Appendicitis | Laparoscopic Appendecectomy

APPENDICITIS is an inflammation of the appendix, a finger-shaped pouch that projects from your colon on the lower right side of your abdomen. The appendix doesn’t seem to have a specific purpose. Appendicitis causes pain in lower right abdomen. However, in most people, pain begins around the navel and then moves. As inflammation worsens, appendicitis pain typically increases and eventually becomes severe. Although anyone can develop appendicitis, most often it occurs in people between the ages of 10 and 30. Standard treatment is surgical removal of the appendix.

Symptoms Signs and symptoms of appendicitis may include:

• Sudden pain that begins on the right side of the lower abdomen Sudden pain that begins around your navel and often shifts to your lower right abdomen

• Pain that worsens if you cough, walk or make other jarring movements

• Nausea and vomiting

• Loss of appetite

• Low-grade fever that may worsen as the illness progresses

• Constipation or diarrhea

• Abdominal bloating Causes A blockage in the lining of the appendix that results in infection is the likely cause of appendicitis.

The bacteria multiply rapidly, causing the appendix to become inflamed, swollen and filled with pus. If not treated promptly, the appendix can rupture.

Complications:

Appendicitis can cause serious complications, such as:

A ruptured appendix. A rupture spreads infection throughout your abdomen (peritonitis). Possibly life-threatening, this condition requires immediate surgery to remove the appendix and clean your abdominal cavity.

A pocket of pus that forms in the abdomen. If your appendix bursts, you may develop a pocket of infection (abscess). In most cases, a surgeon drains the abscess by placing a tube through your abdominal wall into the abscess. The tube is left in place for two weeks, and you’re given antibiotics to clear the infection. Once the infection is clear, you’ll have surgery to remove the appendix. In some cases, the abscess is drained, and the appendix is removed immediately.

Diagnosis

To help diagnose appendicitis, your doctor will likely take a history of your signs and symptoms and examine your abdomen. Tests and procedures used to diagnose appendicitis include: Physical exam to assess your pain. Your doctor may apply gentle pressure on the painful area. When the pressure is suddenly released, appendicitis pain will often feel worse, signaling that the adjacent peritoneum is inflamed. Your doctor also may look for abdominal rigidity and a tendency for you to stiffen your abdominal muscles in response to pressure over the inflamed appendix (guarding).

Blood test. This allows your doctor to check for a high white blood cell count, which may indicate an infection.

Urine test. Your doctor may want you to have a urinalysis to make sure that a urinary tract infection or a kidney stone isn’t causing your pain.

Imaging tests. Your doctor may also recommend an abdominal X-ray, an abdominal ultrasound or a computerized tomography (CT) scan to help confirm appendicitis or find other causes for your pain.

Treatment

Appendicitis Treatment usually involves surgery to remove the inflamed appendix. Before surgery you may be given a dose of antibiotics to prevent infection. Surgery to remove the appendix (appendectomy) Appendectomy can be performed as open surgery using one abdominal incision about 2 to 4 inches (5 to 10 centimetres) long (laparotomy).

LAPAROSCOPIC APPENDECECTOMY , surgery can be done through a few small abdominal incisions During a laparoscopic appendectomy, the surgeon inserts special surgical tools and a video camera into your abdomen to remove your appendix. In general, laparoscopic surgery allows you to RECOVER FASTER and heal with LESS PAIN and scarring. It may be better for people who are elderly or obese. But laparoscopic surgery isn’t appropriate for everyone. If your appendix has ruptured and infection has spread beyond the appendix or you have an abscess, you may need an open appendectomy, which allows your surgeon to clean the abdominal cavity.

Otto is REALLY good at removing squeakers from toys. If he doesn't do it the moment he gets his little paws on a new one, it will happen. It's just a matter of when.

Last night a friend came by for dinner. As it was raining all day and he'd been stuck indoors with his dog, Jake, he brought Jake along for some R&R with Otto.

After much trading of toys, guarding of bones, and competing for chewies Otto normally ignores, my friend and Jake left. They left behind a small red squeaking ball. Otto of course found it.

This afternoon Otto ran about the house squeaking it constantly. This evening, when I went to throw it for him, the squeaker no longer squeaked. It just rattles around inside the ball, a dead piece of plastic.

Somehow Otto managed to disable the squeaker without the usual shredding and left over body parts. A sort of laparoscopic squeakectomy. I've called him the surgeon before when he's dismembered some stuffed critter; I guess he's upgraded his skills.

Endometriosis cysts :- Your endometrium is the covering of your uterus. Once in a while, for reasons that specialists don't totally comprehend, this sort of tissue can begin to fill in different spots like your fallopian cylinders, bladder, or peritoneum (covering of pelvis and midsection). In the event that that occurs, specialists call it endometriosis.

On the off chance that the tissue gets to your ovaries, a growth structures. That is an endometrial growth, otherwise called an endometrioma. One sort of growth can frame on the ovaries.

You could have just one, or you could have a sore on every one of your ovaries. While they're frequently little (under 2 inches), these growths can develop as large as 8 crawls across.

The most widely recognized indication of endometriosis is torment in your lower midsection that doesn't disappear. It can deteriorate previously and during your period. You may likewise have exceptionally substantial dying. Agony during sex is probable.

A few ladies who have an endometrial sore believe sore or notice pressure. Others don't have any side effects whatsoever. You may not realize you have a growth until your PCP feels it during a pelvic test or sees it on ultrasound.

