I have seen this lady inside the coffee shop many times. I vowed that when I saw her outside, I would ask for a portrait. But I never saw her outside. So one day, as I entered, she was getting up from a seat by the door; she was heading for more coffee. I waited for her to return. I planned to meet her and ask her to step outside. When she came back, I made my ask with these words, “I have made a million of portraits right outside...”, and then she cut me off. With an indignant look and hands on her hips, she said “What? You've made a million portraits and never made one of ME?” Oh, this was going to be good.

 

I introduced myself by name and held my hand out. She shook it and gave her name as Five.

“Five?”

“Yeah, Five”

“That's your name?”

“Yeah. Don't you understand Five?”

“Uh, I guess.”

She turned to the woman behind the counter; “Laura, isn't Five my name?”

Laura shrugged and pointed, “She's Number Five.”

Five is the fifth of nine children. When she came home from school, her daddy would say, “Well, number five is still alive.” So she's Five.

 

Number Five went to the University of South Florida four years after me. She had an eclectic career, but spent a lot of it as a school teacher of grammar. She has no children of her own. As a young woman, she had uterine fibroids, a large problem in the African American community. This required a hysterectomy, but Number Five says that she was 'spayed'.

 

Now she volunteers as a teacher at the Precious Jewels Academy in Newtown, an African American, and troubled neighborhood. The private school was founded by a retired educator who opened it up for troubled, at risk kids from a nearby housing project. Ironically, just yesterday the local newspaper had a human interest story about the academy. It is inspirational.

 

Five has been living in the same apartment for several years, but the rent goes up and up. She is ready to move. Five was about to catch a bus to check out a possible new residence, and then another bus home. I offered to drive her. It was to a neighborhood plagued by many motels housing prostitutes. After a police crackdown, many properties were purchased by developers who upgraded the motels into permanent apartments. An affable Brit named Toby was at the front desk (he refused the opportunity to be photographed :-( ). The outside looked nice and the price was right, so Five put her name on a list for occupancy. Toby said that he 'couldn't' call her, that she would have to call daily to check. It struck me odd that he would not call, and that Five didn't check out the interior of an apartment.

 

A few days later she informed me that all was not as it seemed. Five called Toby who said that a unit was available for her. But upon arrival, he was hostile and uncooperative – he was suddenly not sure that he would have an apartment for her. Confused at his change in demeanor, she met another potential renter outside. Apparently the apartments were only minimally improved inside; vestiges of previous history remained. And she confirmed that Toby was unpleasant and uncooperative. Five feels lucky that she didn't sign a lease, and explains Toby's original nice behavior on my being present.

IVF is not usually the first step to treat infertility. In cases of male infertility, the method of intracellular injection (ICSI) - injection of sperm into the egg, is used to fertilize eggs. More particularly, the ICSI is a special form of IVF in which a single sperm is injected into an egg.

Cases of unexplained infertility, where, despite a complete evaluation for known causes of both the man and the woman, the cause of infertility is not identified.

 

A genetic disorder

If your partner or you are a carrier of a genetic disorder, a process, which includes IVF, called Pre-implantation Genetic Diagnosis (PGD) can be applied. The Pre-implantation Genetic Diagnosis is the process of genetic analysis of a living fetus in order to determine the presence, absence or change of a particular gene or chromosome before transferring the embryo to the uterus.

 

Retention of fertility in women with cancer or other health problems

Women can freeze their eggs (oocyte cryopreservation) for future use, or freeze their embryos (embryo cryopreservation) for future use. Women who have no functional uterus or the pregnancy poses a serious risk to their health can choose IVF pregnancy with another woman (surrogate mother). In this case, the woman’s eggs are fertilized with her husband’s sperm, but the resulting embryos are placed in the uterus of a surrogate mother for gestation.

 

In Vitro Fertilization procedure

 

IVF is not a simple process, but a number of different stages. The stages involved in this process are:

 

Follicular stimulation

A combination of medications (fertility drugs) and hormones are usually given to a woman in order to stimulate her ovaries to recruit and mature many follicles so as to take more than the usual one egg per cycle.

