Spinal manipulation, which chiropractors call "spinal adjustment" or "chiropractic adjustment", is the most common treatment used in chiropractic care. Spinal manipulation is a passive manual maneuver during which a three-joint complex is taken past the normal range of movement, but not so far as to dislocate or damage the joint. Its defining factor is a dynamic thrust, which is a sudden force that causes an audible release and attempts to increase a joint's range of motion. High-velocity, low-amplitude spinal manipulation (HVLA-SM) thrusts have physiological effects that signal neural discharge from paraspinal muscle tissues, depending on duration and amplitude of the thrust are factors of the degree in paraspinal muscle spindles activation. Clinical skill in employing HVLA-SM thrusts depends on the ability of the practitioner to handle the duration and magnitude of the load. More generally, spinal manipulative therapy (SMT) describes techniques where the hands are used to manipulate, massage, mobilize, adjust, stimulate, apply traction to, or otherwise influence the spine and related tissues.

There are several schools of chiropractic adjustive techniques, although most chiropractors mix techniques from several schools. The following adjustive procedures were received by more than 10% of patients of licensed U.S. chiropractors in a 2003 survey: Diversified technique (full-spine manipulation, employing various techniques), extremity adjusting, Activator technique (which uses a spring-loaded tool to deliver precise adjustments to the spine), Thompson Technique (which relies on a drop table and detailed procedural protocols), Gonstead (which emphasizes evaluating the spine along with specific adjustment that avoids rotational vectors), Cox/flexion-distraction (a gentle, low-force adjusting procedure which mixes chiropractic with osteopathic principles and utilizes specialized adjusting tables with movable parts), adjustive instrument, Sacro-Occipital Technique (which models the spine as a torsion bar), Nimmo Receptor-Tonus Technique, applied kinesiology(which emphasises "muscle testing" as a diagnostic tool), and cranial. Chiropractic biophysics technique uses inverse functions of rotations during spinal manipulation. Koren Specific Technique (KST) may use their hands, or they may use an electric device known as an "ArthroStim" for assessment and spinal manipulations. Insurers in the US and UK that cover other chiropractic techniques exclude KST from coverage because they consider it to be "experimental and investigational." Medicine-assisted manipulation, such as manipulation under anesthesia, involves sedation or local anesthetic and is done by a team that includes an anesthesiologist; a 2008 systematic review did not find enough evidence to make recommendations about its use for chronic low back pain.

A 2009 study assessing chiropractic students giving or receiving spinal manipulations while attending a U.S. chiropractic college found Diversified, Gonstead, and upper cervical manipulations are frequently used methods.

Four Steps to Better Health Through Quality Chiropractic Care

 

If you decide to become a patient of a CBP doctor, there are four important steps we ask you to go through:

 

History of Injury and Examination,

Postural Analysis (some doctors utilize computerized methods such as the PosturePrint system)

Spinal X-rays (analyzed either by hand or with use of computer aided systems such as PostureRay),

Report of Findings - Where the doctor will explain your problems and if he/she can help through application of chiropractic services and the unique recommended plan for your case.

History of Injury and Examination

 

Our History of Injury is designed to determine whether your condition is within the realm of our care. Based upon the results, one of the doctors will make appropriate recommendations regarding an examination or referral to another doctor if your presenting problem is not a chiropractic condition. Most often, your condition will be treatable by our care.

 

A thorough spinal and musculo-skeletal examination will be performed in order to identify which structures of your body are injured and most likely causing your problem.

 

Computerized Postural Analysis

 

It has been known for years that body posture is like a “Window into the Shape of the Spinal Column”. Figure 1 shows that Abnormal Posture is always associated with an abnormal spine. Body Postural problems aid the doctor of chiropractic in identifying a variety of dysfunctions in the spinal column. Abnormal Postures can cause or relate to a number of health problems. Abnormal Posture causes increased force and pressure on the muscles, ligaments, and bones of your spinal column. These abnormal forces (caused by abnormal posture) lead to a break down of body tissues causing pain and damage.

Posture Digitization: Postural photographs of you the patient can be obtained, in which a sophisticated computer program known as PosturePrint will objectively measure your postural distortions. These photographs enable the doctor to detect and measure abnormal positions of the human skull, shoulders, ribcage, pelvis, knees, and feet. In other words, he or she can show you how far out of alignment your posture is. This helps explain why you may be hurting as well as aid the doctor in prescribing corrective postural-based exercises unique to CBP rehabilitative methods.

