A Fundação Oswaldo Cruz (Fiocruz) Brasília, a Organização Pan-Americana da Saúde (OPAS) e a Secretaria de Saúde do Distrito Federal lançaram o projeto Imuniza Estrutural, que busca aumentar a cobertura de vacinação contra COVID-19 e outras doenças imunopreveníveis, com o envolvimento da comunidade, em área de alta vulnerabilidade socioeconômica.
O projeto possui três pilares: comunicação e mobilização social; monitoramento e sustentabilidade das ações; e vacinação das populações mais vulneráveis.

Jilab Health Center is supported by UNICEF and the Ministry of Health through donations by the Government of Sweden. Maimuna Hussein (wearing burgundy headscarf and dark glasses), Head of Health Center and Nurse Services. They do immunization and nutrition social mobilization, mother and child maternal health 24-hour services, antenatal care, postnatal care. UNICEF supports nutrition, vaccination and general support. Catchment area is I 27,271 people. Some people come from 6 kms away. She says that 99 per cent of mothers get their children vaccinated once they have the consultations. Some community members challenge getting children vaccinated but when there is an outbreak, they realize that the vaccines are necessary. It’s usually fathers that resist because they are away at work when the mobilizers come or when the mom goes to the clinic and gets educated about vaccinations. She said that spreading the information about vaccines should be increased through radio and SMS.
The Jilab Health Center vaccinates around 200 children below 5 years old. They also scan and treat children for SAM. In June they screened 709 children and admitted 21. Due to the drought there was a campaign to teach families about using the MUAC to screen there children at home and this is helping to catch malnutrition early.

Mother Fartuna Abdullahi brought in baby Muhim Mohammed, 9 months old (wearing a white and green shirt), to get his first measles vaccination. She has seven children at home and makes sure they all get their routine childhood vaccinations. She knew to bring Muhim today because the social mobilizers came to her house and let her know about the campaign. She has lives in Jilab IDP camp for 4 years now. She was displaced to Jilab when her family lost all of their animals to the previous drought crisis. To earn small money she cleans for people in the town of Garowe.
There is a mobile clinic that goes into the IDP camp 3 times a week to provide vaccinations as communicable diseases like diarrhea, cholera and measles are more frequent due to lack of water and displacement during the drought crisis. The Mother and Baby Center is one of the locations (a “temporary fixed location”) where a vaccination campaign is taking place today.

10 May 2012, Tajikhan Village, Jabalseraj District Parwan Province, Afghanistan : Immunization log books at the Sar-e-Hause medical health clinic. The clinic is funded by the Strengthening Health Activities for Rural Poor Project (SHARP). SHARP aims to improve the health and nutrition status of Afghans, focusing especially on women and children and the underserved areas of the country. Already remarkable progress has been made in the reduction of infant and under five mortality as well as pregnancy related mortality. With World Bank support in 11 provinces the number of health clinics has nearly tripled from 148 to 432 and about 85% of the population now lives in districts which now have service providers to deliver a basic package of health service. Mohamed Mohud Hassan (on right in white jacket) is the Head of Jawle Health Center in Garowe town. Sitting at the table and recording the children’s names and vaccinations is Dr. Bashir Ali. They are seeing an increase in the number of malnourished children as the drought continues. In July there was a measles outbreak and more children for suffering but there was an outreach campaign in response to the outbreak and the situation soon stabilized. After the outreach about immunization around 530 children under 5 years old were brought into the clinic for their dose.
Dr. Bashir Ali. He has been working at the Jawle Health Center in Garowe for 7 years now. He says, “Today there are many children being brought here because on Wednesday and Thursday the social mobilizers went around to tell parents and caregivers to bring their children for vaccinations and screening and treatment for malnutrition.” They screen an average of 1000 children a month for malnutrition and admit around 30-40 children for moderate acute malnutrition (MAM) and 20-30 for severe acute malnutrition (SAM).
Thef Jawle Health Center in Garowe town serves an area of over 33,000 people that includes many internally displaced persons (IDP) camps. The Health Center has a solar powered cold chain storage unit to keep vaccines at the right temperature. On the right side of the center, mothers bring their children for vaccinations while on the left, the children are being screened and treated as outpatient for malnutrition. The center has mother and child services, nutrition treatment and a pharmacy.
The cold chain storage is one year old and supported by UNICEF through a Chibania grant and the vaccinations, health worker trainings and some incentives are provided by UNCEF with thanks to GAVI. The mother and child maternal health (MCMH) services are supported through the UK Government and RUTF by USAID.
