with buprenorphine by a person who is tolerant to neither opioids nor benzodiazepines. Co-intoxication with ethanol carries the greatest risk for lethal overdose, with the lowest doses of a reported fatality in a 48kg teenage girl with 5mg of diazepam and the equivalent of 8 ounces of beer (1 unit of alcohol), plus around 2mg of buprenorphine. However, this female was tolerant to none of the three drugs she ingested that were the cause of the MDI. 2mg of buprenorphine is equal to roughly 80 milligrammes of morphine. 80 milligrammes of morphine itself is considered an extreme dose for an opioid-naive patient with doses starting around 5 to 10 milligrammes.[23]

Common adverse drug reactions associated with the use of buprenorphine are similar to those of other opioids and include: nausea and vomiting, drowsiness, dizziness, headache, perspiration, itchiness, dry mouth, miosis, orthostatic hypotension, male ejaculatory difficulty, decreased libido, and urinary retention. Constipation and CNS effects are seen less frequently than with morphine.[24] Hepatic necrosis and hepatitis with jaundice have been reported with the use of buprenorphine, especially after intravenous injection of crushed tablets.

The most severe and serious adverse reaction associated with opioid use in general is respiratory depression, the mechanism behind fatal overdose. Buprenorphine behaves differently than other opioids in this respect, as it shows a ceiling effect for respiratory depression.[24] Moreover, former doubts on the antagonisation of the respiratory effects by naloxone have been disproved: Buprenorphine effects can be antagonised with a continuous infusion of naloxone.[25] Concurrent use of buprenorphine and CNS depressants (such as alcohol or benzodiazepines) is contraindicated as it may lead to fatal respiratory depression. Benzodiazepines, in prescribed doses, are not contraindicated in individuals who are tolerant to either opioids or benzodiazepines.

People on medium- to long-term maintenance with Suboxone or Subutex do not have a risk of overdose from buprenorphine alone, no matter what dosage is taken or route of administration it is taken by, due to the "ceiling effect" on respiratory depression. Overdoses occurring in maintenance patients are cases of multiple-drug intoxication, usually buprenorphine taken with excessive amounts of ethanol and/or benzodiazepine drugs. It is safe for a patient to take a prescribed dose of benzodiazepines with buprenorphine as long as the patient has a tolerance to either opioids or benzodiazepines. As a matter of course, all patients on buprenorphine maintenance are tolerant to opioids, and maintenance doses are always higher than the dose at which the "ceiling effect" on respiratory depression is reached (~3±1 milligrammes, depending on method of analysis).[citation needed]

People switching from other opiates should wait until mild to moderate withdrawal symptoms are encountered. Failure to do so can lead to the rapid onset of intense withdrawal symptoms, known as precipitated withdrawal.[26] For short acting opioids such as codeine, hydrocodone, oxycodone, hydromorphone, pethidine, heroin, and morphine, 12–24 hours from the last dose is generally sufficient. For longer acting opioids such as methadone, 2–3 days from the last dose is needed to prevent precipitated withdrawal.

Conversely, switching from buprenorphine to other opioids is generally safe and can occur immediately. For users of Suboxone, it is advised to wait a few hours from the last dose before switching to other opioids to allow the naloxone in Suboxone to be eliminated from the body (it has a short half-life). Generally the new opioid will not be as strong or effective for several days until the remaining buprenorphine has been eliminated from the body. This is due to the blockade effect, where the buprenorphine is strongly bound to the opiate receptors in the brain and not allowing the new (full agonist) opioid to completely bind to and activate the receptors.

The "blockade effect" of buprenorphine further explains the phenomenon where Suboxone users trying to get high off of say oxycodone cannot until many (3 or more) days have passed since their last Suboxone dose. This discourages the patient using Suboxone properly for maintenance therapy from using other opioids recreationally to get high because either a) the "high" experienced by a typical recreational dosage of an opioid (say 30-75mg of oxycodone) is subjectively poor i.e. not euphoric and/or b) the amount of money needed to get "high" off of other opioids becomes so large that the patient may feel that recreational usage of other opioids is simply not worth it and that the calming and soothing feeling of using Suboxone properly under the care of an approved physician is simply a better way to live life.

Buprenorphine and norbuprenorphine, its major active metabolite, may be quantitated in blood or urine to monitor use or abuse, confirm a diagnosis of poisoning or assist in a medicolegal death investigation. There is a significant overlap of drug concentrations in body fluids within the possible spectrum of physiological reactions ranging from asymptomatic to comatose, and therefore it is important to have knowledge of the route of administration of the drug and the individual's tolerance to opioids when interpreting analytical results.[27]

Buprenorphine is also used recreationally, typically by opioid users, often by insufflation. Recreational users of Suboxone who crush the tablet and insufflate it report a euphoric rush similar to other opioids in addition to a slight "upper"-like effect. Those already using buprenorphine/Suboxone for opioid addiction therapy find that insufflation is only slightly, if any stronger than taking the pill sublingually, although it may have a quicker onset. Those taking it for addiction therapy also report that obtaining euphoria is virtually impossible after the first few doses. Many recreational users also report withdrawal symptoms. Due to the high potency of tablet forms of buprenorphine, only a small amount of the drug need be ingested to achieve the desired effects.

Although some people do use buprenorphine for purely recreational reasons, the majority of its illicit users use it for addiction therapy. Many people report it being effective in preventing withdrawals in-between doses of their opiate of choice. Illicit users who do not want it on record may also obtain it on the street to use as a less-painful method of quitting than "cold-turkey". Some report needing as little as one 8 mg tab which is broken up into gradually smaller doses which they take in order to effectively wean themselves off the opiate/opioid they're addicted to. The illegal and potentially dangerous self-dosing of Buprenorphine is deemed by many street users as a less risky alternative to what an addict may do on the streets to obtain money for their addiction (an 8mg tablet of either Suboxone or Subutex sells for as low as $10 in some areas, and as high as $20-25 a piece in Chicago, Illinois, with pharmacies charging generally around $14 for a single tablet and $6-8 per tablet in 30+ quantities under self-pay)[citation needed], and less dangerous than quitting cold turkey. Furthermore, most U.S. doctors authorized to prescribe Suboxone charge ~$300 for a first visit, plus several hundred more for follow-up visits, which makes going through official channels more expensive than simply maintaining the original opiate addiction, for some users.

