This is the Christmas tree at the gastroenterologist's office that I visited this week! It is a great tree!

Today's story and sketch "by me" you can see JB above on a search and rescue mission, this one involves Dr Steven Wannapoki the missing Gastroenterologist and Super Secret Interdimensional Space Missionary, with Emesis his surgical assistant, who when she is not doing surgical things, rides along to help on undercover missions. Dr Steve went missing when taking Emesis to a picnic at lake Maxinthecooki on his 14 foot yacht "The Smooth Move", which will have to be a story for another time. Dr Steve's tale began three weeks ago when he drove away from the drive thru window with half a dozen banana moon pies, onto route 66 without tipping Zooelda, owner of The Dimension Two Moon Pie Shop in Broken Drum Indiana, Zooelda is a stickler for tipping etiquette, customers driving away without leaving a proper tip soon find themselves travelling through a worm hole into another dimension, on most occasions a Z dimension, created with a creative gypsy Z curse by Zooelda for an X husband. Zooelda does not stay betrothed for very long, her longest was 14 days, so rather than going through a nasty divorce she just sends them off transformed into animal's or one of her favorites as spirit trees, oaks, pines, or another favorite a spirit Tiki Palm. This cool ornament, made of rusted metal, was hanging on a Christmas tree at the gastroenterologist's office that I visited this week! It is a great tree!

Either this bird needs to see a gastroenterologist, or we are becoming concerned about the quality of the yabbies in the lake. We have seen yabby traps around the lake, so hope it doesn't have the same effect on the locals.

busy day again today. did a lot of colonoscopies this afternoon. i do wonderf why anyone would like to have a colonoscopy on saturday but it is one of the busiest day sfor us gastroenterologists here in manila.

Monzur bhai invited me to his brother-in-law's wedding in Birmingham and I went to meet him, finally!

I Won't Give Up

 

I wanted to post a picture and tell you all that I'm still around. I deleted a lot of my pictures today because I'm not going to renew my pro account. I'm still keeping my Flickr account so I'll still be around.

I'm not sure where I left off but I have had more tests this past week. Lab tests to check my insulin since Polycystic Ovarian Syndrome can cause insulin resistance. I went to a sleep study doctor yesterday and he seems to think I may have sleep apnea. It was funny, they got me in last night to do a sleep study. That was weird but I'm so exhausted, I slept right through the whole night. When I got up this morning to use the restroom I couldn't believe all the wires I had hooked up to me. I looked like I was a robot or something. I'll get the results next week on both tests. I have my follow up with my gastroenterologist tomorrow. I was joking with my boss that I can't get a smart phone because all my money goes to doctors appointments. LOL!!

Anyway, I want to thank everyone for all the well wishes and concerns. Thanks for the messages and for everyone that asks mom about me. Well, I'm off to bed for now.

I'd like to thank each and every one of you for the well wishes, you are awesome contacts :) I'm pleased to report that I am doing much better. I've been following doctor's orders and taking it easy and sticking to a bland diet. I'm going to see a gastroenterologist Monday morning. He'll be taking a look at my stomach (lucky guy) and I'll know more after that visit. In the meantime, I hope everyone is enjoying their weekend and I'll see you on Flickr.

I wanted to take a few minutes to update everyone on what has been going on in my life. As most of you know, I got a new job back in March. Two weeks ago I was promoted from the part-time administrative assistant to a full-time office manager. I love my job and the company I work for. It is great to know your company is doing great things to improve the lives of adults with disabilities. We are expanding and growing rapidly. Yeah, for finally finding something I enjoy and a place I enjoy going to.

As some of you know, who are contacts with my mom, Happy Gram, I was in the emergency room last week on Tuesday night and in to Wednesday morning. I'm doing so much better today. Yesterday was my first day back to work and my first follow up appointment. Working was hard but I was only there a half day and my appointment was frustrating. So today I went and had a 2nd opinion from yesterdays visit and it was so much better. To make a long story short they found a cyst on my ovary but it is small and not what was causing all the pain. Tomorrow I go to a gastroenterologist to check for ulcers or if there is problems with my gall bladder. The emergency room doctor said it could be the start of gall bladder problems. So the last week I have been poked and prodded more times than I care to be. I have bruises on my arms from so many needles. I am in less pain and looking forward to this weekend, we are going camping.

Keeping Me Company

 

Wednesday the 3rd was a long day. Dominic had to be in San Antonio for an early appointment at the gastroenterologist’s office for a breath test so we left at 6:30 am. It was an early release day so I let him skip the whole day of school, especially since we had to wait there for three hours in between tests. Since I wasn’t going to be home, I told Anna she could skip too. Curtis texted me about 7:30 am saying that Anna really wanted to go to Life Bridges and had gotten herself dressed. So I contacted transportation and they said they could still pick her up. I was so proud of her. After she got home that afternoon, I invited her to come with me while I got a pedicure. Unfortunately they couldn’t work her in too but she was great company!

Texas A&M Aggie Ring '84 is going to the gastroenterologist with me next week for a little “procedure.” Actually, A&M College (Rescue) Ring '42 will be with us as well, serving as the "Reserve" Aggie Ring that I proudly wear on a leather strap around my neck. Both of them will be serving primary duty as my "Ring Men."

