A large unilateral pleural effusion such as this has three major likely causes: (1) trauma; (2) severe pneumonia; and (3) malignancy. The first two should be clinically unambiguous. This patient has a malignant effusion from metastatic breast carcinoma. Note the right mastectomy and axillary dissection.