Buprenorphine abuse is very common in Scandinavia, especially in Finland by Viimeinen Hidas and Sweden. In 2007, the authorities in Uppsala county in Sweden confiscated more buprenorphine than cocaine, ecstasy and GHB.[28] In Finland recreational use of buprenorphine is on the rise; in 2005, Finland's incidence of Subutex abuse (most often injected intravenously) surpassed the incidence of recreational usage of amphetamines. Intravenous administration of dissolved Subutex pills and insufflation of pulverized pills are the most common modes of recreational buprenorphine use.[29]

What is Rectal Prolapse?

 

Rectal prolapse is another form of pelvic organ prolapse. Rectal prolapse occurs as a result of the rectum (the last part of the large intestine) being displaced or forced from its normal position. This can lead to the rectum descending or protruding out of the anus.

 

Rectal prolapse is often more common in older adults, more often in older women, who have experienced prolonged issues with constipation or pelvic floor issues. However, rectal prolapse can affect men and women in various stages of life. Although it is reported to be more common in women postmenopause, it can also occur in people much younger as well.

 

In this article we will address some of the most pressing questions regarding rectal prolapse and how it may be treated.

 

Types of Rectal Prolapse

 

As with other forms of prolapse, rectal prolapse is often a term that defines various stages of a herniated anus. The three types of rectal prolapse include:

 

Stage 1: the rectum has become displaced, but does not protrude out of the anal cavity (also know as an internal prolapse)

 

Stage 2: the rectal lining begins to descend into the anal cavity

 

Stage 3: the rectum extends out of the anus entirely.

Causes of Rectal Prolapse

 

A rectal prolapse may occur due to various health conditions or as a result of aging. In some cases, it may be a result of complications during pregnancy or injuries to the pelvic or bowel region. Below are a few possible causes of rectal prolapse.

 

Chronic constipation or diarrhea

 

Prolonged or chronic history of straining during bowel movements

 

Old age: as you grow older, it is common to experience a weakening of the rectal and anal muscles and ligaments. It is also common to experience similar weakening in the pelvic floor, which can create additional weakness in your core.

 

Weakening of the anal sphincter

 

Injury or past surgical procedures to or near the anal and pelvic regions

 

Nerve damage: If the nerves that control your ability to contract the rectum or anus are damaged, rectal prolapse may occur. This can be due to complications during vaginal delivery, paralysis, a spinal injury, lumbar spine surgery, or other surgeries in the pelvic area.

 

Other diseases, conditions and infections: There are many other possible causes for rectal prolapse. These can include health conditions such as diabetes, cystic fibrosis, parasitic infections, or other diseases. Or, it may be caused by other corrective surgical procedures, such as a hysterectomy.

 

Rectal Hernia Symptoms

 

The most common symptoms of rectal prolapse is the feeling of a bulge or ball extending out of the rectum. This can look like a reddish-colored mass extending outside of the anus, or, in the case of an internal prolapse, feel as if something is stuck in your rectum. These symptoms are often experienced during or after a bowel movement. In many cases, the symptoms will resolve quickly. However, if you have a moderate to severe rectal prolapse, over a prolonged period of time, you may experience the rectum protruding out of the anus at the end of the day. This is often spontaneous and may not require force or straining.

 

Other symptoms may include:

 

Anal pain

 

Rectal bleeding (may appear like a hemorrhoid)

 

Fecal incontinence (mucus, blood, stool)

 

The symptoms you suffer may change depending on the severity and progression of your prolapse.

 

Diagnosis

 

A rectal prolapse requires a proper diagnosis in order to rule out conditions that may correlate. In order for you to receive a proper diagnosis, your doctor will have to perform a rectal exam. Your doctor may ask you to sit on a toilet and attempt to perform a bowel movement in order to see the prolapse.

 

If your symptoms are varied and severe, there may be a need for more advanced testing in order for your doctor to properly diagnose rectal prolapse. This may help rule out other possible conditions. Below are a few tests your doctor may recommend:

 

Anal electromyography (EMG): This test helps determine nerve function and whether or not nerve damage may be causing your rectal prolapse.

 

Anal manometry: During this exam, your doctor will insert a small, thin tube into the rectum to test rectal muscle strength.

 

Anal ultrasound: An anal ultrasound will help check the muscles and supportive tissues to determine whether they are damage or if there are signs of an internal prolapse.

 

Pudendal nerve terminal motor latency test: This test checks to see how well your pudendal nerves function. These nerves control your bowel movements.

 

Proctography: A proctography is an x-ray that is performed during a bowel movement to determine how well your sphincter holds and releases feces.

 

Colonoscopy: A tiny camera is inserted into your rectum to determine rectal and intestinal health and look for any signs that may be causing a prolapse.

 

Proctosigmoidoscopy: Like a colonoscopy, a small tube with a camera attached will check your intestine for signs of injury, inflammation, scarring, or possible tumors.

 

MRI: An MRI will check all of the organs located in the abdomen and pelvic region.

 

Risk Factors

 

It is more common for women to develop a rectal prolapse than men. However, if you are older and have a history of long term issues with constipation or pelvic floor issues, you may be at a higher risk of developing rectal prolapse.

 

Long terms bouts with the following may increase your risk for rectal prolapse:

 

History of constipation, chronic constipation or diarrhea

 

Constant need to strain while pooping

 

Lower back injury or disc disease

 

Muscle weakness in your anus or pelvic floor

 

Genetics or family history of prolapse or weakened pelvic floor

 

Chronic obstructive pulmonary disorder (COPD)

 

Benign prostatic hypertrophy

 

Injuries or complications with ligaments and muscles that keep your rectums attached to the intestinal wall.

 

Congenital bowel disorders like Hirschsprung’s disease or neuronal intestinal dysplasia

 

How to Treat Rectal Prolapse

 

In most cases, your doctor will recommend surgery. There are various surgical procedures that can help resolve severe rectal prolapse, but they will depend on the patient’s age, severity of the prolapse, and your exam results. This may also be determined by the colon and rectal surgeons’ preference and experience. The two most common surgical procedures are abdominal surgery and perineal surgery.

EPO (erythropoietin): EPO is a chemical form of blood doping that first came about in the late 1980s allowing aerobic potential to be increased by increasing the blood's oxygen transfer capacity. Its misuse in the professional ranks arrived soon after it was used on the patients it was designed for. EPO artificially boosted the body’s red blood cell count . Boosting an athlete’s red blood cell count (and thus the efficiency with which oxygen is transported around the body) in order to improve performance, is done by injecting erythropoietin – a hormone produced by the kidneys that stimulates production of red blood cells - and it gives a massive advantage in performance. It was an undectable until a test became available in 2000. The use and abuse of EPO was rampant in professional cycling, according to riders who competed at the time. This is underlined by the amount of cyclists caught using the substance. The greater the amount of red blood cells available, the more oxygen can be carried from the lungs to the muscles. EPO is typically taken prior to a big training block - out of competition - to ensure that by the time of the competition all traces of the synthetic EPO will have disappeared. It enables the doper to train harder and longer than he would be able to “naturally”. Cheaters can also “micro dope” by staying under the fail limit, as an aid to performance on the event.

