Free crochet pattern by Melissa Mall offered on Ravelry or at crochetuncut.com. Very easy, beginner pattern - mostly just rows of single and double crochet. My version is slightly modified from the original pattern. I added a few extra rows and a button detail. The yarn I used also looks to be a slightly thicker worsted weight yarn than is photographed on the original pattern. But I'm happy with the way mine turned out.