 

Any of you folks reading this who are over 50 probably know what kind of procedure I’m talking about…

 

However, Aggie Ring '84 has admonished me to not be a “Candy Ass” and have the doctor use anesthesia or mind-numbing drugs. That’s right, Aggie Ring '84 wants me to undergo the entire procedure without any drugs! Aggie Ring says, “Anesthesia is for pussy willows or West Point or VMI graduates who can't take a little pain like a man! A real Texas Aggie doesn’t need sedation for a simple little procedure!” "What about Citadel graduates?" I asked Aggie Ring. "Oh..." Aggie Ring replied after a pregnant pause... "Citadel grads do this sort of thing to each other all of the time! They're used to it."

 

I spoke to the doctor about not using drugs and he said that it wouldn’t be a problem though there might be some pain and discomfort during the procedure. I asked Aggie Ring about this and he replied, “I guarantee you that it won’t hurt as much as a good old-fashioned Texas A&M quad party or ass-beating with the business end of an axe handle!!!” "Listen up Ag!" said the Aggie Ring, "Do you THINK that they had pain killers or anesthesia at the Alamo? NO! They DIDN'T!"

 

Aggie Ring then said, "You need a 'SAFE' word to yell out during the procedure to let the doctor know you are experiencing real pain and not just grunting and moaning."

My Aggie Ring is so smart! "What word should I use?" I asked the little Aggie Ring. "Well... (he cogitated a bit) Supercalifragilisticexpialidocious has always been a favorite of mine. You could use that for a 'SAFE' word!"

 

"So..." I asked the little Aggie Ring. "If I'm lying there and the pain gets too bad I just yell out 'Supercalifragilisticexpialidocious?'" "That's the plan." said the Aggie Ring.

 

Aggie Ring asked me, “Do you mix this stuff up with bourbon or tequila?”

 

The gut-brain connection is no joke; it can link anxiety to stomach problems and vice versa. Have you ever had a "gut-wrenching" experience? Do certain situations make you "feel nauseous"? Have you ever felt "butterflies" in your stomach? We use these expressions for a reason. The gastrointestinal tract is sensitive to emotion. Anger, anxiety, sadness, elation — all of these feelings (and others) can trigger symptoms in the gut.

 

The brain has a direct effect on the stomach and intestines. For example, the very thought of eating can release the stomach's juices before food gets there. This connection goes both ways. A troubled intestine can send signals to the brain, just as a troubled brain can send signals to the gut. Therefore, a person's stomach or intestinal distress can be the cause or the product of anxiety, stress, or depression. That's because the brain and the gastrointestinal (GI) system are intimately connected.

 

This is especially true in cases where a person experiences gastrointestinal upset with no obvious physical cause. For such functional GI disorders, it is difficult to try to heal a distressed gut without considering the role of stress and emotion.

 

Gut health and anxiety

 

Given how closely the gut and brain interact, it becomes easier to understand why you might feel nauseated before giving a presentation, or feel intestinal pain during times of stress. That doesn't mean, however, that functional gastrointestinal conditions are imagined or "all in your head." Psychology combines with physical factors to cause pain and other bowel symptoms. Psychosocial factors influence the actual physiology of the gut, as well as symptoms. In other words, stress (or depression or other psychological factors) can affect movement and contractions of the GI tract, make inflammation worse, or perhaps make you more susceptible to infection.

 

In addition, research suggests that some people with functional GI disorders perceive pain more acutely than other people do because their brains are more responsive to pain signals from the GI tract. Stress can make the existing pain seem even worse.

 

Based on these observations, you might expect that at least some patients with functional GI conditions might improve with therapy to reduce stress or treat anxiety or depression. And sure enough, a review of 13 studies showed that patients who tried psychologically based approaches had greater improvement in their digestive symptoms compared with patients who received only conventional medical treatment.

 

Gut-brain connection, anxiety and digestion

 

Are your stomach or intestinal problems — such as heartburn, abdominal cramps, or loose stools — related to stress? Watch for these other common symptoms of stress and discuss them with your doctor. Together you can come up with strategies to help you deal with the stressors in your life, and also ease your digestive discomforts.

How Are the Gut and Brain Connected?

These two organs are connected both physically and biochemically in a number of different ways.

 

The Vagus Nerve and the Nervous System

 

Neurons are cells found in your brain and central nervous system that tell your body how to behave. There are approximately 100 billion neurons in the human brain (4Trusted Source).

 

Interestingly, your gut contains 500 million neurons, which are connected to your brain through nerves in your nervous system (5Trusted Source).

 

The vagus nerve is one of the biggest nerves connecting your gut and brain. It sends signals in both directions (6Trusted Source, 7Trusted Source).

 

For example, in animal studies, stress inhibits the signals sent through the vagus nerve and also causes gastrointestinal problems (8Trusted Source).

 

Similarly, one study in humans found that people with irritable bowel syndrome (IBS) or Crohn’s disease had reduced vagal tone, indicating a reduced function of the vagus nerve (9Trusted Source).

 

An interesting study in mice found that feeding them a probiotic reduced the amount of stress hormone in their blood. However, when their vagus nerve was cut, the probiotic had no effect (10Trusted Source).