The obvious danger with EPO is that the body’s capacity to produce red blood cells naturally is compromised, with the athlete ultimately having to rely on injections of the hormone ( for the rest of his life ) .In May 2007, the T-Mobile cycling team suspended two doctors who allegedly supplied EPO to former Tour de France winners Bjarne Riis, Jan Ullrich and other riders on the team (then known as Team Telekom) between 1992 and 1996, more than a decade earlier._Though EPO is now a prohibited substance in professional cycling, and riders are regularly tested for its presence in their bodies, its use is still a problem. CERA is the most efficient form of the Drug; the ‘third generation’ EPO was only detectable in drug testing from MAY 2008.

  

Blood Doping: Blood doping is when an athlete illicitly boosts the number of red blood cells (RBCs) in his or her body in order to enhance athletic performance. An athlete has healthy blood 'removed' during periods when the body is at its freshest (e.g after a period of rest), which blood is stored and transfused 'back' into the rider when needed.

The benefit of a transfusion of half a litre of blood can provide the athlete with an additional half litre of oxygen to muscles per minute, at the same time increasing the capacity of the muscles to use oxygen by up to five percent. Autologous blood transfusion (transfusion of your own blood) is not detectable and is perhaps not technically “doping”, but remains a banned technique affording a massive boost to an athlete over fatigued competition.

Poorly stored blood leads to serious illness and blood clots, and by pushing “thick blood” with high red cell count through the body, massive strain is put on the heart. The main function of red blood cells is to carry oxygen to the muscles, meaning a higher RBC count can dramatically improve an athlete’s performance by kicking up their aerobic capacity and stamina.__Blood doping in the traditional sense involves harvesting red blood cells either from the athlete or compatible donor and then saving them until needed. Red blood cells can be easily frozen and later thawed for use without significant loss of their oxygen-carrying properties. Just prior to a critical competition, these harvested red blood cells are then reinjected into the athlete’s circulatory system._The major blood doping case in professional cycling is known as Operación Puerto, the case name given by Spanish police. The investigation broke in May 2006 when Dr. Eufemiano Fuentes was accused of helping 200 professional athletes from a number of sports engage in blood doping to enhance their performance. Among these athletes were some of the best known cyclists in the world, including several of the top finishers at the most recent Olympic Games and Tours de France._As of May 2007, fifteen cyclists whose names surfaced in the Operación Puerto investigation have been acquitted, while three riders -- Jan Ullrich, Ivan Basso, and Michele Scarponi -- have either admitted doping or been seemingly convicted by substantial evidence linking them to the practice.

 

Human Growth Hormone: When the aim is to increase strength and muscular power (to improve technique) protein, natural or synthetic anabolic agents in combination with hyper-protein diets and muscle-building exercises are frequently used. The balance between the increase in muscle mass and the loss of fat mass can be maintained by growth hormones associated with aminoacids, drugs with anabolic properties, or with nutritional supplements.

Growth hormones stimulate cell growth, aids in muscle mass building and their recovery. Typically it is injected prior to a period of training (to aid muscle development during training), but can be used during competition to speed up recovery from fatigue. Abuse of HGH leads to thickening of the bones (particularly the jaw bone), swelling of hands and feet and increased organ growth.

HGH is currently not tested for efficiently.

Symptoms

 

The symptoms of a prolapse somewhat depend on which organ has descended. If you are suffering from a bladder prolapse, urinary incontinence may occur. If it is a rectal prolapse, constipation, painful bowel movements, and painful sex often occur. Lower back pain, painful sex, and bowel obstruction or incontinence tend to accompany a small intestine prolapse as well. If you are suffering from a prolapse of the uterus, you may also suffer from uncomfortable intercourse, incontinence, and lower back pain.

 

In some cases, there may never be signs of a prolapse or the presence of any painful symptoms. However, in others some women report symptoms ranging from:

 

Pressure or sense of fullness in the pelvic area

 

Lower back pain

 

Painful sex

 

Sensation of something falling out of the vagina

 

Urinary incontinence or frequent urination

 

Constipation

 

Spotting or bleeding from the vagina

 

In cases where there may be a severe prolapse, POP symptoms may worsen. One may notice:

 

Bulging of the vaginal region

 

Needing manual assistance during a bowel movement

 

Difficulty urinating, spraying stream of urine

 

The need to lift the bulging vagina in order to urinate

 

Urinary leakage during intercourse

 

Recurrent UTIs or kidney infections

 

How long does a pelvic floor repair operation take?

 

The length of the surgical procedure can vary greatly from patient to patient. The general estimate for the surgery ranges from around 3-5 hours, but may have a shorter or longer duration depending on various factors. Some of the elements that may determine the length of the operation can include:

 

Internal anatomy

 

Shape of the pelvis

 

Weight

 

Internal scarring or inflammation

 

Prior abdominal/pelvic floor surgery

 

Does Surgery Really Work?

 

Surgery to fix a uterine prolapse are usually successful. In most patients the surgeries for POP have been reported to have an 80% – 95% success rate.

 

The success rate for sacrospinous ligament fixation (surgery to correct the bulging of the vagina) ranges between 80% – 90%, for uterosacral ligament suspension (surgery restoring support to the top of the vagina) is also 80%-90% successful, and sacrocolpopexy (surgery fixing the vaginal vault and the cervix) and sacrohysteropexy (fixing the prolapse of the uterus) have also shown the same success rate.

 

However, even after prolapse surgery, there is still a chance that another part of the vagina or uterus may prolapse.

 

What Can I Expect After POP Surgery?

 

These are the general guidelines that doctors will usually give post POP OP (this is not medical advice): Each day in recovery you can expect to begin to feel stronger. During your recovery, you may need to remain on pain medication for about a week or two and may experience fatigue and weakness. A typical recovery time line varies from 4 to 6 weeks until you recover from an open surgery and anywhere from 1 to 2 weeks to recover from vaginal surgery. Your doctor should give you more specific guidelines.

 

During your recovery period, you should avoid any sexual activity, vigorous exercise, heavy lifting, or any activities that may put pressure or strain your operation area. Your doctor should give you more specific guidelines. The most important aspect to your quick recovery is rest – getting enough sleep will help you recover.

 

During your recovery, you might experience vaginal discharge or light bleeding. If so, avoid using tampons or douche. Use a sanitary pad if necessary. Your doctor should give you more specific guidelines.

