Decades of commercial and recreational fishing in Puget Sound have left behind tons of abandoned fishing gear that kills marine life and degrades habitat and even is a hazard to tidal power stations! The Northwest Straights Commission, Marine Conservation Initiative (Northwest Straights), was authorized by the United States Congress to assess the ecological health of Northwest inland marine waters, works to survey and remove derelict fishing gear in Puget Sound. Where should the removal crews be sent? If wherever they go, nets are removed 100% of the time, this is ideal, but nets move and sonar images of potential nets turn out to be other things. This is where EPA divers come in. By prescreening sites, scientific divers can provide maps to Northwest Straights of where ghost nets are confirmed to expedite removal efforts. EPA receives three types of information from NRC related to potential derelict net locations: 1) side-scan sonar imagery and GPS coordinates where bottom topography and bathymetry show a high potential to snag abandoned nets; 2) reports from fishermen and other reputable sources that have direct knowledge of net locations; and, 3) shoreline reaches with high potential for hosting derelict gear. Divers are deployed in pairs to determine if derelict nets are present. The first day at the hospital is called “Clinic Day”. On this day, prospective patients are evaluated to see who is healthy enough to have surgery, and see what the surgical issue is, and to schedule their surgery. Over 120 patients with clefts were prescreened. We had two local translators, who made everything run smoothly. They worked with the surgeons and Dr. Quintana, our pediatrician, to coordinate between the doctors, patients, and their families. Unfortunately, there was a bit of jockeying going on, since we could only schedule 84 patients. The "leaders" of groups of patients from a particular place were anxious for the most members of their group to be helped.

Today we set up shop at el Hospital Militar Central. Our host, Dr. Jorge Terrazas, is great. He not only prescreened 700 patients prior to our arrival, but he also facilitated setting up our operating site and getting local nurses, residents, and other hospital workers to help out. The clinic itself was captivating; there were so many people in need of correction of hand problems including congenital defects, burns, work-related injuries and accidents. Dr. Kyle Bickel, Dr. David Kim, and Dr. Joyce Chen along with Dr. Jorge Terrazas helped evaluate all patients to determine if an operation could improve function and form of their hands. Witnessing their collaboration with decision making was phenomenal. Everyone on the team was working hard. The pediatricians and anesthesiologists were thoroughly evaluating patients and the team nurses were setting up the quirófanos (ORs) and seeing patients as well. Given that we were starting up quirófanos from scratch, the anesthesiologists worked closely with the local physicians and nurses to ensure that we had everything we needed. Amazing day really, many patients in true need of help and great people working together to make it happen.
