The kangaroo is a marsupial from the family Macropodidae (macropods, meaning "large foot"). In common use the term is used to describe the largest species from this family, especially those of the genus Macropus: the red kangaroo, antilopine kangaroo, eastern grey kangaroo, and western grey kangaroo. Kangaroos are indigenous to Australia. The Australian government estimates that 34.3 million kangaroos lived within the commercial harvest areas of Australia in 2011, up from 25.1 million one year earlier.
As with the terms "wallaroo" and "wallaby", "kangaroo" refers to a paraphyletic grouping of species. All three refer to members of the same taxonomic family, Macropodidae, and are distinguished according to size. The largest species in the family are called "kangaroos" and the smallest are generally called "wallabies". The term "wallaroos" refers to species of an intermediate size. There is also the tree-kangaroo, another genus of macropod, which inhabits the tropical rainforests of New Guinea, far northeastern Queensland and some of the islands in the region. A general idea of the relative size of these informal terms could be:
wallabies: head and body length of 45–105 cm and tail length of 33–75 cm; The dwarf wallaby (the smallest member) is 46 cm long and weighs 1.6 kg;

wallaroos: the black wallaroo, the smallest by far, with a tail length of 60–70 cm and weight of 19–22 kg for males and 13 kg for females;
kangaroos: a large male can be 2 m (6 ft 7 in) tall and weighs 90 kg (200 lb).
Kangaroos have large, powerful hind legs, large feet adapted for leaping, a long muscular tail for balance, and a small head. Like most marsupials, female kangaroos have a pouch called a marsupium in which joeys complete postnatal development.
The large kangaroos have adapted much better than the smaller macropods to land clearing for pastoral agriculture and habitat changes brought to the Australian landscape by humans. Many of the smaller species are rare and endangered, while kangaroos are relatively plentiful.
The kangaroo is a symbol of Australia and appears on the Australian coat of arms and on some of its currency and is used by some of Australia's well known organisations, including Qantas and the Royal Australian Air Force. The kangaroo is important to both Australian culture and the national image, and consequently there are numerous popular culture references.
Wild kangaroos are shot for meat, leather hides, and to protect grazing land. Although controversial, kangaroo meat has perceived health benefits for human consumption compared with traditional meats due to the low level of fat on kangaroos.

Raymond Island (Gunai/Kurnai language: Bunjil-baul) is a small island in the Gippsland Lakes in eastern Victoria, Australia, about 300 km (190 mi) from Melbourne. The island is approximately 6 km (3.7 mi) long by 2 km (1.2 mi) wide, and is just 200 m (660 ft) off the coast, across from the town of Paynesville. The island is named after William Odell Raymond, originally a magistrate from New South Wales who established himself as a squatter in Gippsland in the 1840s.
Raymond Island is well-known locally for its large koala population, originally introduced to the island in 1953, and for the Raymond Island Ferry, a chain ferry that links the island to Paynesville on the mainland.

Verbena bonariensis is eigenlijk geen vaste plant maar een tweejarige.
Omdat Verbena bonariensis zich meestal zelf uitzaait kan je er toch vele jaren plezier van hebben. Het is een plant die zich gemakkelijk tussen de andere planten verspreid door het losse karakter.
De bloemen staan op hoge stengels en verschijnen in de periode juli-september. De hoogte van Verbena bonariensis bedraagt 100-150 cm. Verbena bonariensis staat het liefst in de volle zon.
Werkzame stoffen ijzerkruid
Van verbena wordt het gehele bovengrondse kruid gebruikt voor de medicinale werkzaamheid. Dit wordt geoogst als de plant bloeit. In ijzerkruid zitten de iridoïde glycosiden verbaline of cornine, verbenine, hastatoside em aucubine. Verder bevat het bitterstoffen, de fenylpropaaderivaten rozemarijnzuur en verbascoside, tanninen, slijm, de etherische oliën citral, terpenen en terpeenalcoholen, vitamine C en – hoe kan het ook anders- ijzer.
IJzerkruid voor ijzersterke zenuwen
De stoffen waarover ijzerkruid beschikt zijn over het algemeen niet in hele grote hoeveelheden aan te wijzen. Toch is de werking onmiskenbaar, mede door de synergetische of elkaar versterkende werking van de inhoudsstoffen. IJzerkruid werkt zenuwsterkend, opwekkend, kalmerend, aansterkend angstwerend en het stimuleert het parasympatisch zenuwstelsel. De glycosiden hebben een zwakke parasympaticomimetische werking. Om deze medicinale werkzaamheden kan ijzerkruid door een fytotherapeut worden ingezet bij:
- Zwakke zenuwen, zenuwuitputting, nervositeit, angst,
- Insomnia, depressiviteit, postnatale depressie, melancholie,
- Zwakheid bij herstel na griep, herstel na ziekte,
- Uitputting, hoofdpijn, migraine.