 

How long do you stay in the hospital after prolapse surgery?

 

After your prolapse surgery, it is likely that you will need to remain hospitalized for 2 – 3 days depending on the operation and any underlying medical conditions you have. During this time your medical care takers will help ensure your recovery by monitoring for any potential complications and providing clean, sanitized treatment of the operation site.

 

Are There Complications with Prolapse Surgery?

 

Although pelvic surgery has proven to be a very safe procedure, as with any surgery, there are a few risks and potential complications that can arise. Please discuss this with your doctor but the common ones include:

 

Bleeding

 

Infection

 

Tissue or organ injury

 

Hernia

 

Conversion to open surgery

 

Urinary incontinence

 

Urinary retention

 

Vesicovaginal fistula (rare, abnormal connection between vagina and bladder)

 

If you are experiencing any of these symptoms of complications, it is important that you seek medical assistance. In some cases these symptoms can be common and unserious, but it is always best to consult your doctor if you have any concerns.

 

Can pelvic floor muscles be repaired?

 

If you have experienced a prolapse, uterine or otherwise, it can be a shocking and painful experience. However, there are ways to decrease symptoms without surgery.

 

Nonsurgical Treatments

 

There are several ways in which to treat a uterine or vaginal prolapse. The three most common approaches are pessary, restorative exercises, and biofeedback therapy.

 

Pessary

 

A pessary is probably one of the first treatments your doctor will recommend if you have symptoms of POP. A pessary is a ring shaped device that is inserted into your vagina. This will help support your pelvic organs. Being fitted for a pessary is a lot like being fitted for a diaphragm. Many of my clients with severe POP find that a pessary is useful for allowing them to live fuller lives while they slowly use PT and exercise to help resolve the prolapse.

 

Restorative Exercises & Physical Therapy

 

Pelvic floor exercises help strengthen your pelvic muscles. Although the most common exercise recommended by health professionals today is the kegel exercise, there are many more exercises that you can do that are helpful for restoring PF function and giving support for your pelvic organs. Kegels are a pretty outdated model of care when used alone, without a more whole body approach to healing.

 

What is involved in pelvic floor reconstruction?

 

Pelvic floor reconstruction surgery is a group of procedures that are performed to treat prolapse of uterus, rectum, small intestines, or other pelvic organs.

 

Anterior Vaginal Repair

 

Anterior repair is a surgical procedure that seeks to reinforce weakened layers between the bladder and the vagina. This surgery helps relieve symptoms such as vaginal bulging and to improve bladder function.

 

The surgery can be performed under general anaesthetic. The most common method of anterior repair is by an incision made along the centre of the front wall of the vagina – starting at the entrance and ending at the top of the vagina. The weakened layers are then repaired and anu superfluous tissue is removed. This is typically done with absorbable stitches.

 

Posterior Vaginal Repair

 

A back wall (posterior) vaginal prolapse is usually caused by weakness in the tissues and muscles that divide the vagina from the lower part of the bowel. This surgical procedure is performed to order to repair or restore the weakened layers between the rectum and the vagina.

 

As with an anterior vaginal repair surgery, posterior repair surgery is usually performed under general anesthetic. The procedure involves a minor incision being made along the back wall of the vagina starting at the entrance and finishing near the top. The weakened layers are then repaired using absorbable stitches.

 

Vaginal Hysterectomy

 

A vaginal hysterectomy is typically performed with a prolapsed uterus. This is when the uterus (womb) descends into the vagina and in some severe cases, protrudes or extends outside of the vaginal canal. The surgery is performed by removing the uterus through the vagina.

 

A vaginal hysterectomy is performed under general or spinal anaesthetic. In order to remove the uterus, the surgeon will make an incision at the top of the vagina around the cervix, clears the bowel and bladder from the uterus, and cuts through the connecting tissues to remove the uterus. The surgeon will then close the vaginal vault and commonly adds additional supporting stitches as the surgery is being performed.

This involves contracting the lower abdominal muscles while practicing dynamic and static exercises. If done correctly, it can limit the stress placed on the interior organs during activities which increase intra-abdominal pressure (i.e. squats).

 

PFM Strengthening:

 

PFM strengthening exercises are a great way to help get your body back to its normal function and to heal from POP surgery. I advise getting instruction from a pelvic floor specialist who is trained in assessing the whole person to determine the appropriate exercise routines for you.

A woman’s vagina is separated from her rectum by ligaments and tissues known as the rectovaginal septum. The tissues that line this wall are called fascia. At times, due to compromised pelvic health or pelvic injuries, a woman may experience a weakness of the vaginal wall and rectovaginal septum. When this occurs, a part of the rectum can bulge and protrude into the vagina. This is called a rectocele prolapse.

 

What is Rectocele?

 

Rectocele is a condition which causes the supportive tissues between the rectum and the vagina to weaken. This can occur due to excess pressure in the rectum, excess vaginal pressure, or excess intra-abdominal pressure leading to pelvic floor issues such as pelvic organ prolapse (POP). When this occurs, the separating tissues can herniate causing the front wall of the rectum to bulge into the vagina. This issue is also commonly called a posterior vaginal prolapse. Oftentimes, the prolapse is small and does not pose any painful symptoms or difficulties. However, in severe cases, the bulge may manifest outside of the vaginal opening and require surgical repair.

 

Signs and Symptoms of Rectocele

 

In many cases, someone may have a rectocele for a long period of time without noticing any symptoms.

 

In minor to moderate cases of symptomatic rectocele, someone may experience feelings of mild discomfort, pressure, or minor pain in the vaginal or rectal regions. This may be accompanied by a feeling of incompletion after bowel movements.

 

In moderate to severe cases, women with a rectocele may have an increased sensation of pain and discomfort in their vagina, rectum, or abdomen while pooping. This can be a result of the feces being pushed into the rectocele during a bowel movement. Women with moderate rectocele run a higher risk of constipation, painful intercourse, and lower abdominal and lower back pain. In severe cases, the rectocele can prolapse causing the tissue to protrude out of the vaginal opening.

 

Other symptoms of rectocele may include:

 

A small bulge in the wall of the vagina

 

Needing assistance of fingers to perform a bowel movement

 

Pressure in rectum

 

A feeling like an “air bubble” in the vagina

 

Pain, discomfort, or feelings of looseness during vaginal intercourse

 

Other pelvic organ prolapse issues such as vaginal prolapse, uterine prolapse, rectal prolapse, or other issues with the pelvic floor.

