twyla went in for exploratory surgery this past wednesday...1:00 pm....sometime into it we where called back to talk with her physician...the surgeon had explored her and found the large mass not attached to her spleen but attached and growing around her intestines, much more evolved then splenetic removal..two sections of intestines where intangled...no other organs showed signs of metastasis.....we where given the options to biopsy only, euthanize....or continue on with removal with dissection of her intestines.....
with no signs of spreading yet we chose to continue on...there really was no other option to us. she was in recovery by by around 3 pm....we had just a quick peak, i had to see her before going home.....it was so sad to see her like that for me... critical but stable.....
i got an update thursday morning from her doctor, she had a good night with plenty of good rest....and had been up some that day already, wanted us to be there later that evening with a little bland soft food that might be good to her...she was not wanting to eat for them, thought would do her good to see us....it was so wonderful to see her, she seemed so much smaller.....that mass was about the size of a medium sized cantaloupe...oblong in size... i also felt it really good for her will to want to get better to see us ....this is a picture from that moment...she still wouldn't eat on her own... but it had not been much more than 24hrs....not unusual really....the doctors main concern is that there was no vomiting....
today, this morning i talked with him....she did have a sml amount of V but mostly bile from an empty stomach....she's still not eating on her own.... he feels better keeping her one more night till tomorrow, she in on fluids and injectable antibiotics, meds for the discomfort & anti nausea meds....he said she continues to be in good spirits .. dismissal will be saturday...we meet him at 10:00 am....he wants to see how she progresses at home till monday....i think it will b the best medicine for her...being at home.
this is very serious ....results we be back soon from histopath...
i feel truly blessed for her to have this new lease on her life....one that she would not have without this surgery....will cherish each and every day together

After removing the 3 white abnormal structures (lying next to the gecko), this is what I saw. Everything looked pretty normal. I submitted the abnormal structures as well as some other tissues and the diagnosis came back as xanthomatosis. This is a syndrome that has been reported in geckos and the cause is not known.
Here's the histopath report:
"Sections from the white nodular tissue have only a small amount of loose connective tissue along the sides. The remainder is composed of multiple nests of cholesterol clefts with neovascularization. Small strands of fibrous connective tissue separate some of the cholesterol aggregates. Scattered throughout the mass are foci of multinucleate giant cells. There are rare aggregates of lymphocytes."

Dr. Muraka did everything he could to save this kitten--CPR, mouth-to-mouth (the reason for intubation) and epinepherine injections to restart its heart (the empty syringe can be seen). It's difficult to tell from this photo, but the kitten was cyanotic when it was discovered--it's tongue should be pink but was quite bluish. A sure sign that it was not breathing and had suffered from oxygen deprivation.
A few minutes later another kitten from the same group was discovered in similar condition; it too could not be revived.
Dr. Muraka was visibly shaken by this and ordered a necropsy. This is unusual in shelter medicine, but it seemed that a particular person bringing cats to the Cube had a history of bringing sick cats, many of whom died when they should not have.
There is a history behind those cats, and it gets a bit complicated but the bottom line is that these were cats turned into one shelter and adopted out without proper health screening or care. They were then brought to us in a compromised condition--had they been left in the original shelter we would not be seeing these unexpected deaths. Some of them might have been too sick to save and would have been humanely euthanized, the rest would have been treated--none would have had to die like this.
The necropsy was inconclusive--we will never know why those kittens died. Dr. Budinich reported:
"They didn't find anything abnormal in the histopath samples (I only sent from one cat). I sent a piece of intestine and liver (incase of toxin). Should have probably sent lung too, but also didn't see anything abnormal. We have been seeing panleuk (despite negative test) and calicivirus in cats from ACCT, so wouldn't be surprised if they were sick with something underlying, and the anesthesia pushed them over the edge. Not sure what is going on."....

The photo licensed here is of our youngest Pug Mack. He just went in for a big dental procedure and surgery for a lump on his neck yesterday. So he doesn't look as pretty at the moment....we're calling him "frankenpug" even though we're a bit late for halloween.
He had 9 teeth extracted and has a huge surgical incision on his neck....and is being a big suck. Hoping the histopath results are going to show clean margins but at least he's home now and has me here to devote as much time to him as he needs. Another bonus to my "job", since I make my own schedule :-)

Veterinary dentist Brett Beckman comments: "Radiographically you can see there is an abnormally small and misshapen canine tooth, a maxillary first premolar within an expansile lucency consistent with a dentigerous cyst associated with an unerupted canine and/or first premolar. There are also numerous small toothlike (denticles) densities scattered throughout. This finding is consistent with a compound odontoma. Histopath only can confirm the presence of these masses and definitive care can only be achieved with surgical removal of the entire mass/cyst and its contents."
