Currently the largest cargo hub in the UK, East Midlands Airport sees regular cargo flights by DHL which operates from the Cargo West perimeter of the airport. DHL Aviation operates a number of subsidiaries, as well as flights on behalf of the company that operates from East Midlands; these include Aerologic, ASL Airlines Hungary, ASL Airlines Ireland, Atlantic Airlines (as known as West Atlantic), Cargoair, DHL Air UK, Kalitta Air, MNG Cargo, Southern Air and Swift Air.

Not only does DHL have a major presence at East Midlands; the Royal Mail and UPS also have a major base, however the airport is also well-known for larger freighters appearing, namely during the Formula 1 season. Boeing 747's, Boeing 767's, Boeing 777's and Airbus A330's are the largest freighters to appear at East Midlands.

DHL Air UK provides much of the cargo flights by DHL Aviation, the airline operates a considerable fleet of Boeing 757-200 freighters (a mixture of freighter conversions by Boeing and by Precision Conversions). The long-haul flying is handled by their Boeing 767-300F's.

DHL Air UK operates some of the oldest Boeing 757's anywhere in the world. These very early Boeing 757-200's feature rare Rolls-Royce RB211-535C engines, the Rolls-Royce engines designed specifically for the Boeing 757. The Rolls-Royce RB211-535C engines were soon later replaced by the RB211-535E4 series (featuring the same core as the Rolls-Royce RB211-524 series), this was following pressure by Boeing for Rolls-Royce to produce an engine that produced the same or similar fuel burn as Pratt & Whitney's PW2037 engines also fitted on the Boeing 757... The Pratt & Whitney engines had around 8% better fuel burn compared to the Rolls-Royce RB211-535C engines. The RB211-535E4 engines still couldn't match the Pratt & Whitney's fuel efficiency, but was better at noise and reliability. A few Boeing 757's with Rolls-Royce RB211-535C engines were produced for British Airways and Eastern Airlines. The large portion of these rare Boeing 757's are now operated by DHL Air UK and EAT Leipzig.

Currently, DHL Air UK operates a large fleet of Boeing 757's which currently stands at 25, one Boeing 757-200 in the active fleet is currently stored at Jacksonville, Florida undergoing conversion to a freighter, whilst 2 not listed in the fleet are currently on-order, also undergoing conversion to freighters at Jacksonville, Florida.

Bravo India Kilo X-Ray is one of 25 Boeing 757-200F's in service with DHL Air UK, delivered new to British Airways (with the same registration) in March 1986 carrying Landor colours until 1997 when she gained Dutch-inspired Delftblue Daybreak World Tail. She was withdrawn by British Airways in June 2001 and was soon leased from BCC to DHL subsidiary, European Air Transport as OO-DPJ in December 2002 having been converted to a Boeing 757-200SF in Mobile, Alabama. European Air Transport later became EAT Leipzig and in March 2010 saw her registration changed to D-ALEI. A further transfer came in November 2015 where she transferred to DHL Air UK remaining on lease from BCC and regained her original registration she had gained since new to British Airways, G-BIKX. She is powered by 2 Rolls-Royce RB211-535C engines.