 

Rectocele is a common condition many women may face without ever knowing it. Most often it is associated with postpartum issues, but it can affect women who have never been pregnant as well.

 

If you are experiencing severe issues such as: tissue bulging through your vaginal opening or constant struggles with constipation, it may be time to consult your doctor for a proper diagnosis. Generally a Urogyn is the best bet for diagnosis.

 

What Causes Rectocele?

 

There are many reasons why someone may have pelvic floor related issues. Most commonly the weakening of the pelvic region, the vaginal and rectal tissues, and core strength relate to pregnancy and difficulties in vaginal childbirth – especially birthing multiple children. Women who required equipment assistance during their delivery (suction, forceps, etc) may run a higher risk of developing a rectocele than women who did not. Large babies and longer labor periods can also contribute to weakened pelvic floor muscles.

 

Some other possible causes for rectocele include:

 

Recurrent, chronic constipation

 

Chronic cough

 

Improper heavy lifting techniques

 

Obesity

 

Menopause

 

Age

 

Women going through menopause go through a host of changes and significant hormonal shifts that can affect the pelvic floor muscles. Aging can also affect durability and strength of muscles over time, especially in the pelvic region. Excess pressure in the abdomen like constant abdominal tension, excess abdominal tone, poor core strategy, poor exercise strategy can also contribute to pelvic floor weakness.

 

What happens if Rectocele Goes Untreated?

 

Rectocele is usually not a very serious condition and may go unnoticed for years. However, if someone is experiencing symptomatic rectocele and leaves it untreated, they might suffer a host of painful symptoms. A few risk factors of untreated, symptomatic rectocele include: chronic constipation, vaginal bleeding unassociated with menstrual cycles, prolapse the rectal wall into the back wall of the vagina, and rectocele repair surgery. It is important to consult your doctor if you are experiencing any of the symptoms listed above.

 

Can a Rectocele Go Away on its Own?

 

Unfortunately, it is ot thought that a rectocele will heal on its own. Although it can go asymptomatic for years, it will still remain there unless treated in some way. If ignored, a rectocele can become larger and present more problems over time. During a regular health exam, your doctor may notice the presence of a rectocele. If so, there are many ways you can pursue treatment without the need of surgery.

 

How do you Fix a Rectocele?

 

Fixing a rectocele can be done in many ways.

 

Diet & Exercise

 

Non-surgical treatment depends entirely on the severity of your rectocele. In most cases, rectocele can be treated through:

 

Exercise: Your doctor will most likely recommend performing daily exercise routines or pelvic floor therapy in order to strengthen and heal your pelvic floor. Exercises for POP are designed to heal many forms of pelvic organ prolapse and can greatly aid in your rectocele treatment.

 

Dieting: Dieting can also significantly help with any digestive issues that might be contributing to your rectocele. A high fiber diet and ensuring you are consistently well hydrated can help reduce your risk of constipation and straining during bowel movements.

 

Vaginal Pessary: Your doctor may recommend a vaginal pessary to help support the bulging tissues in your vagina. A pessary is a small rubber or plastic ring used for vaginal health.

 

Surgery

 

At times, exercise based treatment is not enough to treat rectocele. Surgical repair may be considered if you are experiencing: a prolapse which protrudes outside of your vagina, or additional pelvic floor issues that may be increasing the painful symptoms. In this case, rectocele repair surgery might be advised and any co-occurring issues can be repaired at once. In many cases, it is a minimally invasive surgery that removes any excess tissue along the bulge and stitches the area up for support.

Believe it or not, diastasis recti can affect men just as it can affect women. One of the biggest myths surrounding diastasis recti is that it only occurs in women postpartum. However, diastasis recti is a universal occurrence that affects the abdominal wall without gender specificity.

 

What is Diastasis Recti?

 

Diastasis recti occurs when excess pressure is exerted on the abdominal muscles. This excess pressure can cause them to separate – creating a “gap” in the center of the core. Diastasis recti is also associated with the stretching and thinning of the linea alba (the connective tissue that lies underneath the abs). This tissue binds the vertical muscles on each side of your belly button (the abdominis recti or 6-pack muscles). This occurrence leaves a gap between the vertical muscles and leaves the abdominals unsupported, which can lead to a host of issues.

 

In men and women, this gap can be created in the midline of your belly anywhere from the pubic bone to the base of your ribcage. During a crunch or sit-up, where one would normally feel tension and closure, there is a space in between.

 

Symptoms

 

The symptoms of a diastasis recti in men don’t vary much from women. The hallmark sign of a diastasis recti in many men is the vertical bulge that appears between the abdominal walls when performing a crunch or a sit-up. Yet, some other distinguishable symptoms include:

 

Constipation

 

Lower back pain

 

Urinary incontinence

 

Difficulty breathing

 

Discomfort in bodily movements

 

Weakened core

 

Sensation of a weak core

 

*Note: not all of these symptoms are directly related to a diastasis recti. If you are experiencing many or most of these symptoms it could be a sign of another underlying illness. If you are concerned, pay your doctor a visit to determine what else may be going on.

 

It is also important to know that not everyone will experience all of these symptoms. It is common for people to experience only one or a few of these symptoms. A diagnosis from a specialist is important.

 

Diastasis Recti Test

 

If you think you have a diastasis recti, you can perform a self-test at home or have a physical therapist assess you.

 

A self-assessment can be performed as follows:

 

Lie on your back in a comfortable position. Bend your knees and place your feet flat on the floor.

 

Place one hand on your belly with your fingers pointing straight down on your core.

 

If you need support for your head, place your other hand under your head and neck for support. Slowly lift your head and add minimal pressure to your fingers placed on your core. With no diastasis recti, there is the feeling of a toned wall as you lift your head up. If you feel a gap, or your fingers sink into your core, you likely have diastasis recti. In very obvious cases, you can feel the sides of your core muscles in between that gap on the left and right sides.

 

Repeat the process for the areas just above your belly button and below your belly button to determine whether or not the diastasis recti is isolated or in your core as a whole.

 

After performing a self-assessment, there are a few things to keep in mind. A gap that is only one or two finger widths wide is not a big concern, as long as it is shallow – but caution is recommended. However, if you discover that you have a gap ranging from 2.7 cms or larger (2.5 fingers or larger) you should consult a PT or OT. Regardless of the size of the gap, if there is a gap, it is in your best interest to consider core restoration rehab and strength building exercises to help close the gap and prevent it from widening further.

 

What Causes a Diastasis Recti in Men?