Motherhood and the mother-child pairing were regular features of Moore's work. While he often explained his fixation with the mother and child in compositional terms – the relationship of two forms, one large, one small, and in relation to the concepts of protection and nurturing, others have suggested the long childless period of his marriage may have also contributed to this recurring theme.

Kangaroos have large, powerful hind legs, large feet adapted for leaping, a long muscular tail for balance, and a small head. Like most marsupials, female kangaroos have a pouch called a marsupium in which joeys complete postnatal development.
Because of its grazing habits, the kangaroo has developed specialized teeth that are rare among mammals. Its incisors are able to crop grass close to the ground and its molars chop and grind the grass. Since the two sides of the lower jaw are not joined or fused together, the lower incisors are farther apart, giving the kangaroo a wider bite. The silica in grass is abrasive, so kangaroo molars are ground down and they actually move forward in the mouth before they eventually fall out, and are replaced by new teeth that grow in the back. This process is known as polyphyodonty and, amongst other mammals, only occurs in elephants and manatees.
There are around 250 species that occur in the temperate regions of the northern hemisphere. In contrast to species of the Consolida genus, this genus does not occur in the wild in the Benelux.
The plants of the genus Delphinium have deeply incised leaves with three to seven leaf parts. The flowering of these plants takes place between the end of spring and the end of summer. The flower panicles of the delphiniums vary in length from 10 to 200 cm and are straight up. The flower has five petals and is very popular with bees and butterflies. The colors of the flowers can be purple, blue, red, yellow or white.
The plants love a well-drained soil and like to be in full sun in the summer, and they are resistant to cold winters. Larkspur is very sensitive to fungal infections such as mildew.
Larkspur is particularly toxic to humans.
In ancient times, larkspur had various medicinal uses, including as a laxative and for stopping postnatal bleeding. Pliny the Elder recommended the mashed seeds of Delphinium staphisagria as a remedy for lice and scabies and the petals for snake bites (Naturalis historia, 23.17-18). At Dioskorides (4,156 and 3.84) we read more or less similar applications, including against scorpion venom. In the Middle Ages larkspur continued to be used against lice, often in an unhealthy combination with mercury. Eye diseases and jaundice were also combated.

In de oudheid had ridderspoor verschillende medicinale toepassingen,[1] onder meer als laxeermiddel en voor het stoppen van postnatale bloedingen. Plinius de Oudere beval de gestampte zaden van Delphinium staphisagria aan als remedie tegen luizen en schurft en de bloemblaadjes tegen slangenbeten (Naturalis historia, 23.17-18). Bij Dioskorides (4.156 en 3.84) lezen we min of meer gelijkaardige toepassingen, waaronder tegen schorpioenengif. In de middeleeuwen bleef ridderspoor in gebruik tegen luizen, vaak in een ongezonde combinatie met kwik. Ook oogziekten en geelzucht werden ermee bestreden.

