What is culturally shocking to an American is that Peru seems to have no concept of centralized heat or hot water, even in the hotels in Lima in mid-winter, or in Cuzco at nearly 12,000-feet in winter. If people are cold, they put on warmer clothes or add more blankets to the bed. This is a classic Peruvian hot shower maker, installed for the benefit of tourists. It is a point-source electric water heater contained within a shower head. There is no temperature control. If you want really hot water, you reduce the volume of flow through the heater head; if you want greater flow, the water gets progressively cooler. It's odd, but it works well enough.
