Print at 300 dpi, fold on the vertical lines, and tape the ends. For any particular month, just fold it flat so the month is in the top-left corner. The color of the month matches the last day of that month (April, June, September, November in grey have 30 days, February also in grey has 28 days, and all the other months have 31 days).
To see the finished product, here's the front, and back.
The main thing, it seems, for a calendar is to see the date, hence the large numbers. And really, mostly what you care about is the current month. So, mostly, you just roll the calendar to the current month, stick it in your wallet, and that's that. Hence the small months.
Regarding the days of the week, if the current view is only seven days wide, mostly showing the days of the week is redundant. One great visual is to alternate the color of the columns (a bitch to do in Paint Shop Pro 5.0, which is all I had at the moment, leading to this necessary minimalism).
One way to handle holidays, add a superscript of the month to the day of the month. So, for example, for the Fourth of July, put a little 7 (for July) next to the number 4. Simple, discreet, and grabs enough attention as it deserves. It turns out there's only one "overlap", both New Years and Labor Day occur on the first of the month.

Heat-map visualization of tiny chunk of data on age of certain demographic group. Trendy but less self-explanatory than the bar-chart.
Similar to the previous versions, except now whichever month you go to, Monday-Wednesday-Friday are always dark grey.
Makes it easy to find the day of the week: For any given month, the first and last columns (in white) are Sunday and Saturday (respectively), the dark columns are Monday, Wednesday, Friday; and the other two white columns are Tuesday, Thursday.
When printed at 300 dpi, the resulting calendar should end up 3.5"x2", the same as a business card, perfect for putting in your wallet.