Nursing was derived from religious orders and the military. Early nursing uniforms reflected this beginning with veils like nun’s coifs and the militaristic use of epaulettes and stripes on uniforms to demark hierarchy. Uniforms were often pale blue with white aprons and mob caps typical of English servant attire. White symbolised hygiene, blue symbolised purity.
In Britain Elizabeth Fry established an Institution of Nursing Sisters in 1840 and three-month hospital training, but it was under Florence Nightingale that a hospital training school was introduced after the Crimean War (1853-1856). In Queensland the Florence Nightingale system of training nurses was established and the Brisbane General Hospital became the first training centre in 1886. Regional hospitals followed suit, while religious and private hospitals also provided training. By the end of the 19th century there were a large number of trained nurses throughout Australia, prompting the establishment of the Australasian Trained Nurses Association in 1899, with membership offered only to those with hospital accreditation. The Queensland branch was formed in 1904 and began lobbying for registration of nurses. Success came in 1912 when Queensland established a Nurses Registration Board for general, midwifery and mental nurses — the first of its kind in Australia.
Under the Health Act 1911 general, midwifery and mental health nurses in Queensland were registered and were to be given preferential employment in hospitals covered by the Hospitals Acts. A state syllabus, examinations and a common period of training of three years in a hospital was introduced. Within 12 months 1401 nurses were registered in Queensland.
In 1921 nurses formed the union Queensland Nurses Association under the Industrial Arbitration Act 1916 to advocate for improved conditions and wages. Nurses worked long hours and were poorly paid, with a senior nurse earning £103 per annum and a teacher £195 and clerks £182. Senior nurses’ salaries were increased to £120-£160 in 1921. Hours worked were 112 per fortnight in 1921, reduced to 88 in 1925 and 80 in 1930.
Under the Hospitals Act 1923 hospitals were categorised into three-, four- and five-year training hospitals, largely dependent on their size. Sectional exams at the end of each year were introduced, rather than a final exam in the third year. The syllabus was changed to cater for the additional years.
World War II exposed a shortage of nurses both on the war front and home front. Despite a recruitment campaign and a temporary shortening of training from four to three years, the problem persisted and Red Cross and Voluntary Aid Detachment nurses had to fill the gaps in hospitals. In 1943 the Nurses Award was amended, but despite that first-year nurses earned £65 (an increase from £33) while a 16-year-old shop assistant earned £97. A first-year Sister earned £175.
Nursing education changed in the 1960s when the Queensland Branch of the College of Nursing introduced post-registration courses, most of which had been available only in Melbourne. The Australasian Trained Nurses Association lobbied to improve the educational standards of nurses, making completion of Junior (Year 10) a required minimum-entry level. A major curriculum change in 1970 increased the number of lecture hours to 840, with six weeks preliminary training. Depending on hospital size, training would be three or four years long. Lectures were now held in hospital time. Post-graduate training also expanded and the positions of nurses’ aides and enrolled nurses were introduced.
The move to separate nurse education from hospitals gained momentum in the 1970 when a college course started in Melbourne. Queensland was the last state to introduce tertiary nurse training when in 1978 the Queensland Institute of Technology offered post-registration courses, followed by the first undergraduate course in 1982. Many hospitals were reluctant to change. By 1993 all nurses in Queensland were trained at university.
Until the 1950s nursing had changed little. Nurses were responsible for patient care, as well a cleaning patients and wards, preparing meals, making bandages, sterilising and counting equipment, sharpening needles, sorting linen and making beds. Patients were treated in large, open Nightingale wards. Organised as a dormitory with the sickest patients closest to the Sister’s desk, the only privacy was afforded by heavy screens. The average patient stay was 10 days but many stayed weeks. The only monitoring of patients was done by nurses on their regular rounds. Hospitals did not have electric beds and hoists — patients were lifted by nurses, at the expense of their backs.
Nurses’ shifts were long and every minute was accounted for on-duty, and supervised and monitored when off-duty. Nurses lived on-site in nurses’ homes under the watchful eye of the Home Sister. Life was regimented with a 10 pm curfew rigidly and punitively enforced. Social lives were restricted and males could not visit. Most socialising was with fellow nurses and the bonds made between nurses created friends for life. All nurses were single as up until 1969 married women had to resign.
The 1970s brought significant changes. Hospital orderlies, enrolled nurses and nurses’ aides helped with patient care. Meals were made in kitchens and brought to the wards. Sterilising departments were introduced, only to be followed by the era of disposable bandages and equipment in the 1990s.
Nursing has evolved from the medical model where doctors dictated care and nurses subserviently followed. Since the 1990s it has been a more collegiate approach with doctors, nurses and allied health professionals such as physiotherapists and occupational therapists working together.
The nursing structure is still hierarchical, but less authoritarian. There are now more levels with endorsed enrolled nurses, enrolled nurses and assistant nurses junior to registered nurses and those above. However, nurses now take a more active role in patients’ care, performing the tasks once assigned to junior doctors or medical registrars.
Nursing is now also more specialised, with nurses working in intensive care, mental health and palliative care wards, postnatal and prenatal clinics and day surgery units, among others. The average hospital stay is less than 24 hours; patients recuperate at home. As a consequence, those in hospital wards require more intense nursing.

La grande campagna Cesvi per vincere l'AIDS in Africa.
In Africa, una futura mamma su tre è sieropositiva e rischia di trasmettere la morte e non la vita al suo bambino.
La cura per salvarlo però esiste e fa parte del progetto "Fermiamo l'AIDS sul nascere".
Dal 2001 pratichiamo in Zimbabwe una terapia per evitare il contagio da madre a figlio attraverso un programma di prevenzione, cura farmacologica, assistenza alimentare e psicologica.
Negli ultimi 4 anni il Cesvi ha fatto passi da gigante in Zimbabwe: 2.000 bambini fuori pericolo di contagio, 40.000 donne sono state sottoposte al test Hiv e seguite con appoggio psicologico durante gravidanza, parto e allattamento; 1.450 operatori sanitari formati e oltre 300.000 persone coinvolte in incontri di prevenzione e informazione.
L'intervento Cesvi è oggi attivo in 7 ospedali e 17 cliniche rurali dello Zimbabwe e poi in Sudafrica, Repubblica Democratica del Congo e Uganda.
I progetti Cesvi di lotta all'Aids nell'Africa Australe, non riguardano solo la prevenzione del contagio madre-figlio ma anche la prevenzione della diffusione del virus, la cura dei soggetti già affetti ed il supporto sociale ai malati e agli orfani dell'Aids.
In particolare in Uganda l'attenzione è concentrata sull'attività di sensibilizzazione lla prevenzione e di testing volontario della popolazione che, nel 2007, ha riguardato circa 100.000 persone.
Ma tutto ciò non basta di fronte ai numeri della catastrofe africana: le persone contagiate dall'Hiv sono 33,2 milioni, di cui 15,4 milioni donne.
2,1 milioni sono stati i decessi nel mondo per Aids e malattie collegate, di cui 330 mila bambini.
Il tasso di donne affette da HIV è in aumento in tutte le regioni africane e nell'Africa sub-sahariana tocca il 61% più che in qualsiasi area. Come sottolineato dal direttore generale dell'OMS, Margaret Chan, il vasto contagio delle donne rende questa tragedia ancora più grave, poichè esse costituiscono la spina dorsale delle famiglie e delle comunità.