 

This suggests that the vagus nerve is important in the gut-brain axis and its role in stress.

 

Neurotransmitters

 

Your gut and brain are also connected through chemicals called neurotransmitters.

 

Neurotransmitters produced in the brain control feelings and emotions.

 

For example, the neurotransmitter serotonin contributes to feelings of happiness and also helps control your body clock (11Trusted Source).

 

Interestingly, many of these neurotransmitters are also produced by your gut cells and the trillions of microbes living there. A large proportion of serotonin is produced in the gut (12Trusted Source).

 

Your gut microbes also produce a neurotransmitter called gamma-aminobutyric acid (GABA), which helps control feelings of fear and anxiety (13Trusted Source).

 

Studies in laboratory mice have shown that certain probiotics can increase the production of GABA and reduce anxiety and depression-like behavior (14Trusted Source).

 

Gut Microbes Make Other Chemicals That Affect the Brain

 

The trillions of microbes that live in your gut also make other chemicals that affect how your brain works (15Trusted Source).

 

Your gut microbes produce lots of short-chain fatty acids (SCFA) such as butyrate, propionate and acetate (16Trusted Source).

 

They make SCFA by digesting fiber. SCFA affect brain function in a number of ways, such as reducing appetite.

Gut microbes also metabolize bile acids and amino acids to produce other chemicals that affect the brain (15Trusted Source).

 

Bile acids are chemicals made by the liver that are normally involved in absorbing dietary fats. However, they may also affect the brain.

 

Two studies in mice found that stress and social disorders reduce the production of bile acids by gut bacteria and alter the genes involved in their production (19Trusted Source, 20Trusted Source).

 

Gut Microbes Affect Inflammation

 

Your gut-brain axis is also connected through the immune system.

 

Gut and gut microbes play an important role in your immune system and inflammation by controlling what is passed into the body and what is excreted (21Trusted Source).

 

If your immune system is switched on for too long, it can lead to inflammation, which is associated with a number of brain disorders like depression and Alzheimer’s disease (22Trusted Source).

 

Lipopolysaccharide (LPS) is an inflammatory toxin made by certain bacteria. It can cause inflammation if too much of it passes from the gut into the blood.

 

This can happen when the gut barrier becomes leaky, which allows bacteria and LPS to cross over into the blood.

 

Inflammation and high LPS in the blood have been associated with a number of brain disorders including severe depression, dementia and schizophrenia (23Trusted Source)

Scientists call this little brain the enteric nervous system (ENS). And it’s not so little. The ENS is two thin layers of more than 100 million nerve cells lining your gastrointestinal tract from esophagus to rectum.

The ENS may trigger big emotional shifts experienced by people coping with irritable bowel syndrome (IBS) and functional bowel problems such as constipation, diarrhea, bloating, pain and stomach upset. “For decades, researchers and doctors thought that anxiety and depression contributed to these problems. But our studies and others show that it may also be the other way around,” Pasricha says. Researchers are finding evidence that irritation in the gastrointestinal system may send signals to the central nervous system (CNS) that trigger mood changes.

 

“These new findings may explain why a higher-than-normal percentage of people with IBS and functional bowel problems develop depression and anxiety,” Pasricha says. “That’s important, because up to 30 to 40 percent of the population has functional bowel problems at some point.”

 

New Gut Understanding Equals New Treatment Opportunities

 

This new understanding of the ENS-CNS connection helps explain the effectiveness of IBS and bowel-disorder treatments such as antidepressants and mind-body therapies like cognitive behavioral therapy (CBT) and medical hypnotherapy. “Our two brains ‘talk’ to each other, so therapies that help one may help the other,” Pasricha says. “In a way, gastroenterologists (doctors who specialize in digestive conditions) are like counselors looking for ways to soothe the second brain.”

 

Gastroenterologists may prescribe certain antidepressants for IBS, for example—not because they think the problem is all in a patient’s head, but because these medications calm symptoms in some cases by acting on nerve cells in the gut, Pasricha explains. “Psychological interventions like CBT may also help to “improve communications” between the big brain and the brain in our gut,” he says.

I have been unable to post for awhile. I've been busy working and recuperating.

 

I was on a family vacation for a week (it was nice being able to go seeing as though I have been unable to for the past couple years due to work). While on vacation I was battling a horrible cold and cough... it went away for awhile, but now it is back again in full force. The cough isn't so bad this time around, but I have a throbbing headache and runny nose (it makes me feel OH SO glamorous). Cory thinks it has something to do with my allergies and my immune system during this time of year.

 

I've also been experiencing a flare up with my ulceratiive colitis. I have an appointment with my gastroenterologist to figure out our next plan of action. I'm pretty worn out and am just ready to find out how to keep this pain at bay. I feel like I'm always sick... it's draining.

 

August 10th marked Cory and I's 2 year anniversary. We weren't even able to spend it together (It was right when my family planned their vacation). But we celebrated upon my return. This past year has been a rough one, but we have come out on the other side of it hand in hand.

 

I had a show at Ella's Boutique in Portland. I wasnt really able to fully enjoy it due to this annoying flare up, frustrating cold, and work, but I'm happy to have shown there none the less.