This is to enable the collection of multiple oocytes, and hence to increase the probability of successful IVF. In each case there is the possibility of some of the eggs collected not to be fertilized or not to develop normally after fertilization.

 

During the medication period, the development of eggs is monitored by blood tests and transvaginal ultrasound to ensure that their collection will be done at the right time.

 

Various protocols are used for ovarian stimulation, such as the long protocol, the short protocol and the antagonist protocol. Choosing the right protocol for you depends on your age, the levels of FSH and the previous stimulation (if it exists) of the woman.

 

Egg collection

The egg collection is a relatively easy process that takes place in a sterile room in the presence of an anesthesiologist, since a mild anesthesia needed. It takes about 10 - 20 minutes, depending on the number of follicles in the ovary. It is required not to eat anything from the night before and to avoid water, candy or gum the morning of the egg retrieval.

 

During the egg collection, the position of follicles is successfully ascertained using vaginal ultrasound. Then, a needle is directed through the vagina to the follicle and as it pierces the follicles one by one, it aspirates the content.

 

The follicular fluid is delivered directly to the embryologist in the IVF laboratory, where the eggs are identified, isolated from the liquid and transferred to a suitable culture medium and suitable temperature.

After the egg collection you can normally eat, but it is preferable to have a light meal and plenty of fluids. You cannot drive or go to work on the same day. It is recommended to stay at home and rest.

 

Sperm collection

On the day of the egg collection, the husband is required to give us his sperm. The collection of semen is done by masturbation into a sterile urine collection box, and for a better quality of semen, 3 – 4 days abstinence is recommended.

 

However, because the collection of semen on a specific day can be difficult and stressful, the husband can provide the sample from his home earlier on the day of the egg collection, or a few days earlier, for it to be frozen (fresh samples are always preferred).

 

The sample is delivered to the laboratory, and it is kept in an oven at 37oC for a while, and then is subjected to a cleaning process, in order to detect the healthy sperm.

In cases of azoospermia, the semen will be taken directly from the testicles with a biopsy. Also, if using semen from a donor it will be defrosted and prepared with the same procedure.

 

Fertilization

A few hours after the egg collection and the sperm preparation, the fertilization takes place.

To begin the process of fertilization, the eggs are placed in a petri dish, which contains a specific growth medium, for 2 to 3 hours. The sperm, after cleaning, is placed with the eggs and incubated together at 37oC, for 16 to 20 hours.

 

The next morning, the embryologist will examine the oocytes to see which ones are fertilized, and then compose a detailed briefing on the outcome.

 

Fertilized eggs are now called embryos and are incubated in special laboratory conditions for another 48 - 96 hours, depending on the embryonic-transfer-stage, which is to be chosen by your doctor. The fertilization rate is usualy 80 – 100%.

 

Embryo-transfer

The embryo-transfer is a simple procedure, which requires no anesthesia.

Two to five days after the eggs are fertilized, the healthiest ones, according to specific morphological criteria, are selected for placement in the woman’s uterus.

 

The selected embryos are aspirated into a thin tube (catheter) and guided into the woman’s uterus. One to three embryos may be transferred, according to the age and medical history of the woman.

 

Following the procedure, a 24- to 48-hour rest is recommended. The remaining embryos that are not transferred can be frozen at this point and used in a next cycle of in vitro fertilization (IVF).

 

While the partner is encouraged to be present, his/her presence is not necessary, if for some reason he/she cannot attend. However, it is always recommended to be accompanied from the clinic to the house.

It is claimed that on one occasion, when Wilde warned his sons about naughty boys who made their mamas cry, they asked him what happened to absent papas who made mamas cry. Nevertheless, by all accounts, she and Wilde remained on good terms.[4]

After Wilde's imprisonment, Constance changed her and her sons' last name to Holland to dissociate themselves from Wilde's scandal. The couple never divorced and though Constance visited Oscar in prison so she could tell him the news of his mother's death,[5] she also forced him to give up his parental rights and later, after he had been released from prison, refused to send him any money unless he no longer associated with Douglas.

 

Constance died on 7 April 1898 five days after a surgery conducted by Luigi Maria Bossi.