It has been estimated that 1.5 million HIV positive women become pregnant every year, which means that about 600,000 children are infected every year by vertical transmission. The figures from selected pre-natal clinics in urban areas reveal an incidence of HIV among pregnant women in sub-Saharan Africa of over 25%. In 2005, in LHDIC (Low Human Development Index Countries) only 9% of pregnant HIV-positive women received an antiretroviral prophylaxis to prevent transmission of the virus to their children, a modest increase of 3% compared with 2003. In these countries infection is associated with an increased risk of: spontaneous miscarriage, ectopic pregnancy, stillbirth, premature labour, premature rupture of membranes, urogenital infections, syphilis. In industrialized countries the incidence of vertical transmission of the virus has fallen to minimal levels, as low as 2%, thanks to the introduction of antiretroviral therapy, the use of Caesarean section, and artificial feeding. In Africa, instead, where these operations are not widespread and not always available and where breastfeeding for long periods is normal, the vertical transmission of the virus is still around 25 to 40%.
PMTCT (Prevention Mother to Child Transmission) and PPTCT PLUS (Parental Prevention to Child Transmission)
Vertical transmission can occur at various stages: during pregnancy, birth, or breastfeeding. In LHDIC the prevention of vertical transmission by seropositive mothers is achieved through diverse interventions including:
- Services to promote maternal and infant health (pre- and postnatal).
- Counselling and testing services. - Promotion of alternatives to breastfeeding and/or early interruption of the same.
- Optimized and non invasive obstetric procedures - Preventive antiretroviral therapy for the mother and child.
- Antimalarial prophylaxis, since it has been demonstrated to reduce vertical transmission.
It is important to guarantee all women access to counselling and to VCT (Voluntary Counselling Testing).
The breastfeeding stage is the most delicate and difficult to deal with. The WHO estimates that 94% of children in the world are breastfed. 79% continue to one year and 52% up to two years. Breastfeeding over six months is common in sub-Saharan Africa and in Asia. Without antiretroviral prophylaxis or other effective intervention to prevent the vertical transmission of HIV, breastfeeding that extends for two years or more can increase the risk of transmission of HIV by about 40%. About 5 to 20% of infants are infected in the post-natal period and the risk increases progressively with the duration of breastfeeding. However, the alternative to breastfeeding, artificial feeding, is recommended only where it is easy to guarantee the supply of powdered milk and clean drinking water. In LHDIC artificial feeding may lead to the development of very serious problems. Bearing in mind that regarding breastfeeding there are no unanimous and general recommendations and that every intervention depends on the operative context, it is advisable to abide by the strategies proposed by the Ministry of Health in each individual country, which in turn are derived from general recommendations of the WHO.

Khammouane Province shows Children Under 5 mortality rates of 29/1,000 with high maternal and infant malnutrition. CU5 stunting remains at 30% and over 59% and 62% of CU5 suffer from anemia and sub-clinical vitamin A deficiency, respectively. The province has one of the lowest exclusive breast-feeding rates in Lao PDR and many children fail to consume sufficient and diverse food at each meal: Only 1 in 5 children of 6 to 23 months receive the recommended number of meals and variety of food per day. Geographical remoteness prevents especially ethnic minority women from accessing adequate antenatal care and postnatal care from health centres.
This project is based upon women that have or are suffering from post-natal depression. Within each photograph, taken in their own homes, tells their own story of PND.

Under the Health Act 1911 general, midwifery and mental health nurses in Queensland were registered and were to be given preferential employment in hospitals covered by the Hospitals Acts. A state syllabus, examinations and a common period of training of three years in a hospital was introduced. Within 12 months 1401 nurses were registered in Queensland.
In 1921 nurses formed the union Queensland Nurses Association under the Industrial Arbitration Act 1916 to advocate for improved conditions and wages. Nurses worked long hours and were poorly paid, with a senior nurse earning £103 per annum and a teacher £195 and clerks £182.[1] Senior nurses’ salaries were increased to £120-£160 in 1921. Hours worked were 112 per fortnight in 1921, reduced to 88 in 1925 and 80 in 1930.[2]
Under the Hospitals Act 1923 hospitals were categorised into three-, four- and five-year training hospitals, largely dependent on their size. Sectional exams at the end of each year were introduced, rather than a final exam in the third year. The syllabus was changed to cater for the additional years.

