I had to tilt my head way back for it to show up, but the bump on my neck is why I am in Bahrain. The ship does not have the appropriate diagnostic equipment onboard, so they sent me here. The best guess is thyroid. But beside the bump, I am completely asymptomatic. While that is good, it makes is harder to figure out what is going on.
Middle-aged female. Patient noted neck mass a week earlier. Thyroid FNA showed highly cellular specimen with anaplastic features. She was otherwise feeling well at time of procedure but that night returned to the hospital via the emergency room with difficulty breathing. Imaging showed multiple lung masses and bilateral adrenal masses. Lung FNA was identical to the thyroid.
This is the lung core biopsy. The tumor is strongly positive for PAX-8, pankeratin (AE1-AE3), and vimentin. It is negative for SOX-10, thyroglobulin, calcitonin, and synaptophysin. EMA is weakly, focally positive.
Diagnosis: Undifferentiated carcinoma, consistent with metastasis from thyroid primary.

KYSTE DU TRACTUS THYREOGLOSSE
Définition:
Reliquats du tractus thyréoglosse (descente de la thyroïde), 50% avant à l'age de 10 ans, 65% avant 20 ans
Étiologie:
Absence de fermeture du canal thyréoglosse au moment de la descente de la thyroïde dans la 4ème semaine de la vie fœtale
Signes cliniques :
masse cervicale médiane, rénitente, élastique, sous ou en avant de l'os hyoïde, mobile à la déglutition , taille variable, sensation de masse , parfois fistule cutanée. le diagnostique est fait par interrogatoire, examen clinique, imagerie .
Diagnostic différentiel :
1. Thyroïde ectopique
2. Kyste dermoïde
3. Tumeur thyroïdienne
4. Adénopathie inflammatoire ou maligne
Examens complémentaires :
1. Biopsie du kyste : étude anatomie pathologique le kyste entouré d'un épithélium stratifié ou pseudo-stratifié avec des cellules ciliées. Présence de tissu thyroïdien dans 10 à 45% des kystes
2. Échographie cervicale
3. Scintigraphie thyroïdienne pour éliminer une ectopie
Traitement : Hospitalisation de courte durée d'environ 3 jours, ablation des fils au 8ème jour.
1. Seule la chirurgie est curatrice: la résection emporte le kyste, le corps de l'os hyoïde et le trajet fistuleux jusqu'à la base de langue (excision triangulaire).
2. Si le kyste est surinfecté, un traitement médical (antibiotiques, anti-inflammatoires) est d'abord institué. En cas de collection, un drainage est possible.
3. Antibiotiques si surinfection, puis chirurgie
Complications:
1. Surinfection
2. Fistulisation à la peau
3. Dégénérescence maligne: exceptionnelle

Hunter just had his bloodwork done and his thyroid is acting well on his meds. There is supposed to be a food coming out for thyroid control in cats. The vet couldn't tell me if it was just the dry or if ti woudl be canned as well. Mr Picky here probably won't like it so I'm not even going to try. He does good taking his pills in spreadable cheese.

Thyroid disease can range from a small goiter (enlarged gland) that does not require treatment to a life-threatening condition. Inappropriate production of thyroid hormones is the most frequently occurring thyroid problem.

Neko was treated for hyper-thyroidism, a condition where a tumour within the thyroid glad over-produces various hormones. This results in a variety of symptoms, from weight loss (see how skinny she is now compared to before - she lost over 2 kg), excessive moulting, constant thirst, over-eating, etc.
The treatment consists of Neko being injected with radioactive iodine, which is absorbed by the tumour and kills the affected cells (leaving the healthy ones alone - clever!)
So now I have a radioactive cat! (For about another week, anyway, before it dissipates).
I am looking for the first signs of mutant super-powers developing, but so far, no luck.

Last year, we had a huge scare when Abbie was diagnosed with thyroid cancer. We became concerned when she was not able to keep any weight on despite having a great appetite. Her vet run a blood test and discovered that she had a hyperactive thyroid gland. Many tests later, including an iodine scan, Abbie had surgery to remove the cancerous tumors. Her surgeon was confident that she had removed all of it but when the biopsy came back it showed the margins near zero. The recommendation was for chemo and radiation. Now for a little background….
Abbie is a rescue that we adopted when she was 10 months old. Her previous people could not handle her energy level (pointers NEED to fly!) so they locked her in a small cage and pushed food through to her. She was left there for weeks. Our kind neighbor noticed and asked me to help with the rescue. It was love at first sight. The toughest part was convincing the owners that Abbie would be better off with us. What a gift! She is the gentlest, kindest soul. Her early traumatic cage experience causes her to totally panic if she is left alone. Enter dog #2. (A story for another day!)
So… After the surgery they wanted to treat her with very strong radiation, requiring her to be in isolation for about 4 weeks. If we didn’t do it, they estimated she would not make it for a year. This is a dog who panics after 10 minutes and bloodies her paws trying to break out at the vets when undergoing routine tests. We knew the isolation would kill her before the cancer did, so we opted out of the treatment. Let her spend her last days on the beach chasing birds. But a miracle happened when we took her back for her annual checkup. She is now apparently cancer free, with many more years in front of her to make sure the birds on the beach get their exercise. Who says the good die young!
