Mit Beginn des 10 Jhd. gelangte die Anlage in den Besitz des Vatikans. Die folgenden Päpste bauten das Grabmal dann zur Flucht Burg um.
Zwischen der Burg und dem Vatikan gibt es eine verborgenen oberirdischen Fluchtweg den unter andern Papst Klemens VII nutzte, als die Truppen Kaiser Karl V Rom 1527 plünderten. (Sacco di Roma)
Es wurde unter anderem die folgenden Kaiser dort begraben: Marcus Aurelius Antonius Bassianus (Caracalla), Mark Aurel, Hadria mit seiner Frau, Antonius Pius und seine Frau Faustina.