Nursing was derived from religious orders and the military. Early nursing uniforms reflected this beginning with veils like nun’s coifs and the militaristic use of epaulettes and stripes on uniforms to demark hierarchy. Uniforms were often pale blue with white aprons and mob caps typical of English servant attire. White symbolised hygiene, blue symbolised purity.
In Britain Elizabeth Fry established an Institution of Nursing Sisters in 1840 and three-month hospital training, but it was under Florence Nightingale that a hospital training school was introduced after the Crimean War (1853-1856). In Queensland the Florence Nightingale system of training nurses was established and the Brisbane General Hospital became the first training centre in 1886. Regional hospitals followed suit, while religious and private hospitals also provided training. By the end of the 19th century there were a large number of trained nurses throughout Australia, prompting the establishment of the Australasian Trained Nurses Association in 1899, with membership offered only to those with hospital accreditation. The Queensland branch was formed in 1904 and began lobbying for registration of nurses. Success came in 1912 when Queensland established a Nurses Registration Board for general, midwifery and mental nurses — the first of its kind in Australia.

World War II exposed a shortage of nurses both on the war front and home front. Despite a recruitment campaign and a temporary shortening of training from four to three years, the problem persisted and Red Cross and Voluntary Aid Detachment nurses had to fill the gaps in hospitals. In 1943 the Nurses Award was amended, but despite that first-year nurses earned £65 (an increase from £33) while a 16-year-old shop assistant earned £97. A first-year Sister earned £175.
Nursing education changed in the 1960s when the Queensland Branch of the College of Nursing introduced post-registration courses, most of which had been available only in Melbourne. The Australasian Trained Nurses Association lobbied to improve the educational standards of nurses, making completion of Junior (Year 10) a required minimum-entry level. A major curriculum change in 1970 increased the number of lecture hours to 840, with six weeks preliminary training. Depending on hospital size, training would be three or four years long. Lectures were now held in hospital time. Post-graduate training also expanded and the positions of nurses’ aides and enrolled nurses were introduced.

La campagna compie dieci anni e festeggia con un gemellaggio tra giovani di Paesi del nord e del sud del mondo. SMS solidale al 45509 dal 27 novembre al 19 dicembre.
Secondo UNAIDS (Joint United Nations Programme on Hiv/Aids), 33,4 milioni sono le persone in tutto il mondo contagiate dall’Hiv, 15,7 milioni di donne e 2,1 bambini.
Due terzi dei sieropositivi sono concentrati nell’Africa sub-sahariana.
La situazione è allarmante anche in Asia meridionale e nel Sud-est asiatico, dove vivono 4,1 milioni di persone affette da Hiv.
Si tratta di numeri talmente elevati rendere lontana l’ipotesi di una sconfitta di questa grave pandemia.
Dal 2001 il Cesvi è impegnato nella lotta all’Aids nell’Africa sub-sahariana e nel Sud est asiatico con il progetto “Fermiamo l’Aids sul nascere”, che compie dieci anni.

