When I went to see the doctor again on Thursday, I explained that I’d experienced some pain in my chest since taking the tablets he’d prescribed.

 

Of course he whipped out his stethoscope and checked my heart. Not content with that, he immediately sent me to have a further ECG which they performed around the corner in the emergency department.

 

Within minutes I returned with this long strip of paper which he carefully examined and thankfully declared showed nothing untoward. Angina type pains are one of the side effects of the tablets which is mentioned on the leaflet for between 1 and 10 people in every thousand.

 

He also said that my blood pressure was now “muy bien”.

 

Pam and I agree that getting older is not good but then it is better than the alternative.

Saturday evening's pre-dinner entertainment was sudden and startlingly intense chest pain - just in that spot in the upper left which is considered a Really Bad Sign. They did an ECG, which looked all right and some blood tests, so I got on with my dinner. They left all the sticky pads on in case they needed to check again - which they did on Sunday morning - roughly 8 - 10 stickies in all. By the time I had a shower on Sunday morning, I had accumulated more plasters from more blood tests, and what with the dressing, all the red sterilising paint, more sticky marks from stuff in theatre and another massive patch of tape left over from Friday, I looked like a patchwork quilt.

Anyway, the tests didn't reveal anything scary, so I assume the pain was "just" caused by weakness and exhaustion, and probably an injured pec muscle from levering myself in and out of bed. in the weeks pre-surgery, I'd already got weak enough to have chest pain a lot of the time, whenever I tried to do anything, so it didn't surprise me.

The ECG works mostly by detecting and amplifying the tiny electrical changes on the skin that are caused when the heart muscle "depolarizes" during each heart beat. A 39 years old male, complaining of sudden onset of chest pain, is brought to the emergency room by ambulance. The pain began four hours ago and is described as severe and pressure-like. It is central in location and non-radiating. Not relived or aggravated by any factor. Other associated symptoms are shortness of breath and nausea. No previous history of such complaints.

 

Review of systems:

Constitutional : fatigue, No fever, No weight change

Cardiovascular: chest pain, mild SOB.

Gastrointestinal: Nausea, No vomiting, NO abdominal pain

Other systems are unremarkable

 

PMHx: Hyperlipidemia

Allergies: NKA.

Family: unremarkable.

Social Hx: smoker

 

General appearance: The patient appears anxious, he was lying down on bed. He seemed alert, conscious, and oriented to time, place and person.

Vital signs on admission:

Temperature =36.6 °C

Pulse rate = 88 beats/minute

Blood Pressure= 128/85 mmHg

Respiratory rate= 18 breaths/minute

O2 saturation= 99%

 

Cardiac exam: Regular rate and rhythm, normal S1 and S2, no added sounds, no murmur, No lower limb edema

Respiratory: breath sounds equal bilateral, no added sounds like wheezing or crackles

 

TropI : 1.3 (H)

A young lady suddenly collapsed to the ground. Bystanders started CPR and called 911. Paramedics resuscitate her and rush her to the ER in hypotensive shock. This 12-Lead #ECG is recorded.

Distinguishing features; Classic triad of short PR interval, slurred upstroke of QRS (delta wave) and broad QRS complex. These changes represent pre-excitation; that is depolarization spreading to the ventricle via a bypass tract. This depolarization fuses with that occurring via the AV node yielding the broad QRS complex. The broader the QRS the earlier the passage of depolarization via the bypass tract suggesting it is closer to the AV node.

Distinguishing features; ST elevation in the anterior leads consistent with acute STEMI

Distinguishing features; PR interval approximately 280 milliseconds. 200 being the upper limit of normal. This defines first degree AV block.

This is an example of incorrect lead connection. R wave progression through lead V2 is obviously abnormal.
