Dr Plastic surgeon, immediately picked up on my insecurity about these "finger toes" & tried to make me feel better. He said, "What's wrong with long toes? I have long toes! But the damage had been done. He would either volunteer to show his shameful long toes or he would never see me in sandles again :)
this is nothing but the product of Sunday morning lemonade making with my sister. I thought it would be a funny and somewhat deep concept. I hope you think the same. :)

Kanchana is a bright young nurse in Sri Lanka. She graduated eleventh out of 30,000 nursing students in her country and is dedicated and excited to pursue her dream helping patients. Where she is today is a far cry from where she was a few years ago.
During her first year in nursing school, someone threw acid on her while she was on her way to class. The violent practice is common in her region of the world; most women are targeted for their beauty, out of jealousy, or for refusing romantic advances.
Not only did Kanchana have to undergo painful surgeries, hair loss and social stigma, but she also had to fight to stay in school, as the administrators thought that her gruesome appearance might disturb the other students, as well as the patients. Dr. Chandini Perera, ReSurge International's partner in Sri Lanka, performed many of her surgeries and fought for her to continue with her nursing studies.
Kanchana, now healed, works as a nurse at Perera’s hospital, after another fight with hospital administrators who believed her scars would disturb the patients. Kanchana’s smile and efficiency are doing just the opposite; she is telling her story when asked, and so many patients have stories of their own.
Kanchana is having an empowering effect on those she meets and is raising awareness about violence against women and the discrimination against those with disfigurement and disabilities.
Cisse, a 15-month-old baby girl from Koutala in the South of Mali was born with a cleft palate. She had a cleft of her soft palate (Veau 1) which would result in poor speech. Her mother cared for her other three children and Cisse eventually overcame her difficulty with feeding, which is a common problem associated with cleft palates. She is now at the ideal age to undergo repair of palate as she develops her speech, so I repaired her cleft palate (Furlow double apposing Z-plasty) and was assisted by Dr Coulibaly. The local surgeons watched the procedure closely in order to learn and hopefully get further training to offer cleft palate repairs in Mali.

I don't know what to make of this. My name, Seth, is very hard to pronounce for Chinese people, since there is no "th" sound in Chinese. As a result, the children say "Ses", which after much giggling and laughter, morphed into "Sars". SARS (severe acute respiratory syndrome) hit China like a ton of bricks a couple of years back, and everyone still remembers wearing surgical masks and avoiding public places. Since I go in and out of the operating room, I often have on one of the masks, the name sorta stuck.
I'm not sure if being nicknamed after a horrible infectious disease is inherently a good thing, but the kids get a laugh out of it, and so do I.

Well, their distraction of my obsession with my reconstuctive breasts worked. He and his nurse (disguised to protect the un-innocent--Val Bertoneli), kept me (in-stitches). The surgical precision and care they gave me with my reconstuction, adding implants and reconstructing the nipples was nothing but amazing. I had no visual to compare my results. I have a 17 year background in healthcare and pharmacology. I FELT GOOD about how they looked!
However, when I saw my perfectionist Dr.'s face (the symetry, the profile, the nipples--he kept a stoic face. After working with physicians for so long, I waited toI read his face. He was sooo pleased ! He walked toward me to explain the expanders gave some distortion and the reconstucted nipples are swollen and will----I cut him off! I looked at Valerie Bertinelli (his RN and surgical nurse) and smiled. She had her eyes red and almost losing a tear.
I KNEW I had the breasts after this long journey that would help me to feel like a woman again. My damn single 44 self! My physical self could move forward and I was so, so grateful.
I was and am grateful to him and all of the people who support him and his practice---and also supported me to make this process one that could help me with $ and the insurance and all those hassles. They allowed me to focus on the mental healing that I needed so much.

There are tons of these "Let The Harelip Children Smile Happily" posters all over the hospital. It isn't quite the preferred nomenclature (we prefer the term "cleft lip" to harelip, but their hearts are in the right place.

The hardest part of an Interplast trip is telling parents that we can't operate on their child. The parents often travel for hours or days at great expense because they trust Interplast to give their kid a normal life. Unfortunately, we can't help everyone.
As is obvious by the picture, this kid has multiple medical problems. Not only does he have a cleft lip and palate, but hs has no left ear and a misshapen skull. In cases such as these, the cleft lip is indicative of major neurological problems, and putting such a child under anesthesia risks death. We do not have a neurologist on our team, and since there are many lives we can change without such a risk and we only have a limited time, we had to turn him down.
It's so hard to tell a mother the reasons why we can't operate on their child, and watch their eyes get progressively teary and withdrawn as the translator explains. All we can do is concentrate on the kids we can help, and recognize that so many need our help.