According to The Guardian, "speculative theories [about her death] have ranged from spinal damage following a fall down stairs to syphilis caught from her husband. However, again according to The Guardian, Merlin Holland, grandson of Oscar Wilde,;unearthed medical evidence within private family letters, which has enabled a doctor to determine the likely cause of Constance’s demise. The letters reveal symptoms nowadays associated with multiple sclerosis but apparently wrongly diagnosed by her two doctors. This is also due to the fact that at the time multiple sclerosis was a little-known disease.

Constance sought help from two doctors. One of them was a "nerve doctor" from Heidelberg, Germany, who resorted to dubious remedies. The second doctor—Luigi Maria Bossi—conducted two operations (for uterine fibroid) in 1895 and 1898, the latter of which ultimately led to her death.[6] According to The Lancet, "the surgery Bossi performed in December 1895 was probably an anterior vaginal wall repair to correct urinary difficulties from a presumed bladder prolapse. In retrospect, the actual problem was probably neurogenic and not structural in origin." During the second surgery in April 1898 Bossi probably "did not attempt a hysterectomy but merely excised the tumour in a myomectomy". However, shortly after the surgery Constance developed uncontrollable vomiting, which led to dehydration and death. The immediate cause of death was likely severe paralytic ileus, which developed either as a result of the surgery itself or of intra-abdominal sepsis.

Constance is buried in Genoa (Italy), in the Monumental Cemetery of Staglieno.[8]

How can I describe the feelings I have for motherhood and for the times spent with my boys? What exactly is infertility?

The problems with either conceiving a child, or with carrying out the pregnancy to its eventual fruitful end, fall under the definition of infertility. Infertility is the incapability of an individual to become pregnant, in case of females, or the incapability to induce pregnancy, in case of the males. The inability of an individual to carry out a pregnancy to its full term is also dubbed infertility. How does one recognize infertility? What are the signs of infertility?

 

Signs of infertility are not always evident. Most people go through life without knowing there is a problem with their reproductive systems, attributing failed pregnancies to providence. In fact, miscarriages are the most common indicator of infertility. Signs of infertility in women:

 

In women, the signs of infertility are more readily recognized as compared to men. Endometriosis causes the lining of the uterus to grow outside the uterus.

 

Bacterial infections may begin around the uterus and spread to other reproductive organs, resulting in infertility. Fibroids in the uterus are indicative of infertility. Tumors in the cervix often cause stenosis, or narrowing of the cervix, which is a common indicator of infertility.

 

Ovulating before the tenth day and after the twentieth day of one's monthly cycle, pre-menstrual spotting, menopausal symptoms, etc. are indicative of luteal phase defect, and thus in turn are signs too.

 

Irregular menstrual cycles are the most common indication in females that they might have some problems with fertility. However, an irregular menstrual cycle is not conclusive in itself, but it is definitely one of the signs of infertility.

 

Issues regarding body weight are often indicators of being infertile. For a woman, being too thin, or anorexic, will definitely hinder pregnancy, since the body does not have the proper nutritional requirements, or the required strength.

 

Alternatively, obesity can also be a sign. Obesity is accompanied by hormonal imbalance, which affects the reproductive system and pregnancy. Signs of Infertility in Men:

 

Like females, in males too, either obesity, or anorexia, is an indication that he is infertile. Apart from these, anatomical defects may also be signs of infertility. Undescended testicles, or damage to scrotum and the gonads, are possible indicators too. Wearing tight undergarments, or exposing the testicles to heat, may render the person unable to produce the required number of sperms, or unable to produce sperms altogether, resulting in infertility.

 

Determining the signs of infertility:

 

There are many medical procedures for detecting the signs that help to determine whether an individual is infertile or not. Doctors usually prescribe one or more of the following medical tests:

 

. Hysterosalpingography. A dye injected into the vagina is monitored to check for blockage in the fallopian tubes or uterus.

 

. Laparoscopy. If disease and other physical problems are present in the ovaries, fallopian tubes, or in the uterus, infertility is suggested. This may be detected through laparoscopy.

 

Infertility is a problem which can be solved if it is addressed in the holistic way, which is, using a multifaceted method of healing.