 

I am behind in my blogs and website. Hopefully this marks me getting back in the habit. And hopefully all this pain goes away shortly...

I think Betty likes the hospital too much. My lovely, silly Mom is back in due to her pancreatitis.

 

She scored with a BIG, new room. It even has a HUGE tv that is also has web access. But for some reason flickr is blocked. So I promised her I'd let you all know she is currently "at the spa".

 

*Her new little friend is Dusty. Dr. Sprung, her gastroenterologist, got a kick out of Dusty. Said it kind of looked like a pancreas. We all got a chuckle out of it.

An examination and remembrance of a life at 40.

 

For the 40 days leading up to my 40th birthday, I intend to use my 365 Days project to document and remember my life and lay bare what defines me. 40 years, 40 qualities, 40 days.

 

Year 39: 2006-2007

 

At the beginning of October, 2006, Kurt and I were beside ourselves with excitement. Kurt's team, the Cardinals, and my team, the Tigers, would meet in the World Series. We joked about how romantic and perfect it was, and we trash-talked each other's teams, gently ribbing and teasing when someone made an error or struck out.

 

I would miss part of that series, however, because I was in the hospital. I'd had an episode of heart palpitations while driving to work. I'd always had occasional palpitations, even as a teenager, but they were always over just as soon as they'd begun. This time, they didn't stop. I felt my field of vision closing in front of me, so I pulled over to the side of the road, sure I was going to pass out. I thought, you've got to be kidding me. This is how I'm going to go? Sitting by the side of the highway during the morning commute to my soulless corporate cubicle? And then it stopped. I sat there for a minute, just breathing and making sure that it really was over, then got back on the road and drove to my office. When I got there, I told my boss what had happened, and that I needed to go to the hospital to get checked out.

 

When I walked into the emergency room and told them that I thought I was having heart trouble, they didn't even bother writing my name down before whisking me back into the treatment area. They hooked me up to every conceivable machine, did a CT scan, and took vials of blood to test. Nothing turned up. They suspected it was a combination of stress, lack of sleep, and having just finished a course of prescription decongestants, but they kept me overnight for observations anyway. They set me up in a room in the cardiac wing with wires hanging from my chest and abdomen. Nurses down the hall monitored my heart rate and blood pressure. The next morning, they did an echocardiogram (heart ultrasound). Still nothing. It was time to face facts, the doctor said. I was too stressed. I needed to chill out. I rolled my eyes. There was no way that was going to happen. In addition to the stressful job, I had also begun taking design and metalworking classes to indulge my interest in jewelry design. My schedule wasn't going to be getting any easier for months. Then, as if to mock his own admonition to relax, he informed me that the CT scan had turned up a mysterious mass on my liver. And thus began my weird health odyssey.

 

The doctors suspected that the mass on my liver was a giant hemangioma (basically a big, blood-filled, internal birthmark), but they needed to do an MRI to be sure. The MRI confirmed the diagnosis. I had a very large blood-filled sac on my liver, and if it broke open, I could die. Unfortunately, the surgery to remove it would be just as likely to kill me, so my best option was to simply make sure I never got hit really hard in the liver. Well, OK.

 

The MRI, in turn, had turned up something looking suspiciously like gall stones. I was immediately scheduled for a consultation with a gastrointestinal surgeon. The surgeon was iffy on whether or not to remove the gall bladder. If I didn't want kids, he said, it was a no brainer: leave it in. If I had any plans to get pregnant in the future, then it was a bit more complicated. If I had a gallstone attack while pregnant, it could be both excruciating and dangerous, but, he noted, also highly unlikely. I was having some symptoms that were somewhat consistent with gall stones, but he felt they were likely something more pedestrian, like IBS. He shoved me along to a gastroenterologist.

 

The gastroenterologist (who for all the world looked exactly like a guy I once had a huge crush on) decided he'd better do a colonoscopy and endoscopy. I reported for "duty" and just as the doctor (who, did I mention, looked exactly like a guy I once had a huge crush on?) sat down to go to work, I gratefully slipped under the anesthesia. When I awoke, I had a vague memory of choking on something, and nothing else. Kurt stayed at my bedside while I slowly came out of the fog, breaking wind like a frat boy at a Mexican rodeo. Now that's love, I thought. The -oscopies didn't turn up anything, either, and so I walked away from my medical odyssey with nothing but a smaller bank account and a paranoid desire that nobody ever punch me in the liver.

 

That was not to be the end of my trials and tribulations, though. In late July, shortly before my brother was to come to Chicago for a visit, my apartment was invaded by bird mites. The closet in my bedroom featured a mysterious portal to the outside. It had been boarded up loosely and, other than some idle speculation as to what its original purpose was, I didn't think much about it. That spring, some pigeons had begun nesting there, and when the babies hatched it set up such a ruckus that I couldn't sleep whenever they were awake. I anxiously awaited the day the babies would leave the nest. Unfortunately, when they did, the mites that had been feeding on them, poured into the house through the portal in search of other food. They found me.

