This photo satisfies requirement 2, since it is taken indoors and the only light source is natural sunlight.
This is a photo of my girlfriend's nephew. He was camera shy, so I had to sneak a photo when he wasn't ready, hence the food bits on his face. It was tough to get him positioned correctly, and this photo was a bit of luck with regard to the timing and lighting. Since it was very sunny outside, there was plenty of available light through the glass back door he is standing next to, and the indoor lights were off. The sun serves as both the key and fill light. The bright key light is the sun coming through the glass door, lighting up the right side of his face (left side when looking at the photo). The fill light is provided by some sunlight coming through the windows on the other side of the room, about 15-20 feet away off the right side of the photo, creating just a bit of light to lift some of the shadows. The lighting creates interesting highlights over the texture of his hair.
Since it was dim inside (even with the sunlight coming through the windows), and there were various lighting patterns throughout the room between the sun and shade areas, I decided to use aperture mode and let the camera take care of the rest to avoid having to make too many rapid manual adjustments between pictures. In post, I bumped up the sharpness a bit and removed the color, giving the photograph a black-and-white style, which I think goes well with his somewhat somber expression.
