Diffuse large B-cell lymphoma of the testicle in a patient with history of polycythemia vera. There is a diffuse infiltrate of anaplastic tumor cells in the interstitium surrounding atrophic seminiferous tubules.

Hot tub lung is associated with exposure to Mycobacterium avium intracellulare complex (MAC) when present in aerosols from indoor hot tubs/spas as well as swimming pools, showers, humidifiers, and wind instruments.

High resolution computerized tomography (HRCT) findings are usually diffuse centrilobular nodules and/or ground glass opacities. Cavitary nodules have also been reported.

It is generally considered to be a form of hypersensitivity pneumonitis {HP), but since MAC has been cultured from lung tissue there is uncertainty as to whether it represents an infection, hypersensitivity, or a combination of both. Biopsy usually shows non-necrotizing granulomas which may be localized around airways; necrotizing granulomas are sometimes present. Granulomas are present in the lumens of small bronchioles as well as in the interstitium and airspaces. Airspace granulomas may also be seen in sarcoidosis, but they are not nearly as numerous and conspicuous as they are in hot tub lung. The granulomas are usually larger and more well-formed than those seen in non-mycobacterial HP. Organizing pneumonia is often present. AFB stains of granulomas are usually negative but may be positive. Lymphangitic distribution of granulomas and granulomatous vasculitis are characteristic features of sarcoidosis that are not seen in hot tub lung. Hot tub lung can be suspected, but not diagnosed, based on biopsy findings alone. In addition to histologic findings, definitive diagnosis requires a history of indoor hot tub use or exposure to aerosolized water contaminated with MAC.

Pneumocystis infection with typical foamy exudate involving alveolar walls (interstitium) as well as alveoli.

This foreign material came from a follow-up transbronchial biopsy of a lung transplant. It is located in the interstitium adjacent to the airway. It is an exogenous material with the capacity to induce a giant cell foreign body reaction. It has not been identified, but it has been suggested to be plant or synthetic material used in previous surgery, as well as hydrophilic polymeric material. Right image -polarized.

Katie, postbac '13, is keeping a blog of her Field Work Term experiences as a research assistant at UVA Health System, which affords her the opportunity to shadow in the cardiology PICU, observe surgeries, attend Morning Report and author and co-author research articles. Here’s Katie’s post titled Chief’s Case:

 

“Every morning in the pediatrics department there is a meeting called Morning Report, during which residents present all the cases on the pediatrics floor, and then at the end, someone gives an informative presentation on a topic of his/her choosing. Today, the presentation was a chief’s case — an interesting case that the chief resident presents, followed by a discussion in which the interns and other physicians in the room figure out the diagnosis. “This was the most interesting meeting I’ve sat in on yet. It gave me a glimpse into the realm of medicine that involves solving puzzles. To a certain degree, every morning on rounds is a puzzle in the sense that keeping a patient stable in the PICU requires a delicate balance of various meds. But most of the time (at least as far as I can tell), by the point the patient is under the care of the cardiologists, the diagnosis has been made.

 

“What I saw today involved figuring out why a baby arrived at the emergency room with a fever, cough, and reddish purple coloring. After that description, the chief turned it over to the interns and said, “OK, so what do you want to know?” And everyone worked together to get to the right diagnosis — there was brainstorming and discussion and questioning of why the interns wanted to know certain things (e.g., why do you care about sodium levels?). The level of challenging seemed just right — they had to defend their reasoning, but nothing was ever combative. Overall, it seemed like a supportive learning environment, and one that reminded me a lot of discussions at Bennington.

 

“Eventually, they got to the correct diagnosis of nephrogenic DI (diabetes insipidus), which occurs when the body can’t concentrate urine because of insensitivity to ADH (anti-diuretic hormone). Treatment includes medicines (e.g., hydrochlorothiazide) that are actually diuretics, but can also reduce urine output in people with NDI. I don’t understand the mechanism yet, despite my Wiki attempts. I’m still unclear how reducing the permeability of the distal convoluted tubule to NaCl (thereby causing less water to osmose from the tubule into the interstitium, resulting in more water lost in urine) would treat the condition. But hey, I know some people who would know, so I’ll find out why.”

 

Katie is also finding time to meet new friends and explore her surroundings by hiking in the Appalachian mountains around Charlottesville.

Glomeruli (G) show mesangial expansion due to an increase in mesangial cells and matrix. The proximal convoluted tubule (PT), distal convoluted tubule (DT) and the interstitium (INT) are unremarkable.

1. Infiltration of lymphocytes and plasma cells in the interstitium.

2. Atrophy of renal tubules.

3. Thyroidization of the renal tubules.

4. Interstitial fibrosis.

Klimmelstiel-Wilson figures, characteristic of diabetic nephropathy. There is nodular sclerosis in glomeruli secondary to expansion of extracellular material. These nodules stain positively to PAS and MT special stain, and they are negative for Congo red. The interstitium shows chronic inflammation.

The glomerulus (G) is associated with a cellular crescent (C). Bowman’s capsule is discontinuous in the region of the crescent (C). The tubules (T) and interstitium are unremarkable.

The glomerulus on the left has focal segmental sclerosis. The glomerulus on the right has a fibrous crescent and notable thickening of GBMs. Within the interstitium, there is tubular simplification, tubular atrophy, and inflammation.

Globally sclerosed glomerulus in the bottom center of the photomicrograph. 31 glomeruli available, 9 globally sclerosed. Segmental sclerosis in the glomerulus in the bottom left corner. Artery in the upper center shows severe hyaline arteriolopathy. Acute tubular injury that consists of dilated tubular lumina, attenuation of the tubular epithelium, & scattered loss of tubular epithelial cell nuclei. In the interstitium is a mild to moderate mononuclear inflammatory infiltrate confined to areas of interstitial scarring

There are two glomeruli (G). One glomerulus shows segmental endocapillary proliferation (E), a segmental double contour (open arrow) and a fibrocellular cresecent. An inflammatory infiltrate is seen in the interstitium (INT) adjacent to Bowman’s capsule. An unremarkable renal tubule (T) is also seen.

In the center of this field, there is an arteriole (one smooth muscle layer, flattened endothelial cells) with a considerable amount of bright pink (PAS positive) material in the subendothelium and smooth muscle layers. Within the surrounding interstitium, there are multiple atrophic tubules, interstitial fibrosis, and some inflammation. In the top left corner of the photomicrograph, there is another small artery (two layers of smooth muscle), only the bottom half is visible, and there is similar PAS positive material in the smooth muscle.
