Carla underwent cleft lip repair by the interplast Cusco, Peru surgical team last year, and was treated this morning for her cleft palate, which was wide open. It is common for patients to have a cleft lip repaired one year and a cleft palate the following year. By seperating the surgeries, Interplast doctors give the child time to heal and avoid keeping the patient under anesthesia for too long.

Eddie, 84, who has outlived his siblings despite smoking his whole life told me that he started smoking when he did his national service. He said that it was simply the culture then for someone to say "here, have a fag" and so he did, though he does regret it now. He told me that everyone smoked then, even the doctors during appointments. It was advertised as curing coughs and being good for the nerves but he told me that he hates to see young people smoking. Telling me that with all of the information about the dangers now, it makes no sense for someone to start smoking.
Eddie was a lovely guy to spend a few moments with and a privilege to make a portrait for him. Enjoy!
One-year-old girl held by her smiling mother. She was born with a bilateral cleft lip and cleft palate, and right microphthalmia (small orbit and undeveloped eye).
A typical surgical approach to helping this child would be:
1) to close her cleft lip first, to allow her to speak and eat better, then
2) return a few months later to close her cleft palate (hole in the roof of her mouth) through a more complex surgery, again to enhance speech and eating, plus limit ear, sinus, and respiratory infections,
3) remove the remnant of the existing right eye, perform an orbital (eye socket) reconstruction, and place a fat graft aimed to stimulate growth inside the orbital space,
4) place a "globe prosthesis" (space holder for the eye socket) as the child hopefully grows more over all, and
5) place a customized artificial polymer/acrylic eye, perhaps in time for her to begin school.
There is much hope for this intelligent, happy toddler!
FACE the Challenge, Inc. (FTC) is a humanitarian non-profit organization based in Centennial, Colorado, USA. Its all-volunteer teams and representatives have been traveling to various parts of the world to provide facial reconstructive surgeries free-of-charge.
Since 1994 FTC has been taking specialists to HCMc/Saigon to provide care for those with great needs. These long trips take about 40 hours of flight time (round-trip) and cover about 20,000 miles (round-trip). It still is gratifying and worth all our efforts!

"Cleft palate and low-set ears???" Despite being the most exhausted I had ever felt, I could not sleep, or think about anything else. Within the hour a team of professionals arrived talking and shining torches in her little mouth. She had her first surgery when she was 9 months old, but she was over 2 before she got her hearing aids. I never realised speech would be so hard for her. When I hear parents say "He/She never stops talking...I wish he/she would just shut up" or when I hear a 'normal' 7 year old speaking articulately my insides feel twisted tight and knotted like one of those thin cotton crinkle-shirts from Boden, that you have to wind round and round then tie to dry.

These images are cross sections of the upper lip taken by high resolution ultrasound in order to visualize the orbicularis oris muscles—colored in the images. Some of the muscles are smooth and continuous, some have breaks or other defects. We hypothesize that these defects represent a minor manifestation of the risk genes for cleft lip or cleft palate, and help to explain the reduced penetrance seen in families of individuals with these common birth defects.

The child had a cleft palate which has now been repaired by the wonderful team of Operation Smile Bangladesh Singapore Friendship Mission. The team does their best to make the patients feel as comfortable as possible! A child smiling on an OR Table is not something you see very often!

In this 3D image, given by ultrasound, Dr. Layyous provides an image of a fetal face. Many patients look forward to this scan which usually happens in weeks 18-20 (commonly referred to as the anatomy scan) as a way to see the developing fetus and check the anatomical features. In terms of the fetal face, the eyes, lips, skull, and jaws are imaged, ensuring that everything appears normal and there is no cleft lip.
Dr. James Wallace performed Carla's surgery and was assitanced by a local surgery resident who is training under the supervision of our host, Dr. Mario Cornejo. It's really amazing to be associated with an organization like Interplast the changes the lives of so many children like Carla.
In this Ultrasound, we have a 3D image that shows a fetal face. This image is brought to you by Dr. Najeeb Layyous. In this image, the baby in the womb appears to be resting against an arm. This image shows the facial chracteristics as still undergoing development and clarity. Part of the diagnostics in ultrasound and pregnancy are an evaluation of the baby's characteristics. These are carefully monitored during development. In terms of the fetal face, the eyes, lips, skull, and jaws are imaged, ensuring that everything appears normal and there is no cleft lip or cleft palate.
Today was clinic day, which (I’ve been told) is usually hectic. Mothers and fathers walked or traveled for hours with their children in hopes that the Interplast team might be able to help them. Most of them were dressed in their traditional clothing--brightly colored sweaters, skirts and hats. Dozens of children were screened, most with cleft lips and/or palates, some with polydactyl and others with burns. Many children that were treated last year by Interplast came back for follow up surgeries. Dr. Joseph Herbert, volunteer pediatrician from Santa Cruz, examines a young boy during clinic.
Baby "Hope" was brought to us by her Mother in Bazi, So. Sudan during a PHH Medical Clinic.
We were able to assist in funding a corrective surgery that was perfomed in the Congo.

Jeanette and Maja are the two translators/coordinators of the Interplast team in Nicaragua. Besides all the paperwork like registering patients and keeping their charts updated and ready for surgery, they are busy taking the patients to the operating room on time, consoling moms and dads waiting in the holding area and helping out with translation in the recovery room.
One of the best parts of the translator/coordinator job is entertaining both kids and parents after surgery with coloring books, candy and by taking Polaroid pictures.
Interplast has instituted very high standards in order to provide safe treatment to these children. Unfortunately, this means that some are turned away due to iron deficiencies and malnutrition. It was very difficult for the surgeons and other professionals to break the news to the families--they came so far, and had so much hope. However, most of the children were surgical candidates, and in the following days, I will share their stories. Dr. Jim Scott, volunteer anesthesiologist from Albuquerque, makes notes about the toddler on clinic day.

Andi lives on the Ignenio San Antonio in Nicaragua. When he was one, he was pushed into a hole filled with burning garbage, leaving him with a contracted right foot and several scars. In the past, he had difficulty walking, but thanks to Interplast surgeons, he can run with his friends
My name is Heidi Huntsman, from Salt Lake City, Utah. While I have served on the Interplast board since June 2004, this is my first Interplast trip. We are visiting Cusco, Peru, which sits high in the Andes—more than 11,000 ft! Most of the indigenous people here are descendants of the Incans, which ruled the region until the mid-1500s. The region is extremely poor and has very little access to health care. Even if they had services available to them, they couldn’t possibly afford to pay for these surgeries themselves. We are very grateful that so many medical professionals are willing to donate their time and give their services to people who have so little.

Here is an example of one of my cleft patients. Ortiz was born with a right
complete unilateral cleft lip and palate. This condition is more common on
the left side which makes right sided repairs more challanging. You can see
how involved the parents are in their child's care.

Children with cleft lips and cleft palates are frequently shunned and denied an education because of their appearance and speech impediments. In many countries, clefts carry significant stigmas.
One of our favorites is cute 1-year-old Dilan. He was born with a drooping left eyelid and his mom tells us she is so excited to see him after surgery (us too!!). Our head nurse Ruth is shown here just about to take him into the operating room.
