Organization within an alveolar duct (upper right) and an alveolus (left). Other areas of this core biopsy specimen showed intra-alveolar fibrin masses.

Acute fibrinous and organizing pneumonia (AFOP) is a relatively rare form of acute or subacute lung injury. AFOP can be idiopathic or associated with a variety of underlying conditions including connective tissue diseases, drugs reactions, occupational exposures, immune system disorders, long-term steroid therapy, poorly controlled diabetes, alcoholism, lymphoma, and infections.

Typical microscopic findings are the presence of large fibrin balls or masses in airspaces and foci of organizing pneumonia. The number of fibrin balls may be variable throughout the lesion. Absence of hyaline membranes distinguishes AFOP from DAD.

Patients with AFOP may present with an acute illness; mortality in these cases is high. Those who present with a subacute illness appear to have a better prognosis.

Avitene (microfibrillar collagen hemostat)is absorbable collagen hemostatic material that is used in surgical procedures to accelerate clot formation. When ap plied to tissues Avitene induces a marked inflammatory reaction with a predominance of eosinophiles.

The alveolitis of sarcoidosis is characterized by T-helper lymphocyte infiltration of the alveolar walls and precedes granuloma formation. In this case granulomas were seen elsewhere in the specimen.