 

While a diastasis recti is usually caused by pregnancy in women, research has helped reveal several causes of the condition in men. Some of the common reasons for a diastasis recti to develop are:

 

Frequent or rapid changes in weight

 

Weightlifting

 

Certain core exercises

 

Underlying Abdominal Illnesses (i.e. stomach cancer, or cirrhosis)

 

Obesity

 

The most common cause is improper exercise and weight-lifting, especially when it comes to abdominal exercises like: sit-ups, crunches, push-ups, and front planks. These exercises are not harmful if approached intelligently. However, making compromises or taking cheats can greatly affect your physical well-being and lead to an injury like diastasis recti.

 

In addition to this, excess weight and obesity can be a leading cause of diastasis recti in men. Excess weight can place undue stress on the abdominal muscles and cause them to separate and weaken. However, it is important to note that this is not limited to excess body fat. Increased levels of weight gain or weight loss due to muscular expansion or can also go hand in hand with a diastasis recti. That is why diastasis recti is most commonly seen in heavy-weight bodybuilders.

 

Finally, age and certain medical conditions can lead to a diastasis recti. Conditions such as cirrhosis of the liver and abdominal wall cancer could also be culprits of diastasis recti in men.

 

How then can we properly approach exercise and strength training without causing injury while promoting strength, functionality, and health?

 

Core Restoration Through Effective Training

 

The best approach to healing a diastasis recti is core training versus a standard exercise routine. How we hold our bodies, how we stand, whether we have a tense, tight belly, our exercise regimens, and our breathing patterns all play part in core functionality or lead to a system fault. The biggest issue exercise is how often we compensate. Not because it’s a fault of ours, but because our body is intelligent enough to implement other bodily systems when another one fails.

 

Arbitrary sit-ups, crunches, and planks will not fix a core system failure. We need to train our core, not just exercise our core. In training, we are teaching our core to do its job, to manage load (weights and movement), our weight, and endurance. Being good at exercise does not properly train your core to be resilient, reflexive, and responsive. With a diastasis recti, the body has lost its innate support system it once had. However, it is not gone forever, but clearing the fog surrounding exercise and health is the primary goal to healing. It is what I work on daily in my Restore Your Core Program.

 

Some key ground rules when working to strengthen your care via effective core training are:

 

Do not bulge your abs when doing core exercises!

 

Do not brace (do not overreact or compensate during core exercises)

 

Do not bear down when doing core exercises. Bearing down is a movement similar to making a bowel movement. Do not add additional pressure to your abdomen.

 

Never hold your breath while doing core exercises!

Functionality Exercises for Diastasis Recti

 

Restoring your core is all about gaining functionality back. The exercises below are great examples of how to help build strength beginning from a basic level.

 

Candles:

 

Candles is a breathing technique that helps with ab contraction. This exercise can be done while in any position. Inhale normally and on your exhale, release your breath slowly (i.e. hiss, soft blow, between teeth) as if you were blowing out a lot of candles. If done properly, you should feel your abdominal muscles contract/respond.

 

Tabletop or Reverse Marching:

 

Lie down similarly to that of a diastasis test. On each exhale, practice candles. As you exhale, alternate bringing your knee, while still bent, toward your body.

 

Goddess Side Bend:

 

While standing with wide feet placement (knees aligned with feet), begin candles exhale and bend your knees slowly until they are level with your ankles. Stabilize your body by pressing your heels into the floor. Side bend by raising your arm while maintaining resistance. Repeat side rotations 3x and then return to a standing position.

 

Goddess Squat Twist:

 

Repeat steps outlined above but have your arms stacked across your chest. While in the goddess position, twist by using your ribcage and not your arms. Repeat side twists 4x and return to a standing position

 

Lunge with a Twist and Chop:

 

Practice lunging. As you bring one leg forward–all while maintaining correct posture–lift your hands, palms crossed, in front of your face. Then slowly begin to twist toward your forward leg. As you twist toward your forward leg, bring your arms across your thigh in a chopping motion.

Exercises to Avoid

 

There are several everyday exercises and movements that those with a diastasis recti should avoid. You want to avoid any exercises that put extra stress on the midline. These can stretch or expand the abdominal wall and can lead to the condition worsening.

 

Avoid doing the following:

 

Yoga poses or stretching routines that stretch the abs

 

Most crunches

 

Full push-ups

 

Any exercises that cause your abdominal wall to bulge

 

Remember that healing will look different for each person. The time it takes to heal depends on the severity of the injury to the linea alba and connective tissues. Yet, you should be hopeful knowing it takes time! For some people it can take years to properly heal and to restore full function, but healing is possible.

Atropa belladonna or Atropa bella-donna, commonly known as Belladonna, Devil's Berries, Death Cherries or Deadly Nightshade, is a perennial herbaceous plant in the family Solanaceae, native to Europe, North Africa, and Western Asia. The foliage and berries are extremely toxic, containing tropane alkaloids. These toxins include scopolamine and hyoscyamine which cause a bizarre delirium and hallucinations, and are also used as pharmaceutical anticholinergics. The drug atropine is derived from the plant.

 

It has a long history of use as a medicine, cosmetic, and poison. Before the Middle Ages, it was used as an anesthetic for surgery, the ancient Romans used it as a poison (the wife of Emperor Augustus and the wife of Claudius both used it to murder contemporaries) and predating this it was used to make poison tipped arrows. The genus name "atropa" comes from Atropos, one of the three Fates in Greek mythology, and the name "bella donna" is derived from Italian and means "beautiful woman".

 

Belladonna is one of the most toxic plants found in the Western hemisphere. All parts of the plant contain tropane alkaloids. The berries pose the greatest danger to children because they look attractive and have a somewhat sweet taste. The consumption of two to five berries by children and ten to twenty berries by adults can be lethal. The root of the plant is generally the most toxic part, though this can vary from one specimen to another. Ingestion of a single leaf of the plant can be fatal to an adult.

 

The active agents in Belladonna, atropine, hyoscine (scopolamine), and hyoscyamine, have anticholinergic properties. The symptoms of belladonna poisoning include dilated pupils, sensitivity to light, blurred vision, tachycardia, loss of balance, staggering, headache, rash, flushing, dry mouth and throat, slurred speech, urinary retention, constipation, confusion, hallucinations, delirium, and convulsions. In 2009, atropa belladonna that was mistaken for blueberries with six berries ingested by an adult woman was documented to result in severe anticholinergic syndrome. The plant's deadly symptoms are caused by atropine's disruption of the parasympathetic nervous system's ability to regulate involuntary activities such as sweating, breathing, and heart rate. The antidote for belladonna poisoning is physostigmine or pilocarpine, the same as for atropine.