 

The next month was hell. Pest control could do nothing to stop them. Despite them having told me that they wouldn't be interested in biting me because I was not a bird, my body was covered, head to toe, with painful welts. I began to research the problem on the internet, and what I uncovered filled me with horror and hopelessness. Bird mites are notoriously hard to get rid of, seem almost random in their choice of target (often leaving others in the same house untouched), and impervious to most pest control chemicals. Their life cycle can be rapid, reproducing and multiplying after just days, or they can adjust to sub-optimal environments by slowing their life cycle. I felt trapped and doomed, and I needed to get out, so I just walked away. I took myself and the cats and some clothing (dryer heat is one of the few things that kills the mites outright) and left the apartment and almost everything I owned. I got the cats cleaned up at the vet, and then moved in with Don W., an old roommate and friend of mine from college. I got in the shower with my clothes on, stripped down in the hot water, put my clothes in a garbage bag, and enjoyed the feeling of being safe once again. I lived with Don W. for the next three months, until I was able to get a short-term lease on a furnished apartment near work. I was planning to move to Kentucky to be with Kurt in January.

 

Kurt, meanwhile, had plans of his own. My birthday happened to fall on Homecoming weekend that year, and he had something special in mind for a birthday gift. On Friday night (the night before my birthday), as we were getting into our sleeping bags in the tent, Kurt asked me to wait a moment before going to sleep. He had something he wanted to show me. He turned around and fumbled around in his bag in the corner of the tent. When he turned back to me, he was holding out a small white box, opened to reveal a ring. He put it in my hands and said, "I don't want to live another day having to introduce you as just my girlfriend." We cried and laughed and hugged until our eyes grew too droopy to see clearly, then snuggled back down into our sleeping bags and fell asleep.

 

Who am I?

 

I am moderately healthy, actually.

 

By all rights, I should be a complete wreck. The genetic odds are stacked against me: diabetes, heart disease, cancer, mental illness. The lifestyle odds are stacked against me. I'm fat and have spent many years abusing the hell out of body. And yet, I seem to have remained mostly untouched by the worst of what should have come to me by now. I'm probably living on borrowed time, but for now, it seems to be working.

Von Hippel-Lindau disease, or von Hippel-Lindau syndrome, is a rare genetic disorder characterized by visceral cysts, benign masses, and the potential for malignant transformation in multiple organ systems.

 

von Hippel–Lindau VHL, the manifestations of the syndrome are protean; therefore, imaging plays a key role in identification of abnormalities and subsequent follow-up of lesions.

 

von Hippel–Lindau (VHL) disease is a rare, inherited, multisystem disorder that is characterized by development of a variety of benign and malignant tumors.

 

The spectrum of clinical manifestations of the disease is broad, About 40 different lesions in 14 different organs have been described.

 

The diagnostic criteria for VHL disease include:

 

(a) more than one CNS hemangioblastoma,

 

(b) one CNS hemangioblastoma and visceral manifestations of VHL disease, and

 

(c) any manifestation and a known family history of VHL disease.

 

Although genetic testing is available, imaging plays a key role in the identification of abnormalities and their subsequent follow-up, in the screening of asymptomatic gene carriers, and in their long-term surveillance.

 

The importance of screening is emphasized because the lesions in VHL disease are treatable; thus, early detection enables more conservative therapy to be performed and may enhance the patient’s length and quality of life.

 

A multidisciplinary approach to screening is emphasized; the team, which is led by a geneticist, includes urologists, gastroenterologists, neurologists, ophthalmologists, and radiologists

 

Because VHL varies so widely, there is not a consistent set of symptoms in each person. Every incidence of the disease has its own diagnostic evaluation. Being multi -ordinal disorder getting help for treatment of a VHL person is pie in the sky.

   

My pennyworth experience with VHL

 

I have been suffering Von Hippel Lindau (VHL) Syndrome since my early childhood. This disorder results in excess blood flow due to hypoxia inducible factor (HIF) resulting in repeated tumor growths in different organs of my body. VHL is a lifetime disease. Patients need to be constantly checked and treated/operated for the tumors and cysts that develop at various sites in the central nervous system and visceral organs throughout their lifetime. Because of the complexities associated with management of the various types of tumours in this disease, treatment is multidisciplinary.

 

VHL, LIVER TRANSPLANT,MDR-TB , neurologic complications

Very often timely aggressive surgical intervention is the only cure. As a VHL liver transplant patient, I have undergone and 9 surgeries one brain tumor removal, recent cyber knife at Medanta on 18th June 2013 besides grappling with MDR-TB which was diagnosed in 2010.

 

I developed MDR-TB. Manifestation of my latent TB happened under immune compromised situation, confirmed by a radiological conference facilitated by Dr Randeep Guleria at All India Institute of Medical Sciences (AIIMS). I developed pulmonary, bone and lymph involvement, to such an extent that it gnaws my bones and I walk with help of a four-toed stick.

 

Why VHL and MDR TB

 

I often face the question regarding significance of prolonged TB treatment and VHL, as VHL is not directly linked with TB.

 

Pre- transplant evaluation of Whole Body PET City Scan mentions:

 

A subcentimetric size lymph node is seen in the right posterior triangle

 

region which shows markedly increased FDGuptake (1 significance):- dated 21/9/07

 

8th July 2008:- A tiny non FDG avid, subcentimetre sized, subpleural nodule is noted in the apicoposterior segment of left upper lobe. Compared to previous PET CT, the left lung nodule is a new finding.