Eight Days of Hanukkah … Eight Days of Tikkun Olam

This year I am celebrating Hanukkah in a different way. I have no menorah. I will not be eating latkes and apple sauce. But I am witnessing the most generous giving of gifts ever. I am in Nepal with a group of talented professionals who are restoring lives, giving the gift of renewal and hope, and inspiring others to do the same.
My travel companions are Sara Anderson, chief communications and advocacy officer for ReSurge International and Darcy Padilla, a brilliant award-winning documentary photographer and recent recipient of the prestigious W. Eugene Smith Grant for Humanistic Photography. We are being hosted by: Dr. Kiran Narkami, director of the ReSurge Nepal Surgical Outreach Program, an accomplished (and quite handsome) plastic and hand surgeon; Hemanta Dhoj Joshi, the program’s chief coordinator; and Dr. Shankar Man Rai, the visionary founder of what has become ReSurge International’s flagship outreach program.
Our first days of Hanukkah have been spent at the Kathmandu hospital clinic and visiting the surgical suite and post-op wards. The patients are benefactors of ReSurge’s program of free reconstructive surgery to those who could not otherwise afford it to repair congenital anomalies and restore mobility and form to victims of disfiguring, catastrophic burns.
Sara arrived bearing gifts for the staff: a dermatome, the instrument for use in procuring thin slices of skin from a donor area to produce skin grafts in a burn or other reconstructive surgical procedure, Flip video cameras for the team to use in tracking patient progress, and perhaps the most poignant gift of all, a Blue Tooth for Hemanta.
Hemanta is responsible for communicating with the patients’ families and organizing the logistics of bringing people to their surgical destinations. This might involve contacting local postal workers to connect to the postal workers in the remote villages of Nepal so that the postal workers can contact the patients and their families. This is often accomplished on his mobile phone while transporting passengers (such as us) through the crowded, unpaved streets of Kathmandu. The Blue Tooth will surely help reduce the risk of accidently hitting one of the many sacred cows that are free-roaming the streets of Nepal (a crime punishable by life imprisonment).
For me, this Hanukkah is about Tikkun Olam, or “repairing the world.” So I’m appealing to all of my Jewish friends and colleagues, or anybody who knows anybody who is Jewish, to give consideration to the many gifts you have to offer to repair the world. The gift of compassion is one that returns so much in so many ways; especially to the person who gives.

My plastic surgeon had made a bit of a dig at my long toes during a consult. I immediately became self-conscious because my sister (piggy toes) had always made fun of me. He could tell I was a bit uncomfortable as I told him my "childhood toes post traumatic stress disorder". Mys sister had teased me that I could play the piano with these toes. The dry-wit of my plastic surgeon was--well if you had started early enough!! Very funny...distract me from my breast reconstuction. I am on to you!

Many Americans have been making treks to Nepal since the 1950s, but our journey is not your typical holiday. Three of us are here to chronicle and observe how a few Nepalese medical professionals, with support from our organization, are restoring hope and function to thousands of children and adults each year through reconstructive surgery.
Because of the Maoist rebellion, few aid workers from our organization have crossed into Nepal in the last decade, but the political instability did not stop our Nepalese colleagues from dramatically changing the landscape of surgical care in their country. Violence from the rebellion created chaos and turmoil; yet, in the midst of its destruction, new hope emerged as they built medical capacity and delivered life-changing surgeries to children and to their parents who never dreamed that their children would walk again, talk, feed themselves and now go to school because their medical issues were now behind them.
It sounds like a miracle and in some ways it is, but underlying this resurgence of hope is the extraordinary vision and steadfast determination by a few humble Nepalese doctors to provide a better life to the poorest of their country. In the upcoming days, we will tell you more about these amazing colleagues and their courageous patients and their families. I hope you will join us by reading our posts – chronicling the transforming journey of these patients, our colleagues and what I am sure will be a transforming trek for us as well.

After a delicious breakfast of congee, pickled vegetables, marinated garlic and other assorted treats, we arrived at the hospital. We were warmly greeted by the hospital director and representatives of the China People's Welfare Foundation and the BeiBei Family Planning Association.
One thing I have learned very quickly here is that ceremony, bureacracy and status all go hand in hand. While this can be a time drag, it usually involves smiling and eating/drinking, so I don't mind.