 

Atropa belladonna is also toxic to many domestic animals, causing narcosis and paralysis. However, cattle and rabbits eat the plant seemingly without suffering harmful effects. In humans its anticholinergic properties will cause the disruption of cognitive capacities like memory and learning.

Pelvic floor physiotherapists help women rehabilitate their pelvic floor region. The muscle groups within the core and pelvic region can be weakened by childbirth, surgery, genetics, heavy lifting, rapid weight gain, constipation, menopause, improper breathing mechanics, and more. Banana is the common name for a type of fruit and also the herbaceous plants of the genus Musa which produce this commonly eaten fruit. They are native to the tropical region of Southeast Asia. Today, they are cultivated throughout the tropics.

By a high potassium to sodium content, bananas may prevent high blood pressure and its complications. High fiber content may also contribute to this effect. High potassium may also prevent renal calcium loss, in effect preventing bone breakdown. In diarrhea, it contributes with electrolyte replacement, as well as increased absorption of nutrients. Bananas also have some antacid effect, protecting from peptic ulcers. Pectin content, a hydrocolloid, can ease constipation by normalizing movement through the intestine. The low glycemic index in unripe bananas is of particular benefit to people with diabetes. High fructooligosaccharide content may work as a prebiotic, nourishing the intestinal flora to produce beneficial vitamins and enzymes. Carotenoid content has antioxidant effects, and protects against vitamin A deficiency, resulting in e.g. night blindness. Moderate consumption decreases risk of kidney cancer, possibly due to antioxidant phenolic compounds. In contrast, large consumption of highly processed fruit juice increases the risk of kidney cancer. In a 2001 study (Hamilton & Jensen) it was established that all bananas contain the same number of calories. The study determined that the calorific density varied depending on the size of banana to keep the calorific value at a constant 163 Kcal.

Under the "Lome Convention" the EU used to agree to buy 8% of it's bananas from former colonies in the Caribbean. These banana sales are crucial to the livelihoods of 200,000 people in countries where unemployment rates are 30-50%.

 

The Chiquita Company is a U.S. company that owns banana plantations in Colombia, Costa Rica, Honduras and Panama where thousands of underpaid workers are exposed to dangerous pesticides and unions are banned. Chiquita supplies 50% of the EU's banana imports each year, but wants an even larger market share. Radish juice taken in the morning and night. Start with 1/4 a cup and increase to 1/2 a cup gradually over a period of 1 month.

Tip 3:

Wash 3-4 figs. Soak in a glass of water over night. Eat them first thing in the morning. Again repeat in the night.

Tip 4: Boil 1 ripe banana in 1 cup of milk, mash well. Take 3-4 times a day.

Tip 5: Crush bitter gourd leaves and extract the juice. Mix 3 tsp in 1 glass of buttermilk, and drink on an empty stomach every morning, for a month.

Pelvic floor physiotherapists help women rehabilitate their pelvic floor region. The muscle groups within the core and pelvic region can be weakened by childbirth, surgery, genetics, heavy lifting, rapid weight gain, constipation, menopause, improper breathing mechanics, and more. These muscle groups aid in supporting the uterus, bladder, and bowels. They form a slinglike grouping from the pubic bone to the front of the tailbone at the back. Damaged or weakened pelvic floors can affect bladder and bowel control, leading to urinary and rectal incontinence or even pelvic organ prolapse.

 

At times, the pelvic floor can become overactive or hypertonic. This means that their pelvic floor muscles are overly tight and tense all of the time, even when they should be relaxed. Learning to relax and release the pelvic floor muscles (muscles contractions like the short contraction of the bulging of a bicep or long contraction, like the slow, contained stretching out of a bicep) will help ease overactivity. An overactive pelvic floor can cause many symptoms such as: back pain, pain during sex, heaviness or incontinence.

 

What is a pelvic floor specialist?

 

Doctors who specialize in pelvic floor dysfunction issues are called urogynecologists and physiotherapists. A urogynecologist will care for women with pelvic floor disorders by providing services that help evaluate pelvic floor health and provide primarily surgical treatments for pelvic organ prolapse. Yoga believes that, first and foremost, one should know one’s own body and its particular requirements. Each part of the body has its own importance. Navel is a very important part among them. It is considered to be the centre of all the organs of the body. There are 72000 nerves are spread all over the body. It is normally pulled by some tissues, which keep it in its place if for any reason these tissues get pulled then the navel gets displaced.

If the navel is displaced upward, then diseases like constipation, heart palpitation anxiety and stress. And if the navel is displaced downward then problems like loose motion, night fall, indigestion, week-ness etc. if it is displaced to side then there is shooting stomach aches. Take a deep breath and raise your head trunk and legs above the round in order to left legs, pull hands and legs in opposite, direction support the entire body on the floor. Hold the position for as long as is comfortable and then slowly relaxing the leg muscles lower the legs, chest and head to the starting position. Do it minimum 3 times daily.

Benefits: This asana helps to improve digestion by stimulating gastric secretions. The liver abdominal organ and muscles are massaged. This asana is recommended for the management of diabetes, menstrual disorders and neck pain. Caution: Patients of colitis, hernia and slipped disc should avoid this asana. Heart patients and hyper tension patients should avoid this asana also.

2 Chakraasana (wheel pose): Lie flat on your back with legs straight on the floor. Bend your legs from the knees and with the help of the hands, bring the heels as close to the hips as possible slowly raise the body and arch the back, allowing the crown on the head to support the weight of the upper body. Putting pressure on the palms, raise your body from the floor arching the back straight then the knees further by moving the trunk towards the head let the head hang hand between the straight arms. Hold the final position for as long as is comfortable slowly lower the body so the head rests on the floor and then lower the rest of the body. This is one round repeat 3 times.

Benefits: This asana tones up the nervous, digestive, respiratory, cardiovascular and glandular systems. This asana keep dislodged navel at its right position.

Caution: while lifting the body leave the neck loose or else there is fear of a sprain in the neck. People with any illness, during pregnancy and weak wrists should not practice this asana

3 Matsyasana (Fish Pose): This posture is dedicated to Matsya. Matsya means a fish. Lie flat on you back keeping the legs straight on the floor with feet together. Bend the legs and do padmasana in the lying position. Carefully bend back ward, supporting the body with the arms and elbows. Lift the chest slightly, take the head back and lower the crown of the head to the floor. Hold the big toes and rest the elbows on the floor. Adjust the position of head and hands so that the back is arched to the maximum. Relax the arms and the whole body, allowing the head, buttocks and legs to support the weight of the body close the eyes and breathe slowly and deeply. Stay in this posture as long as you feel comfortable. Return to starting position, reversing the order of movements. Do it one time only.