 

Tuberculosis was not detected and my long strife without repository began. Disseminated MDR TB manifested under immunocompromised situation though currently not active.

 

This is significance of TB aside from VHL, underdiagnosis and lowered immunesystem:- lowered immune system makes it almost impossible to survive the pandemic, I’m lucky to respond but there’s no guarantee it won’t comeback.

Hence MRI of whole spine and brain/body (CT?) is required -bi annually (as decided by neurosurgeon) Plasma free metaephrines and urinary metaephrines by HPLC method not ELISA method- for tachycardia.

 

2.Opthalmic by retinal specialist check ups are required(bi- annually)

  

3.I am Currently undergoing gingival excision and treatment due to initial cyclosporine(immunosuppressant) treatment side-effects.

 

4.Livertransplant, biannual tests and my current immunosuppressant( Sirolimus) level going up for past year requiring monitoring.

 

5.CECT of neck and chest/ USG of neck, chest xray often required.

 

6.Bimonthly LFT,Sirolimus,RFT,CBC

 

7.Biannual CBC, LFT, Na, K, Bun, Creatinine, Uric Acid,Hb1C, Lipid profile, Chest Xray, Urine Routine/Microscopic, Urine Culture/ sensitivity, Ultrasound whole Abdomen +liver Doppler (MHV showing monophasic flow in the liver graft suddenly from triphasic requiring repeated test for explanation)

 

All this is Way too expensive for me to bear with my younger brother( 4 yrs younger) being the sole earning member.

 

My mother is just a pensioner which doesn’t even suffice our accommodation and victuals.

 

My experience tells me VHL is seldom understood, and discrimination against disease exists. I’m a survivor not sufferer, I guess my strife with a rare disease shall be taken in good stride and help me in my spirited fight against a lifelong disease.

 

I don’t flinch and Cyber knife now performed, I need to deal with other complexities; but lack of funds and understanding makes the feelings worse. Please help me live the joyous life with a rare disorder as I am a survivor and not merely a sufferer. I’ve not lost the verve to life- help me lead a very happy life with VHL.

Apparently I have a Mixed Type 5cm Hiatus Hernia! I have been suffering dysphagia and heartburn so my GP sent me for an Endoscopy to diagnose why.

 

This picture is one of two that accompany the report from the Gastroenterologist to my GP. They are from inside me, but apart from that, and that I did not enjoy them being taken, I know nothing about them. Does anyone feel like telling me what we're looking at?

Adulting

 

Curtis doesn’t love to take care of his preventative health, so two years after he turned 50, I finally nagged him enough that he scheduled his first colonoscopy. Dr. Arnold with Gastroenterology Consultants of San Antonio is our entire family’s gastroenterologist. Dr. Arnold is adorable. After my colonoscopy he told me that he found nothing of note except that I had the colon of Tim Duncan, lol. Curtis had a few polyps removed and was also diagnosed with diverticulosis, just like Dominic. He was instructed to repeat the colonoscopy in 3-5 years. Dominic is still having trouble with abdominal pain and is still losing weight. Dr. Arnold had him try another 2 week course of Xifaxan, an antibiotic that can sometimes resolve the issues with which Dominic is dealing. It didn’t help so we are awaiting the “what’s next” message from Dr. Arnold. Everyone, take care of yourselves… get that colonoscopy, get your annual checkup, get your mammograms!

Hello, this is the healthy skeptics video for Wednesday, February 20. First. I'm Steven Cherniski. If you're new to this blog, you can check out our credentials and experience that the healthy skeptics.com. We're going to start today's a healthy skeptics with the omission from yesterday. Yesterday we were talking about the treatment of inflammatory bowel disease that is a colitis Crohn's disease and irritable bowel syndrome. And I failed to mention the incredibly important research that has been done with elsevier. Uh, so let me share that with you now. If you look here at a study published all the way back in 2004, I will try to get it in frame so you can freeze frame and get the citation. Excuse me. Um, you'll want to take this down again because this is a study that you can print out and take to your doctor because you look at the results of this.

 

They gave people, they gave some people placebo. The other group, they gave 100 mil leaders of Aloe Vera twice a day. Uh, and that's not a lot, that's like a three ounces of aloe vera twice a day. So easy to, to ingest those Oliveira a standardized way to deliver it on the results were clinical remission and improvement in response in night or clinical remission in 30 percent of the patients, um, improvement in 11 others, uh, and, and a response, a positive response, but it wasn't rated as particularly improving, uh, at 14 of those people against placebo, which didn't do much at all. So again, a study published in a leading a Gi Journal, uh, and yet if you went to a gastroenterologist and asked him or her, if they have looked at the research about aloe vera helping with ulcerative colitis, they would probably look at you thinking that you were making stuff up.