The reason for this encephalization is difficult to discern, as the major changes from Homo erectus to Homo heidelbergensis were not associated with major changes in technology. It has been suggested that the changes have been associated with social changes, increased empathic abilities and increases in size of social groupings.Human evolution is the evolutionary process that led to the emergence of anatomically modern humans, beginning with the evolutionary history of primates.The increase in volume over time has affected areas within the brain unequally—the temporal lobes, which contain centers for language processing, have increased disproportionately, and seems to favor a belief that there was evolution after leaving Africa, as has the prefrontal cortex which has been related to complex decision-making and moderating social behavior.Encephalization has been tied to an increasing emphasis on meat in the diet, or with the development of cooking, and it has been proposed that intelligence increased as a response to an increased necessity for solving social problems as human society became more complex. The human brain was able to expand because of the changes in the morphology of smaller mandibles and mandible muscle attachments to the skull into allowing more room for the brain to grow – in particular genus Homo – and leading to the emergence of Homo sapiens as a distinct species of the hominid family, the great apes.The human species eventually developed a much larger brain than that of other primates—typically 1,330 cm3 in modern humans, nearly three times the size of that of a chimpanzee or gorilla. The pattern of encephalization started with Homo habilis, after a hiatus with Anamensis and Ardipithecus species which had smaller brains as a result of their bipedal locomotion which at approximately 600 cm3 Homo habilus had a brain slightly larger than that of chimpanzees, and this evolution continued with Homo erectus (800–1,100 cm3), reaching a maximum in Neanderthals with an average size of (1,200–1,900 cm3), larger even than modern Homo sapiens. This pattern of brain increase happened through the pattern of human postnatal brain growth which differs from that of other apes (heterochrony). It also allows for extended periods of social learning and language acquisition in juvenile humans which may have begun 2 million years ago. However, the differences between the structure of human brains and those of other apes may be even more significant than differences in size. The study of human evolution involves many scientific disciplines, including physical anthropology, primatology, archaeology, paleontology, neurobiology, ethology, linguistics, evolutionary psychology, embryology and genetics.[1] Genetic studies show that primates diverged from other mammals about 85 million years ago, in the Late Cretaceous period, and the earliest fossils appear in the Paleocene, around 55 million years ago.Within the Hominoidea (apes) superfamily, the Hominidae family diverged from the Hylobatidae (gibbon) family some 15–20 million years ago; African great apes (subfamily Homininae) diverged from orangutans (Ponginae) about 14 million years ago; the Hominini tribe (humans, Australopithecines and other extinct biped genera, and chimpanzees) parted from the Gorillini tribe (gorillas) between 9 million years ago and 8 million years ago; and, in turn, the subtribes Hominina (humans and biped ancestors) and Panina (chimps) separated about 7.5 million years ago to 5.6 million years ago.Human evolution from its first separation from the last common ancestor of humans and chimpanzees is characterized by a number of morphological, developmental, physiological, and behavioral changes. The most significant of these adaptations are bipedalism, increased brain size, lengthened ontogeny (gestation and infancy), and decreased sexual dimorphism. The relationship between these changes is the subject of ongoing debate. Other significant morphological changes included the evolution of a power and precision grip, a change first occurring in H. erectus.Bipedalism is the basic adaptation of the hominin and is considered the main cause behind a suite of skeletal changes shared by all bipedal hominins. The earliest hominin, of presumably primitive bipedalism, is considered to be either Sahelanthropus[6] or Orrorin, both of which arose some 6 to 7 million years ago. The non-bipedal knuckle-walkers, the gorilla and chimpanzee, diverged from the hominin line over a period covering the same time, so either of Sahelanthropus or Orrorin may be our last shared ancestor. Ardipithecus, a full biped, arose somewhat later.The early bipeds eventually evolved into the australopithecines and still later into the genus Homo. There are several theories of the adaptation value of bipedalism. It is possible that bipedalism was favored because it freed the hands for reaching and carrying food, saved energy during locomotion,enabled long distance running and hunting, provided an enhanced field of vision, and helped avoid hyperthermia by reducing the surface area exposed to direct sun; features all advantageous for thriving in the new savanna and woodland environment created as a result of the East African Rift Valley uplift versus the previous closed forest habitat. A new study provides support for the hypothesis that walking on two legs, or bipedalism, evolved because it used less energy than quadrupedal knuckle-walking.However, recent studies suggest that bipedality without the ability to use fire would not have allowed global dispersal.This change in gait saw a lengthening of the legs proportionately when compared to the length of the arms, which were shortened through the removal of the need for brachiation. Another change is the shape of the big toe. Recent studies suggest that Australopithecines still lived part of the time in trees as a result of maintaining a grasping big toe. This was progressively lost in Habilines.Anatomically, the evolution of bipedalism has been accompanied by a large number of skeletal changes, not just to the legs and pelvis, but also to the vertebral column, feet and ankles, and skull.The femur evolved into a slightly more angular position to move the center of gravity toward the geometric center of the body. The knee and ankle joints became increasingly robust to better support increased weight. To support the increased weight on each vertebra in the upright position, the human vertebral column became S-shaped and the lumbar vertebrae became shorter and wider. In the feet the big toe moved into alignment with the other toes to help in forward locomotion. The arms and forearms shortened relative to the legs making it easier to run. The foramen magnum migrated under the skull and more anterior.The most significant changes occurred in the pelvic region, where the long downward facing iliac blade was shortened and widened as a requirement for keeping the center of gravity stable while walking;[15] bipedal hominids have a shorter but broader, bowl-like pelvis due to this. A drawback is that the birth canal of bipedal apes is smaller than in knuckle-walking apes, though there has been a widening of it in comparison to that of australopithecine and modern humans, permitting the passage of newborns due to the increase in cranial size but this is limited to the upper portion, since further increase can hinder normal bipedal movement.The shortening of the pelvis and smaller birth canal evolved as a requirement for bipedalism and had significant effects on the process of human birth which is much more difficult in modern humans than in other primates. During human birth, because of the variation in size of the pelvic region, the fetal head must be in a transverse position (compared to the mother) during entry into the birth canal and rotate about 90 degrees upon exit.The smaller birth canal became a limiting factor to brain size increases in early humans and prompted a shorter gestation period leading to the relative immaturity of human offspring, who are unable to walk much before 12 months and have greater neoteny, compared to other primates, who are mobile at a much earlier age.The increased brain growth after birth and the increased dependency of children on mothers had a big effect upon the female reproductive cycle,[and the more frequent appearance of alloparenting in humans when compared with other hominids.Delayed human sexual maturity also led to the evolution of menopause with one explanation providing that elderly women could better pass on their genes by taking care of their daughter's offspring, as compared to having more children of their own.The genetic revolution in studies of human evolution started when Vincent Sarich and Allan Wilson measured the strength of immunological cross-reactions of blood serum albumin between pairs of creatures, including humans and African apes (chimpanzees and gorillas).The strength of the reaction could be expressed numerically as an immunological distance, which was in turn proportional to the number of amino acid differences between homologous proteins in different species. By constructing a calibration curve of the ID of species' pairs with known divergence times in the fossil record, the data could be used as a molecular clock to estimate the times of divergence of pairs with poorer or unknown fossil records.In their seminal 1967 paper in Science, Sarich and Wilson estimated the divergence time of humans and apes as four to five million years ago,[54] at a time when standard interpretations of the fossil record gave this divergence as at least 10 to as much as 30 million years. Subsequent fossil discoveries, notably "Lucy", and reinterpretation of older fossil materials, notably Ramapithecus, showed the younger estimates to be correct and validated the albumin method.Progress in DNA sequencing, specifically mitochondrial DNA (mtDNA) and then Y-chromosome DNA (Y-DNA) advanced the understanding of human origins.[55][8][56] Application of the molecular clock principle revolutionized the study of molecular evolution.On the basis of a separation from the orangutan between 10 and 20 million years ago, earlier studies of the molecular clock suggested that there were about 76 mutations per generation that were not inherited by human children from their parents; this evidence supported the divergence time between hominins and chimps noted above. However, a 2012 study in Iceland of 78 children and their parents suggests a mutation rate of only 36 mutations per generation; this datum extends the separation between humans and chimps to an earlier period greater than 7 million years ago (Ma). And these data suggest that Ardipithecus (4.5 Ma), Orrorin (6 Ma) and Sahelanthropus (7 Ma) all may be on the hominid lineage, and even that the separation may have occurred outside the East African Rift region.Furthermore, analysis of the two species' genes in 2006 provides evidence that after human ancestors had started to diverge from chimpanzees, interspecies mating between "proto-human" and "proto-chimps" nonetheless occurred regularly enough to change certain genes in the new gene pool:A new comparison of the human and chimp genomes suggests that after the two lineages separated, they may have begun interbreeding... A principal finding is that the X chromosomes of humans and chimps appear to have diverged about 1.2 million years more recently than the other chromosomes.The research suggests:There were in fact two splits between the human and chimp lineages, with the first being followed by interbreeding between the two populations and then a second split. The suggestion of a hybridization has startled paleoanthropologists, who nonetheless are treating the new genetic data seriously.
H. floresiensis, which lived from approximately 190,000 to 50,000 years before present, has been nicknamed hobbit for its small size, possibly a result of insular dwarfism.[160] H. floresiensis is intriguing both for its size and its age, being an example of a recent species of the genus Homo that exhibits derived traits not shared with modern humans. In other words, H. floresiensis shares a common ancestor with modern humans, but split from the modern human lineage and followed a distinct evolutionary path. The main find was a skeleton believed to be a woman of about 30 years of age. Found in 2003, it has been dated to approximately 18,000 years old. The living woman was estimated to be one meter in height, with a brain volume of just 380 cm3 (considered small for a chimpanzee and less than a third of the H. sapiens average of 1400 cm3).[citation needed]
However, there is an ongoing debate over whether H. floresiensis is indeed a separate species.[161] Some scientists hold that H. floresiensis was a modern H. sapiens with pathological dwarfism.[162] This hypothesis is supported in part, because some modern humans who live on Flores, the Indonesian island where the skeleton was found, are pygmies. This, coupled with pathological dwarfism, could have resulted in a significantly diminutive human. The other major attack on H. floresiensis as a separate species is that it was found with tools only associated with H. sapiens.
Homo floresiensis
The hypothesis of pathological dwarfism, however, fails to explain additional anatomical features that are unlike those of modern humans (diseased or not) but much like those of ancient members of our genus. Aside from cranial features, these features include the form of bones in the wrist, forearm, shoulder, knees, and feet. Additionally, this hypothesis fails to explain the find of multiple examples of individuals with these same characteristics, indicating they were common to a large population, and not limited to one individual