Benefits: This asana stretches the intestines and abdominal organs and is useful for all abdominal ailments. The dorsal region is fully extended in this posture and the chest is well expanded. Practice of this asana is very good for asthma and bronchitis as it encourages deep respiration. The asana is very useful for the treatment of constipation, tonsillitis, neck pain and disorders of reproduction system

Caution: People who suffer from heart disease, hernia, ulcers, H.B.P, Acute neck pain and pregnant women should not practice this asana.

Pelvic floor therapy is recommended for conditions where the pelvic floor and core system is not functioning optimally. Pelvic Floor Dysfunction and its related conditions can be caused by many different things. These can include:

 

Infections

 

Pregnancy or childbirth

 

Poor posture

 

Trauma

 

Chronic back pain

 

Surgery

 

However, PFD can also seem to have no cause and present itself with a host of painful symptoms. In some women, the cause of PFD can be a result of postpartum diastasis recti. The pelvic floor is made up of muscles and other tissues which form a sling from the pubic symphysis to the tailbone. Many cases of PFD stem from a lack of sufficient support from the pelvic floor. These pelvic floor muscles assist in maintaining correct posture, abdominal and pelvic organ support, and aid in bladder and bowel control as well as sexual activity. If these muscles become overactive or overused (hypertonic), the results can be quite painful and function can decrease significantly. Less often, the pelvic floor muscles are hypotonic–lacking sufficient resting tension to perform their jobs. Yet, due to the complexity of the anatomy and functions of the pelvic region, the underlying cause of pain can be difficult to determine. In this case, the whole body must be treated and physical therapy including pelvic floor exercise can greatly aid in men and women in their healing process and recovery.

 

Why do I need pelvic floor therapy?

 

(POP) or pelvic organ prolapse is a type of pelvic floor dysfunction in which one or more pelvic floor organs (i.e. bladder, rectum, small bowel, uterus, etc) shift toward or down into the vaginal canal. This most commonly happens with conditions like diastasis recti, which create an imbalance of muscle and ligament tension supporting the pelvic floor; many people who have POP also have a DRA. Women who experience a pelvic organ prolapse sometimes describe the occurrence as feeling like a “stuck tampon,” a heavy pelvic floor, or as bubbles in the urethra. Some other symptoms present may include:

 

constipation

 

pelvic pain during sex

 

urinary/fecal incontinence

 

Most fitness gurus try to educate their clients with core exercise routines that engage the pelvic floor and the core together. They believe that if you engage your core in any activity, you should also engage your pelvic floor. However, I believe there is a lot wrong with this routine and practice.

 

As a trained and educated professional, I never, ever attempt to teach pelvic floor muscles to engage in exercise. Rather, it is important to train the pelvic floor to lift and release appropriately depending on the exercise and the weight. This is what I teach in my 13-week program, Restore Your Core.

 

What do they do in pelvic floor physical therapy?

 

In physical therapy for pelvic floor issues therapeutic treatments may vary. An initial assessment will most likely be made by your physical therapist. This can include a manual palpation either vaginally or rectally. Palpitation of the vaginal wall helps physical therapists determine muscles tightness, areas of tenderness and pain, and muscle strength.

 

Getting to Know Your Pelvic Floor

 

Imbalances of the PF muscles – can be difficult to feel and perceive. One of the most unhelpful tips that pelvic floor exercisers tend to give is the misguided and incessant demand for exercising kegels. Kegels can prevent treatment from happening because it makes everything tighter and even harder to feel. While Kegel exercises performed correctly can be useful for toning the pelvic floor muscles, in the case of hypertonic muscles, you’re adding tension to tension, and not actually increasing the responsiveness of the muscles. As a result, doing 100 Kegels a day may actually make your symptoms worse; increasing the tension in the pelvic floor muscles can also make it harder to perceive the movement of the pelvic floor.

 

One of the key elements of the healing process is awareness. When you are aware of the structure of your body and whether or not it is performing correctly, you can begin resolving your problems. If you are blind to it, you may continue to practice bad habits due to motor programming. However, if we want to shift things around in order to eliminate physical pain and encourage realignment, we must shift our habits as well.

 

In the case of POP or other pelvic floor issues, there can be a lot of confusion. Some of the most common questions I get asked are:

 

Is my pelvic floor too tight or not tight enough?

 

How can I tell if I’m practicing resting tension?

 

How can I fix this?

 

Before we can begin resolving any present issues, we have to get to know our pelvic floor a bit.

 

We must understand what it means to engage the PF, what it feels like to release and contract, and how to both control contractions and release. Only then can we properly discern what unhealthy tendencies we may fall into. At first, many women find that they have little to no control over their muscles. Over time and with practice, this often changes.

 

Reduce Intra Abdominal Pressure: Breathing Mechanics Matter

 

One of the key pieces in resolving pelvic floor dysfunction is breathing mechanics. In Restore Your Core, I spend a great deal of time with my clients teaching them proper breathing mechanics. How you breathe has a significant impact on the integrity of your core and pelvic floor. However, many of us do not even realize how easy it is for us to fall into incorrect breathing patterns.

 

Many people are belly breathers. This means that every time they inhale their bellies inflate. Think of it this way: if you were to fill an oval-shaped balloon with water and then squeeze the top, what would happen? It would bulge! The same thing happens when we belly breathe. When the bulge happens, you’re straining your core and pelvic floor with an excessive amount of pressure. This can damage the muscles and organs in that region. Our PF is not designed to handle a great deal of stress and pressure.

 

What I teach my clients instead is a technique called 3D breathing. This breathing method engages the rib cage and the diaphragm. When you inhale, your ribs expand instead of your belly. Breathing this way reduces excessive intraabdominal pressure.In fact, this technique promotes proper core engagement and response in your daily activities. Reduce Intra Abdominal Pressure: Alignment Matters

 

Daily movement habits and activities can greatly affect the health of our pelvic floor as well as our joints, muscles, and bones. When I work with my clients with POP or PFD, I always want to ensure their pelvic alignment is optimal. Without even realizing it, many people end up standing and pushing their hips forward. This shift can greatly affect the effectiveness of the core muscles. Treatment for pelvic floor restoration should include educated instruction on correct posture and physical therapy exercises.

 

How you stand can greatly affect your body over time. Muscles holding your alignment in place can shift and move to accommodate habitual stances and movements. To stand correctly, stand up straight and tall. Focus your weight on the balls of your heels and level your feet with your shoulders, knees bent. Next, relax your arms at your side. Keep your head straight and level (earlobes aligned with shoulders) and pull your shoulders back. If you’re standing for long periods of time, shift your weight from your toes to your heels.
