Faulty P/M steel.
Steel powders are compacted under high temp (1100~ 1200?) ato get rid of voids between particles in order to make a "solid" steel. Sometimes, voids are not completely removed and are left inside the steel. After machining, these voids open up to free surface. In this case, the voids open up on functional edge of a tooling serve as stress riser / crack initiator and cause the punch to fail prematurely.
Unless my memory is faulty, this one probably comes from an old Marine, one of the survivors of the 2nd Raider Battalion's Makin Island Raid, who later became an arms specialist with the weapons company, 4th Marine Regiment.
It's been polished and the guard bent straight, but it's original and I suspect worth much more than was paid for it sixty years ago.

Because of their faulty American Express Machine at the Bar at Cafe Leopold my friend Linda had to face an embarrassing situation , they accepted her card for the food bill , but the wine bill , their machine was faulty they made her go to an ATM machine and here too there was some glitch the waiter wanted her to change her dollars on the road as this huge restaurant does not accept foreign exchange we were about Rs 1000 short as Linda had spent all her Indian money on Attar .I immediately called my friend Ramesh Mulchandani who owns Strand Hotel he sent the amount and we left Cafe Leopold , sadly the managers at the restaurant and specially the two cashiers at Cafe Leopold were totally insensitive rude and arrogant and failed to understand Devo Atithi Bhava but a few Indian guys on the next table were willing to pay our shortage but I thanked them and told them my friend was sending the money a measly Rs 1000 they could have kept her dollars till I could have arranged the same ,,,Whatever money I had I gave too..
And though one of the manager was apologetic he was not apologetic about his faulty American Express machine I told Linda that whenever she travels she should always carry a Visa card .. I bet they dont have faulty machines ,,
And I was sad for my friend who wanted to write good things about Mumbai .. and Linda Scaefer a great photographer has worked with Mother Theresa .. she was really upset by this uncalled for incident at tony Cafe Leopold Colaba .

This hotel serves mostly foreigners and has no foreign exchange facility , their credit card machines dont work , their waiters want the guests to change money on the streets and this was one bad experience for my friend Linda at Cafe Leopold.
When this Lenovo ThinkPad T61 notebook is switched on, it beeps two times, after displays a “fan error” (faulty fan), then beeps again and shuts down. We’ve opened that notebook and cleaned its non-spinning fan.

Diodes and a transistor got damaged. Could not locate any spare parts so I had to get me a new logic board.
My character designs for "Faulty Choirs" - a music video I designed, directed and animated for OSYMYSO were commended at the 2009 Digital Artist Awards in Shoreditch, London.
They are on display as part of a Digital Artist exhibition, running at Blackall Studios, Shoreditch, London, from Friday 2nd October to Saturday 10th October.

An afternoon spent trying change a faulty 4wd bulb. In the end, the bulb hadn't failed, just a bad contact apparently, but a useful exercise anyway.
Not a very sharp shot (too close for the phone camera), but this is the bulb, pushed through the board from the rear and twisted to lock in position. Many people seem to advocate replacing these with l.e.d. alternatives, but I'm concerned that, as the problem here was lack of continuity between bulb and board, a l.e.d. would be even more prone to trouble, with very little current flow.

...but a broken camera producing a distorted landscape of stalactites in pretty pinks (I suspect a faulty CCD connection). I am so disappointed! No more photos for a while:(
UPDATE: Canon confirmed the faulty CCD connection and repaired the camera free of charge as this has been a common occurrence in many of their point and shoot cameras. So if this happens to your camera and the warranty has expired there is still hope:)
