The good thing about this tool is that it helps you identify the problems, prompting you to call for the ostomy care if needed.

93 old Caucasian male, alert, oriented x 3 lives alone at home. Patient had bladder removal and Urostomy status in December 2009. Patient wife died few years ago. Patient is able to ambulate in the house with the help of a walker. Patient is weak and ambulation causes pain in right hip, leg and groin region. Patient describes the pain at level 8 (1-10 scale). Patient is on hydrocodone and fentanyl patch regimen for the management of pain. Patient is scheduled to start radiation therapy this month.

 

Assessment:

Urostomy site skin is intact and healthy. Stoma is red in color with clear urinary output. Currently patient is changing urostomy pouches every 5 days. Patient mentioned that self application of the urostomy pouches is becoming difficult now and it takes him several hours to apply the pouch. Patient is using following urostomy pouches:

Treatment:

Urostomy pouch was removed and new pouch was re-applied during the visit. Onchomycosis of the toe nails also noted. Bilateral feet toe nails were also trimmed.

 

Education:

Following subjects were discussed during the visit:

Urostomy pouch application, Peri-stoma skin care, Emptying of the appliance, Fall prevention, Infection control.

 

Recommendation:

•Home health care nurse to visit q weekly and assist with Urostomy care / pouch application and skin assessment.

•Patient needs assistance with ordering of the pouches and supplies.

•After urostomy, your urethra is not going to be used for urine excretion. Instead, it is going to be eliminated through urostomy.

•Without a sphincter muscle in the stoma, you are not going to be able to voluntarily control the excretion of your urine. And that’s where you are going to have to wear a pouch to collect your urine.

 

Types of urostomies

Urostomy can be created through different methods. The most common method is called an ileal conduit. The basics of this procedure are as under.

 

•A small portion of your small intestine (ileum) is removed surgically. This part is then used for the flowing out of urine.

•Intestine is then reconnected to allow it to perform its normal function.

•One end of the conduit is then closed by surgeon and ureters are inserted in it.

•Open end of the conduit goes through the abdominal wall. That serves as the exit point for the urine. The opening appearing on the outside of the abdominal wall is known as stoma.

 

Another type of ostomy is known as colostomy which is used for the removal of stool. This procedure is done to bypass the part of colon that is used for the stool to be given a way out of the body.

 

Measuring your stoma is one of the most crucial skills to know. Not only will it help you prevent leaks, but it will also allow you to extend the appliance wear time. It will ultimately help you save your #skin.

Some conditions that may result in the change of the size of the stoma include a prolapsing and parastomal hernia. • Mirror. It will be helpful if your stoma is away from the scope of vision due to any reason.

My ostemy rating 4.5 out of 5. The small shelf to place bags, wipes, etc is too high and behind the toilet seat and way off the wash-hand basin for changing bags standing up. Has coat hangers: been in a few that haven't. Would be good to have a chair in the room, either to sit on in front of basin or place accoutrements on.
