On the left, the 6"x 8" Apollo 16 LM Lunar Surface Checklist, 75 double-sided pages, typed and edited and annotated by hand. Serial Number 001, Part Number SKB32100122-363
It has 13 tabbed sections including EVA 1 Prep, EVA 1 Post, EVA 2 (Prep and Post), EVA 3 (Prep and Post), Launch Prep, One Man EVA Prep, Emergency Liftoff, Manual Ascent, etc. Complete procedures, switch positions and checklists for each activity. Heavily used in training by Apollo 16 astronauts with quite a few annotations inside, especially near the back. The tabs are worn from use, but the pages are still nice condition. This copy was for use in the Lunar Module Simulator LMS-7.

Researching my Apollo 16 artifacts, I read through the flight transcript and found some powerful passages, especially near the end as the pressure gave way to playfulness:

I love how everything was done with typewriters back then.
This one went to the moon for three days, and covered the procedures upon returning from EVA 2, the second moonwalk. (This was the expedition with the clever lunar rover fender repair. Following the guidance from astronaut John Young at Mission Control, they rebuilt the fender using lunar maps and clamps from the optical alignment telescope lamp. )
EVA2 was also the trip to Shorty Crater where the famous "orange soil" was discovered. Shorty is actually an impact crater and the orange soil is an older volcanic deposit. They also used the traverse gravimeter there, and I have a build of that instrument on display as well.

It comes from the personal collection of Mission Commander Gene Cernan, the last man to walk on the moon. Apollo 17 was the first and only night launch of a Saturn V.

The Egyptian nightjar (Caprimulgus aegyptius) is a medium-small nightjar which occurs in south west Asia and north Africa, and winters in tropical Africa. It is a late migrant, seldom appearing before the end of April or beginning of May. It is a rare visitor to Europe, but, remarkably, has twice occurred as far away as Great Britain.
Open desert with a few trees or bushes are the haunts of this crepuscular nightjar. It flies at dusk, most often at sundown, with an easy, silent moth-like flight; its strong and deliberate wingbeats alternate with sweeps and wheels with motionless wings.

eBird Checklist: S28830880. Found by Robert Proniewych April 5th on the median by Oak Beach, Suffolk County, Long Island. I found it easily on April 9th, on a rainy, cloudy day, which may be why it's so hunkered down. I've never seen an Upie with his head so withdrawn!

This superb picture shows Al holding the vacuum-sealed Special Environmental Sample Container (SESC). Photographer Pete Conrad has just poured soil into the container. We can see that it is nearly full. The lid is hanging by a cord from the bottom. This photo gives us a good view of the top of Al's Hasselblad camera and the cuff checklist and watch on his left sleeve. Note also that reflections of Pete and the Hand Tool Carrier are visible in Al's visor. Pete took this photo during the stop at Sharp Crater. It is also cited in the comments at 121:31:45.

Conrad, who took this picture, is reflected in Bean's helmet visor. Conrad and Bean descended in the Apollo 12 Lunar Module (LM) to explore the lunar surface while astronaut Richard F. Gordon Jr., command module pilot, remained with the Command and Service Modules (CSM) in lunar orbit."
