I've mounted the Somewhat Defanged Camera Guts in a tupperwaresque box I got long ago (it contained a used Sega Genesis cartridge). The Guts is now powered with a D battery instead of an AA for a longer life. The watch will, of course, use something smaller - AA or AAA or short-AA, whatever seems good for size and lifespan. I also hope to run it with a rechargeable battery, but I'm not sure those will work.
The Guts throws out about 340 volts with no load, but the voltage drops off pretty sharply when you actually make it power something. Here, it's driving an IN-17 nixie with a 100K anode resistor. The total voltage ended up being about 225V, with about 1mA current going through the tube. It looks pretty good - 4 of these multiplexed ought to make a fine watch.
Further testing will include pulling all the non-essential parts from the Guts, including the ready light. LEDs take a large amount of current, by battery-powered circuit standards, so it might be stealing quite a bit of juice that could be going to the nixie. Then again, I might end up using it or something like it as a “colon” in the watch.
Now just to make some nice teeny circuitry to handle it all - like I said before, it might be a while before I get to it. Proof of concept, though? Check!

more than just blink. I added bits for a connector and regulating the power from 5v for the Atmega168 to 3.3v for LCD, accelerometer, etc.
I messed up the 3.3v voltage regulator and took a few tries bending the pins different ways before getting to work.

This is a little switchmode power supply that takes 9-12 volts DC and kicks out somewhere between 100 to 200 volts DC. It's intended for powering nixie tubes, and a kit version is sold by LEDSales in Australia, who I assume also developed it. They were thoughtful enough to include the circuit parts list and diagram in the kit assembly instructions (pdf). I wanted a version that used surface-mount parts, so I found surface-mount equivalents to all the listed kit parts and designed a board around them.
LEDSales' board is the one used in this nifty clock by Sascha Grant.
My board here is a proof of concept to see if the surface-mount parts I chose actually work. Despite using smaller parts it's bigger in area than LEDSales' board, though lower in profile - sort of; on the other side of the board there are a few largish components. I bet with careful routing I can squash it down to a very small size, like not much bigger than a square inch.
Happily, the concept is at least partly proven. Given 12V from a wall-wart it sits there and throws out 180V with no complaints. I haven't tried loading it with anything more than a single NE2 bulb, so we'll see how it does under pressure. Its voltage tends to drift downward, so there may be something wrong with the feedback part of the circuit, though it looks like it stabilizes after a while.

The PCBs for my nixie tube clock arrived, and I even got twice as many as I ordered! Hurrah for Gold Phoenix.
At any rate after a marathon soldering session they are finally assembled and awaiting testing and programming and all that. The power supply works, and the AVR was programmable, so that's a good sign!

Testing the entire power supply assembly after hooking it all up. "It all" is the power entry module, 500 mA resettable fuse, main power switch (in my hand there), and adjustable dual DC voltage regulator (the big green circuit board). The potentiometer hanging off the left of the circuit board adjusts the negative DC voltage. There's another one hanging off the right, out of the picture, for the positive side. The LEDs on the mini breadboard are dummy loads, which are required for the voltage regulators to be adjusted properly. IT ALL WORKS. The main switch even lights up! Hooray!