Spinal X-rays

 

Many patients often ask if a chiropractor should take x-rays of their spines. The answer is simple. Properly taken spinal x-rays are like a blue print of your spine. With x-rays, the chiropractor can see how an individual spine deviates from normal alignment and we can clearly decide on appropriate treatment. Without an x-ray they would only be guessing. Would you want a mechanic to work on your car engine without looking under the hood?

 

For example, the figure below is a sample of common abnormal neck alignment subluxations (viewed from the side) with an estimate on average time needed to rehab such problems. It is clearly evident the further your neck is from the normal, then the longer you will need to perform rehabilitative measures. Please note that these are only generic estimates, as many factors dictate how fast a patient responds such as age, the length of time the problem has been there, amount spinal degeneration, etc.

   

Another important finding that properly taken x-rays will identify is spinal arthritis. Many people are not even aware that spinal disc disease and bone arthritis are developing in their spines. For many people this is a “time bomb” waiting to explode. Figure 6 below shows an abnormal neck (viewed from the side) with arthritis in the mid neck.

Report of Findings

 

The report of findings is a very important part of communication in a CBP chiropractor's office. Before they decide if Chiropractic care is right for you, they will sit down with you (the patient) and explain the findings of the examination, posture analysis, and spinal x-rays. Many times patients tell the CBP chiropractor that other doctors never have shown them their spinal x-rays. This never happens at at a CBP practioner's office. We are confident that you will find that CBP trained chiropractors are very thorough in the search, identification and explanation of your spinal problem.

 

Once they have identified your particular spinal problem, the doctor will recommend an appropriate treatment plan specific to your condition.

Arthritis- Spinal & Other Joints

What Is Arthritis?

Arthritis is a relatively generic term that actually encompasses a variety of different conditions. There are a various types of arthritis. Some of the more common are described Table 1. Figure 1A shows relatively normal joints making up the cervical spine, or bones of the neck. However Figure 1B shows significant degeneration of the joints.

 

Who Does Arthritis Affect?

More than 27 million people are affected by osteoarthritis (OA), making it the most common type of arthritis in the U.S. OA of the knee and hips is the most common cause of arthritis-related disability in the U.S. Most people with OA are over the age 45. Overall, more women are more likely than ment o develop OA. Radiographic signs of OA are sometimes seen in people as young as their 20’s.

What Causes Arthritis?

While some forms of arthritis can be hereditary, others can be related to immune system dysfunction. The cause of many forms of arthritis is idiopathic, or unknown. Primary osteoarthritis, the most common form of arthritis, is referred to as “wear-and-tear” arthritis, because it is known that the joints can wear out faster when used more or inappropriately. It is sometimes assumed that arthritis is a “normal” consequence of aging. However, not all people within any advanced age group develop arthritis. There are other factors that can lead to the development of osteoarthritis. For example, if there is a condition whereby your joints are under increased pressure during regular daily activities, those joints are likely to degenerate at an advanced rate.

Examination Procedures for the Diagnosis of Arthritis

While the diagnosis of arthritis is often assumed by the history and exam, sometimes other tests are ordered. This can include blood tests, usually if rheumatoid or another variant is a possibility, and X-rays of the involved and related joints. X-rays can show signs of arthritis and help to delineate which form a person may have.

Common Chiropractic & Other Conservative Treatments

Pain from arthritis is commonly helped from Chiropractic care, including spinal adjustments, manual stretching, exercises and traction. Chiropractic Biophysics® care can help restore normal alignment of the spine and pelvis thereby taking pressure off of the joints. Diminishing this pressure may diminish or stop the rate of degeneration of the spine, hips and other related joints.

Chiropractic Care May Have a Positive Influence on Arthritis

Many thorough studies have been performed on patients with symptoms that are often related to arthritis, in particular osteoarthritis and have shown Chiropractic care to be beneficial.1-17

Chiropractic Biophysics® (CBP®) care, by correcting or improving the overall structure of the spine can have an additional positive benefit of diminishing abnormal pressure on the affected joints.