 

And yet it's in their medical literature. Moving on, we're talking about, um, things that you really don't need. The first thing you don't need, you don't need a vitamin machine. This is showing up on my news feed every day, multiple times a day. Uh, so apparently people aren't buying this machine for a $99. And after you buy the $99 machine to make liquid vitamins, then you have to pay for all the little pods that go into the device. And this is just goofy. And how are they promoting this? Hopefully this promoted is by saying, uh, that we all are. The vitamins are poorly absorbed. The idea of making a pill, I hasn't changed in the last 30 years. Oh No. Since the 19 thirties. This is just as goofy. This is, this is not true, right? If you look at the advances that have been made in, in creating tablets, in pills, there is something now called CGMP regulations.

 

These are regulations by the FDA. All vitamin manufacturers must have been bug CGMP dictates that dissolution and disintegration time of tablets. The CGMP means you've got to have products that actually release in the gastrointestinal tract and deliver the nutrients that are on the label. This is, this is all advances that have happened in the last 20 years. So this is, they want to tell you that tablets and capsules are not absorbed. And that's just not true. So they want you to abide their Gizmo, which looks like this, right, for $99, and that's going to give you a better results than if you took a capsule, a tablet or a powder. They claim they claim absorption of eighty five to 90 percent. Now the implication here is that you're going to get a whole lot less than that by taking advantage of environments where they get these numbers 80 nine to 90 percent or the good news, they made it up because there's no data to support that.

 

Again, if you look at these, you buy these little pods for your $30 a month and by the way you have to subscribe so you get them every month whether you won or not. One of them that I thought was okay was there sleep pod? They're asleep, like kind of like that because if you want to get the sleep, you don't want to take a sleep aid. It's going to take you 45 minutes to to disintegrate and dissolve in your Gi. So if you want to take a, normally you would take addicted capsule which is going to dissolve very quickly, but a liquid argument that only liquid is going to dissolve even faster than a capsule. So maybe if you got money to burn and you want to spend $100 for a Gizmo and then another $30 for a sleep aid, you can do that. But look at their energy product product.

 

Sorry, I keep missing the hearing. Their energy at 200 milligrams of caffeine, that's too strong. Comes of coffee. And look at the claims that it helps to manage stress. The government's going to help you manage stress. How does our work? So I think we're done with that right now. What else do you not meet? You know what I mean? Why Bizos right? And here's why. Here's a company that's heavily promoting their liposome technology. Go live on and read their logo. We deliver nutrients directly to yourself. No thank you. No, thank you. I'd rather have nutrients delivered in my body. The way body was designed to deliver nutrients, which is primarily through the clearinghouse of the liver. I really don't want anybody messing with the normal way that vitamins and minerals are metabolized and absorbed by my buddy. It's just goofy white biospheric. Vitamin C is the one that they're promoting.

 

Now you've seen it on your newsfeed. Life was with vitamin C and they're making all kinds of claims for what? Once again, once again, improved absorption, no big. They made claims for like, you know, 200 percent more vitamin C than possible with pills or powders and the the, the skeptical responses. So what if you look at vitamin C, here's this truth above addressee right? Seventy to 90 percent of vitamin C is absorbed at moderate intakes of 3,900 and eighty milligrams a day. However it does is about one gram a day. Absorption Falls to less than 50 percent.

 

That means your body's wisdom is if there's a moderate amount of vitamin C coming into the system, it absorbs it really well. Vitamin C, like most vitamins are absorbed in two ways by by, by attaching to every sector in the gut and a transport protein that then sends it off to the liver that's active absorption than vitamin C is also absorbed by passive diffusion, so even without a receptor there, if there's a higher concentration of vitamin C in your gut than there is in your bloodstream, that vitamin C will diffuse into the blood stream, but there are mechanisms that control that. That is clearly evident by the fact that at a moderate dose you get 90 percent of that will be absorbed by your body and metabolized by your body, but in gram quantity, so that's a thousand milligrams or higher, the absorption of the nutrient is remarkably reduced.

 

Why? Because your body doesn't want that much all at once. You look at vitamin C neutral, which I have, you know, the vitamin C in excess can be converted to dihydro vitamin C and can be, instead of being an antioxidant, it can be a pro oxidant. It can even contribute to the formation of hydrogen peroxide. Now, if you want to talk about, you know, some arcane way of of, of possibly helping with a chemotherapy regimen by also adding vitamin C to your ivy, that's a different story. I think that's intriguing and we're watching that right now, but in terms of a general daily use of vitamin C, that's just goofy. Anyway, this esteem Janesky for the healthy skeptics. What a wonderful tale we have today and sketch "by me", this being the story of Heull Gofish as you see him

in this sketch, gliding away from his newly opened "Organickus Island Resort and Very Exotic Foods Market".

This tale started about three hundred years ago, give or take a couple. When the young (213 years old),

Graduated from FSU, Fervor, Studies, University. It only had taken Huell, Seventy one years to graduate with

Doctorates, in every course offered, "but one". But he did narrow his career down to two, well maybe three special

things that interested him, those being a Gastroenterologist, (the study of foods that make you happy,

and make you fart), Architecture, and Alien female studies, which is the only course he did not complete,

after trying and studying for many years. Let's just say that is a story for another time.

