Studio shot with two umbrellas. One above camera and one below camera. No fill (see how the shadow rounds off of her shoulder going dark. If I had used fill cards the shoulders would be lit or reflecting the fill. These are medium umbrellas and they are in really close. Perfect for speedlight photography as the proximity of the lights means that you don't need too much power. I would suggest two small umbrellas and matching speedlights at 1/4 or 1/8 power to start. Add fill cards to taste. Stir in hairlight as needed. Mini tutorial and setup shots here:

Photoshop... yeah, a little, but mostly the face and skin look like this when the light is brought in really close. I think I used some mamashan powder magic, but not much else.