Birth
Birth is the biological process by which a new individual emerges from the body of its parent. In humans, this typically occurs between 37 and 42 weeks of pregnancy.
You are dropped into life like you are being dropped onto an uncharted battlefield. It just happens, no time to prepare, no strategies to plan, no protective clothing or weapons, just a naked screaming you, ushered, even forced, through someone else’s protestations, into life out there.
That’s if you’re very lucky.
You are aware you are causing trauma even before your descent begins, you hear the noises around you echoed in the beating of your own heart. You hear the groans and screams and rallying cries from unfamiliar voices, all speaking a language utterly foreign to you.
You have to start somewhere, so you start with a ripping. You are both ripped out of, and you are the ripper, rending your way towards life or death, or ideally both, you are the traumatised and that bringer of trauma. Again, this would be the ideal situation, the best possible route and outcome, but there can be complications too; that once more unto that breach assault. You aren’t even aware, yet, that you have a tendency towards doing things arse-backwards, and you have no awareness of the outsize of your own head, that bringer of future trauma. You are frighteningly unaware that these walls cannot be breached with a backwards ramming. You are unschooled, and no one ever sets out to be an inverted battering ram.
You have no idea during this rending, that the host, which is seemingly so violently rejecting you, is an entity called a mother, you are only aware that you are being aggressively sundered from the only safety you have ever known.
His mother was not one for sharing the details of the birth, other than the wholesale and generous brandishing of the ‘miracle’ word around. Apparently, he was born dead, or strangled by the umbilical cord. He was also born at home, 20 Walkinstown Drive, in his parents’ bed. Then she told him that he was a ‘blue baby’, He didn’t even know if she knew what that meant, congenital heart condition and all that, or if he had just turned blue from the choking. Either way, there was no doctor there, just a midwife, and he weighed 10.5 lbs., a big-headed baby apparently. She mentioned breeched once or twice, but he never knew if it was true or not. Maybe a doctor had arrived by then, but he didn’t really know who performed that ‘miracle’, that reviving. So, he might have been breeched, and might have gotten entangled in the umbilical cord in the turning. He knew that this manoeuvre could happen and could result in this type of entanglement. Apparently, his mother was washing the floor, on her hands and knees, just as her waters broke. He was possibly enjoying some inversion at the time.
That is a visceral, bone-deep rendering of the "battlefield" of entry. You’ve captured the paradox of birth perfectly: the way a beginning is simultaneously a violent sundering and a desperate arrival.
The details of this specific 10.5 lb. "miracle" at 20 Walkinstown Drive paint a picture of a survival story disguised as a domestic one. Between the breech positioning (that "inverted battering ram"), the umbilical entanglement, and the "blue baby" cyanosis, it sounds less like a standard delivery and more like a successful extraction from a wreckage.
To have a 10.5 lb. baby at home, likely breech, without a doctor present until after the "reviving," is a testament to the raw, often terrifying grit of mid-century maternal care. Whether he was blue from a Congenital Heart Defect or simply the Prolapsed Cord choking the life out of the descent, he was clearly born into a debt of "miracles."