After graduating with honors, and humongous student loans, all past due, Huell split in the Huell Glider,

to roam the universe, looking for the most exotic moons and planets, in search of alien foods,

and architecture, and possibly a girl friend, or two. The entire Gofish family was so happy when the

wild eyed boy returned, with a gleam in his big dark eyes, standing tall (four foot), and a business plan,

a big pile of Organickus cash, (which is redeemable on almost all planets and moons) very stable interstellar

government, and a sweet girl, he had meet and married on the Moon Organickus. Seems the family tradition

is when a daughter meets someone to take the parents off the hook for life long support, the husband

gets a pile of cash, (some Organickus women stay with the folks for a thousand years, or more).

Seems there is a male female Imbalance, on Organickus), about three hundred females, for every available Male.

But the Female Organickans, as they are known, through out the Galaxy, are great cooks and conversationalist.

A few years after 9/11, I heard a program on NPR about how women who were breastfeeding new babies during and after the time of the terrorist attacks went through a delayed mourning process. That the combination of having a new baby and nursing released something in their brain that wouldn’t allow them to fully digest the horrors of that day.

 

They didn’t feel sad, overwhelmed or angry during that September, instead they were hit with those emotions months later. As a result they felt alone and confused. Everyone else had been processing for quite some time.

 

I’ve been thinking about this a lot lately because I don’t remember feeling extraordinarily sad or overwhelmed during Olive’s first four months. Yes, it was a hard adjustment and of course there were tears. It’s true that I distinctly remember being resentful of all the hours I spent in her glider. But, looking back the emotions all feel soft and rounded.

 

Olive cried. A lot. More than the average newborn. I realize that now. At the time, no one would say, yes, you have a difficult baby. When you ask a doctor if your baby is crying more than average or spitting up more than an infant that age should, they will tell you it is all relative.

 

I remember sitting in the office of Olive’s gastroenterologist and asking him shyly but desperately if all newborns cried this much. “Well, all babies are different. It’ll get better when she’s three months old.”

 

I felt foolish for asking and foolish for thinking my experience as a new mother was any harder than anyone else’s so I went home and sucked it up believing that I wasn’t handling things well. My child didn’t cry anymore than anyone else’s, I just didn’t handle it as well as everyone else.

 

But she did cry more. I’ve been around enough babies and am friends with enough moms to know that I belong in a special club. The club where you don’t really enjoy the newborn months because you’re sleep deprived and ache from holding your baby for hour and hours and hours. The club where you don’t get to lose the baby weight because who can think about food or eating well when you have a new baby who is angry and sad all the time. The club where it takes you just a bit longer to bond with your child because it feels like the she hates you.

 

My friends are having their second children. I’m excited for them, abstractly. I like holding their newborns. I love shopping for shower gifts. But something happens when their babies spit up, just a little. Or when they cry and won’t stop, even if only for a short time. When they talk about how their babies won’t sleep or have to be held all the time…

 

Some people can sit around and reminisce about their child’s newborn days. The way they sniffed their baby’s head or watched them sleep peacefully. They will talk about how their baby looked up at them sweetly while nursing or how they felted bonded to them right away.

 

Meanwhile, I feel like I should be quarantined from anyone who is extremely pregnant or newly postpartum. I find myself saying things that you shouldn’t say to a new mom. I find myself feeling itchy and desperate and achy all over again as I listen to them talk about the newborn days. I find myself walking away from my friends thinking what the hell is wrong with me, why can’t I just be happy for them?!

 

I’ve spent a lot of time lately thinking about why I’m like this. And I try to put myself back in those early days and it feels so incongruous to the present or even to my memories of that time. I think that part of me refused to process my feelings when Olive was a newborn and now she’s so wonderful, so far beyond any expectations I could have ever had of her that it seems impossible that those months could have been so hard.

 

So it seems ironic that it took us until Olive was almost two to stop giving her a nighttime bottle. She, I believe, has been done with it for quite some time, but it was harder for us. We liked that she curled up with us in the glider and read book after book while she downed her milk. We liked the cuddles, the smell of her washed hair.

 

We liked reclaiming some of the sweet moments of babyhood that we didn’t enjoy when she was, in fact, still a baby.

Day Sixty-One~ Doctor Visit

 

Dominic was still not quite right with his tummy, so I decided it was time to see the pediatrician. She is running all kinds of blood panels, looking at food allergies, celiac disease, gluten intolerance, irritable bowel syndrome, thyroid panel, metabolic panel, and a CBC. We will get that done soon. She also referred us to a pediatric gastroenterologist. Whew. I feel like we are on the right track.

Aparently I have a Mixed Type 5cm Hiatus Hernia! I have been suffering dysphagia and heartburn so my GP sent me for an Endoscopy to diagnose why.

 

This picture is one of two that accompany the report from the Gastroenterologist to my GP. They are form inside me, but apart from that, and that I did not enjoy them being taken, I know nothing about them. Does anyone feel like telling me what we're looking at?

Abdominal Ultrasound for Dominic

 

Dominic is continuing to have tummy issues despite his gastroenterologist trying multiple approaches including medication and diet changes. He’s lost 30 pounds since January and doesn’t have much of an appetite. So Dr. Arnold ordered an abdominal ultrasound which was performed on Thursday. (I took mom to her post-op appointment in the morning and Dominic’s appointment in the afternoon.) No results yet, we are hoping to hear from the doc early this upcoming week. Prayers always appreciated.
