#CaseOfTheWeek:The NICU team at Nanavati Super Speciality Hospital managed

to discharge baby as small as 650 gms(23 weeks of gestation). He was

intubated and put on a mechanical ventilator for breathing support wherein

he received two doses of surfactant for lung maturity. The infant underwent

regular investigations for sepsis and electrolyte levels & received

transfusions of packed red blood cells, iron supplements and erythropoietin

for Anaemia. At the time of discharge, the baby weighed 2.7Kg and was on

breastfeed. He maintained adequate oxygen saturation by breathing normal

room air.

#CaseOfTheWeek: A 46-year-old male was brought to the casualty of Nanavati

Super Speciality Hospital. He complained of pain and deformity of his right

hip with an inability to bear weight. X-rays & CT scan confirmed the

clinical diagnosis of a hip dislocation and an associated fracture of the

femoral head. The patient was taken up the next day for fixation of the

fracture. The safe surgical dislocation approach as described by Ganz was

used. Patient was discharged on the fifth day & gradually progressed to

full weight bearing by three months. The fracture healed without any

complications.

Case Of The Week: A 56 year old man suffering from Orthopnoea for past 3-4

days was admitted to Nanavati Super Speciality Hospital. His examination

revealed a large mobile mass, thrombus (a blood clot formed in situ within

the vascular system of the body and impeding blood flow) in the right

atrium prolapsing into the right ventricle. Patient was taken up for

emergency surgery which lasts for 12 hour and a large tubular 30cmx2cm

thrombus was extricated from the right atrium. The patient recovered well

and was discharged by post operative day 10. The patient followed up after

3 months with near normal pulmonary artery pressures on 2D echo and is now

back to a normal life.
