‪I embroidered a little hybrid physician from a 15th century Book of Hours (Bodleian MS Douce 62, f.113r). He’s holding a urine flask to the light to study it. The study of urine, uroscopy, was a vital diagnostic tool and “urine wheels” are often found in medical manuscripts.‬

In eastern Upper Canada, before Confederation, there were at least 65 licensed physicians, most of whom were Canadian graduates of medical schools at McGill or Queen's.

 

These physicians made a good living even though they seldom collected all their fees. For a working-class family, a single visit by a doctor represented a day's wage. Hence, one only called for the local doctor in extreme need. Otherwise, self-medication was the norm. The local store sold many inexpensive and harmless patent medicines and anxious mothers often consulted either a trusted neighbour or home medicine book.

 

Most general practitioners, however, were kept busy travelling around the countryside seeing sick people in their own homes. Aside from delivering babies, they treated various ailments by means of common remedies such as bleeding, blistering, or emetics and purgatives to rid the body of the poisons of disease. Surgery at this time was confined to the removal of tumours or amputations and infection was a common risk. The anaesthetic in general use in the 1860s was chloroform administered by a mask. The first medical use of antiseptic sterilization did not occur in Canada until after 1867.

 

Doctors prepared and dispensed their own medications even though there were pharmacies in larger cities and towns. Rural doctors also pulled teeth although there were licensed dentists in cities who could save them with gold or silver fillings.

 

The Physician's house at Upper Canada Village is an attractive neo-Grecian cottage which, in its original location in the village of Aultsville was not the home of a physician but was the home of Michael Cook, the man who first bred Holstein cattle in Canada.

Secretary Walker was the inaugural speaker for Grand Rounds as part of the Leslie and Edward Goldenberg, MD, Disease Prevention and Health Promotion Lectureship on Jan. 18 at Christiana Hospital’s John H. Ammon Medical Education Center. Secretary Walker’s talk was entitled, “Expanding Our SUD Treatment System: Why Physicians Should Embrace MAT.” She was introduced by Dr. Neil Jasani, Chief Academic and Learning Officer for Christiana Care.

 

Today, Secretary Walker said there about 90 registered medication-assisted treatment (MAT) providers in Delaware, representing 6.5% of primary care physicians. Delaware’s goal is to increase the number of physicians, nurse practitioners and physician assistants who can prescribe medications for opioid addiction and serve as an entryway into the treatment system. If Delaware could increase MAT providers by 50% to 135, that alone would increase access or capacity to MAT by more than 1,300 people, assuming a limit of 30 individuals per practitioner. In 2016, Delaware lost 308 people to overdoses, far exceeding the 120 lives lost in traffic accidents that year.

 

“For those with opioid addiction, studies show that it is most effective to combine behavioral therapy with medication-assisted treatment,” Secretary Walker said. If more physicians embraced MAT it would further destigmatize addiction and allow for comprehensive care of the individual by treating other medical problems along with the substance use disorder, she said. “We need to improve harm reduction,” Secretary Walker said, encouraging the more than 200 people in the room to access provider information on screening, identifying and treating addiction on DHSS’ website, www.HelpIsHereDE.com