Your primary care physician will converse with you, pay attention to your indications, and get some information about any torment you have. During a pelvic test, they'll press on regions in your stomach. They might have the option to feel a pimple thusly.

Your primary care physician will presumably need to do an imaging test to see inside your body. One way is with a ultrasound. It utilizes sound waves to make an image. Attractive reverberation imaging (MRI) gives a more nitty gritty perspective on your ovaries. It utilizes an amazing attractive field.

Your primary care physician may likewise do a blood test to check for malignant growth, check whether you're pregnant, or then again in case there's a disease.

One more approach to check for an endometrial sore is through a laparoscopy. During this outpatient medical procedure, your PCP will make a small entry point (cut) in your tummy catch and addition a slight camera. This permits your her to perceive any sores very close, survey their size, and choose how best to treat them.

Hydrosalpinx is the name for a condition in which a woman’s fallopian tube becomes blocked with fluid.

There are different causes for this condition and symptoms can vary depending on the individual. Some women do not experience any symptoms, but hydrosalpinx can have a severe impact on fertility.

Symptoms of hydrosalpinx include:

abdominal and pelvic pain

unusual vaginal discharge

Diagnosis

Hydrosalpinx is characterized by one or both fallopian tubes being filled with water.

A doctor can diagnose hydrosalpinx using several methods. These include:

An ultrasound: This is one way of looking for hydrosalpinx, but is not as effective.

Hysterosalpingogram (HSG): This is a type of X-ray that can display tubal blockages. A special dye visible to the X-ray is introduced through the vagina and cervix.

Laparoscopy: Also known as keyhole surgery, a laparoscopy is a surgical procedure where small incisions are made in the abdomen, and a camera is inserted. This allows a surgeon to see the organs and remove fluid or other issues.

A diagnostic laparoscopy can also determine whether there is another issue that may be causing infertility, such as endometriosis.

A surgeon will perform a hysterectomy through the vagina, a large incision in the abdomen, or laparoscopy. You can either have a total hysterectomy, a supracervical hysterectomy, or a radical hysterectomy. During the procedure, the surgeon will remove your fibroids by removing your entire uterus. The great part about a hysterectomy is that your fibroids can’t come back, but this procedure has the longest recovery time and poses the highest surgical risk.

The interventional radiologist will then numb your upper thigh, make a small incision, and insert a tiny catheter. A real-time x-ray called fluoroscopy helps guide the catheter into the uterine artery that supplies blood to fibroids.

Vancouver General Hospital Establishes a Robot-Assisted Laparoscopy Program

In July of 2005, H. Krueger & Associates Inc. created a five-year business plan for Dr. Larry Goldenberg analyzing the cost-effectiveness of initiating a robot-assisted laparoscopy program in Vancouver Coastal Health. The plan concluded that a high-precision surgical robot, such as the da Vinci Robot, would be worth the high initial expenses because of its unique overall benefits. The da Vinci robot allows surgeons to operate inside difficult areas of the body such as the pelvis and chest, using smaller incisions and resulting in greater accuracy in procedures where meticulous precision is required. The surgical benefits to the patient are less blood loss, less pain and trauma, faster recoveries and shorter hospital stays.

Due to generous private donations from Jack Poole and Jim Pattison. the robot arrived at Vancouver General Hospital September 8, 2007. Jack Poole, the chairman of Vanoc, is a prostate cancer survivor himself and agreed to donate $3 million towards the initial costs of the surgical robot on the amusing condition that the robot be named “Jack”. A $1.5 million donation from Jim Pattison along with smaller private donations and some fund raising will round out the $6 million required to bring “Jack” to VGH.

After it’s arrival, VGH will be the third hospital in the country to have such a robot. H. Krueger & Associates Inc. is pleased to have been an integral part in this advancement of BC’s medical endeavors.

Evening before surgery: Waiting for the mag citrate to work...

(because of my history with abdominal adhesions and implants my dr wanted to make sure my bowels were empty before operating tomorrow, to reduce the risk of fatal complications. I don't like the thought of pooping myself half to death tonight, and I REALLY don't like the taste of the laxative, but I absolutely don't want her to have an increased risk of nicking my bowel and killing me :P ... so yeah :P )...

(If it was a chance it was YOUR last morning alive, you'd kiss your sweetie too!! lots.)

✌Went in for my second opinion last Saturday and the new doctor is a million times better than the first. She had the same recommendation: hysterectomy, but she spent about an hour with me explaining why, and doing another exam & ultrasound to make sure she knew what was going on.

✂️ Because I have several complications, she was leaning towards having to do the abdominal incision (instead of laparoscopic surgery), and she had me come back yesterday for a biopsy (not fun) to make sure there are no signs of cancer.

After the procedure yesterday she feels more confident that laparoscopy will be possible & that the results will be benign. But the actual pathology report comes back on Sat.

Until then: no lifting, no competing, try to rest & recover from the biopsy and all the stress.

I kinda lost my 💩 during training this past Monday when it all hit me (swipe to see that look). But I’m slowly starting to get my brain around it.

➡️ Surgery sucks, but this is a relatively common one, and I feel much more confident & supported by this doctor.

➡️ Missing this meet sucks, but it’s just one, there will be more.

➡️ Time off lifting sucks, but I’m grateful this was caught now before it got any worse.

One day at a time!
