The stomach planet Inspired by 80's Cuban Sci-Fi stories. I remember a book where astronauts land in a planet that has some sort of root or primitive plant all over its entire surface. There were some lakes around, all with bright different water color. While exploring the surface the astronauts realize everything human that comes in contact with the surface of the planets erodes rapidly. The ship, the trucks, the boots, and soon after the ship starts to been suck into the colorful lakes. They discover after the entire planet is alive and all shaped to eat, digest everything that lands on it.

The major driver for the growth is the growing usage of deep learning technology across various industrial vertical such as advertisement, finance, automotive, medical and among other. Ritgerðin ber heitið: Notkun gervigreindar til greiningar og forspár fyrir horfur sjúklinga í heilsugæslu. The Use of Artificial Intelligence for Diagnosis and Outcome Prediction in Primary Care.
Andmælendur eru dr. Ronny Gunnarsson, prófessor við Gautaborgarháskóla, og dr. Hercules Dalianis, prófessor við Stokkhólmsháskóla.
Umsjónarkennari var Emil L. Sigurðsson, prófessor, og með leiðbeinandi var Hrafn Loftsson, dósent. Auk þeirra sátu í doktorsnefnd Jón Steinar Jónsson, heimilislæknir, Margrét Ólafía Tómasdóttir, lektor, og Yngvi Björnsson, prófessor.

Ágrip
Markmið þessa doktorsverkefnis var að rannsaka hvort nýta mætti textagögn úr samantektarnótum lækna til að þjálfa gervigreindarlíkön til þess að spá fyrir um greiningar og horfur sjúklinga í heilsugæslu, greina innri virkni líkananna og meta frammistöðu þeirra. Doktorsritgerðin byggir á þremur rannsóknum: Í þeirri fyrstu var líkan þjálfað á merktum greiningarsérkennum úr samantektarnótum lækna til að spá fyrir um frumkomna höfuðverkjagreiningu hjá fullorðnum í heilsugæslu, þar sem niðurstöður sýndu að líkanið stóð sig jafn vel eða betur en læknar. Greining á SHAP-gildum benti til þess að líkanið studdist við svipuð greiningarsérkenni og læknar við myndun úttaks. Í annarri rannsókninni var gervigreindarlíkan þjálfað á merktum greiningarsérkennum úr samantektarnótum lækna sem hitt höfðu fullorðna sjúklinga með ákveðna greiningarkóða öndunarfærasýkinga í heilsugæslu. Niðurstöður rannsóknarinnar sýndu að líkanið raðaði sjúklingum í áhættuhópa á áreiðanlegan hátt og greindi á milli alvarlegra og vægari tilfella með réttum hætti. Í þriðju rannsókninni var líkanið sem þjálfað var í rannsókn 2 metið framsýnt í raunverulegum klínískum aðstæðum. Niðurstöður þeirrar rannsóknar sýndu að líkanið raðaði sjúklingum með alvarlegar útkomur í háan áhættu hóp og sjúklingum með vægari útkomur í lágan áhættu hóp. Niðurstöður doktorsverkefnisins benda til að gervigreindarlíkön, sem þjálfuð eru á textagögnum úr sjúkraskrám, geta spáð fyrir um horfur sjúklinga og greiningar í heilsugæslu. Einnig nota líkönin svipaðar upplýsingar og læknar þegar úttak þeirra myndast við frumkomnar höfuðverkjagreiningar.
Abstract
The aim of this doctoral project was to investigate whether text data from doctors' summary notes could be used to train AI models to predict diagnoses and outcomes for patients in primary care, to examine the models’ internal functionality, and to evaluate their performance. The dissertation is based on three studies: In the first study, an AI model was trained on labeled diagnostic features from doctors’ summary notes to predict primary headache diagnoses in adults in primary care. The results showed that the model performed as well as or slightly better than doctors. An analysis of the SHAP values indicated that the model relied on similar diagnostic features to those used by doctors when generating its outputs. In the second study, an AI model was trained on labeled diagnostic features from doctors’ notes for adult primary care patients with certain respiratory infection codes. The study’s results showed that the model reliably stratified patients into risk groups and correctly distinguished between severe and milder cases. In the third study, the model from Study 2 was evaluated prospectively. The results showed that the model triaged patients with severe outcomes to a high-risk group and those with milder outcomes to a low-risk group. The overall findings of this doctoral project suggest that AI models trained on text data from medical records can predict patients’ prognoses and diagnoses in primary care. Moreover, the models use similar information to that of physicians during inference for primary headache diagnoses.

Steindór Oddur Ellertsson er fæddur árið 1985 í Reykjavík. Hann útskrifaðist af náttúrufræðibraut Menntaskólans í Reykjavík árið 2005 og lauk kandídatsprófi í læknisfræði frá Háskóla Íslands árið 2012. Steindór hóf rannsóknir samhliða klínísku starfi árið 2019 sem leiddu til þess að hann hóf doktorsnám í læknavísindum við Háskóla Íslands í febrúar 2022. Rannsóknarverkefnið hlaut styrk úr Vísindasjóði Félags íslenskra heimilislækna og RANNÍS. Samhliða doktorsnámi hefur Steindór starfað sem sérnámslæknir í heimilislækningum í Noregi og á Íslandi. Foreldrar Steindórs eru Ellert Kristján Steindórsson og Rannveig Alma Einarsdóttir. Eiginkona Steindórs er Olga Hrönn Jónsdóttir og saman eiga þau börnin Lísu, Björt og Styrkár.