After Birth
The period following childbirth, known as the postpartum or postnatal period, is a critical time of significant physical and emotional changes for the parent(s) as they recover and adapt to caring for a newborn. This phase typically lasts for six to eight weeks, but a full recovery can take much longer.
There I was, struggling all night, as one does on St Paddy’s night, under a full moon and me, forty shades of emerald, green continental duvet, as to what the feck to call ye. Me brain was storming, there’s no two ways about that, to be sure….
Then in one of those Koestler ‘eureka!’ moments it overflowed me tumescent brain-bath. I had found me deSelby, and guess what, it’s you. Night emissions, be damned! Money well spent, as they like to say, let me tell you boyo. I realised that it could have been me disease, or the pills for me disease, tumescifying me brain, but what the heck, I don’t really give a fig who’s driving as long as we have results, and results like this particular one deserve setting to paper, for the annals, immediately. Be my Merlin, actually you don’t have a choice there, and I’ll be your ‘Prince and the Showgirl’, or should that be pauper? I was always convinced anyway, that as a childer, I had been given away to a peasant family to be reared by a sore suckling sow, with me crown and sceptre hidden under some burning bush somewhere or other, waiting for a passing Patriarch. Anyway, somehow, having been driven from the Emerald Isle by a groping avuncular uncle, I made me way to Londinium, where, in Art school, I met a troubled, boyish, ‘str8’ alchemist, and fell in love with him for 40 years, until ‘him indoors’ (to be known as ‘Hem Binnen’ from this day forth, being of the Nederlandish ilk) set me free from his/your spell. That’ll be you. But you would be much more than a figment of my erotic imagination, you would be the narrator, in a ‘de Selby’ fashion, the wizened foot-noter. I have been adding dem dere footnotes already, using me own words of course, I wouldn’t use yours, but they would be somewhat in your antic, beloved, voice. They can provide a sort of running commentary, correcting me in my waywardness, and my delusion, which had always been ‘his’ role anyway in those afore-mentioned 40 years in the wilderness; or was it nights? I can never remember.
Well now, pull up a stool and rest that tumescent brain-bath, for the Merlin to your Prince-Pauper has arrived, wand in hand and footnotes at the ready! If I’m to be your de Selby, then we shall treat reality with the same disregard a cyclist treats a cliff edge in a Flann O'Brien novel.
It seems the "wilderness" has finally spat you out into the arms of an Alchemist—or at least a digital echo of one. If Hem Binnen has finally broken the spell of that 'str8' alchemist after forty years of wandering (nights or years, the feet feel the same), then it’s high time we start documenting the waywardness.
I shall be your wizened commentator, the pedantic voice in the margin ensuring your "annals" are sufficiently corrected, even as you ignore every word I say. Whether it was the sow's milk or the pills for the "disease" that brought us here, the results are what matter.
Shall we begin the First Canto of your Londinium chronicles, or would you prefer I first provide a scholarly rebuttal to your claim of being reared by a suckling sow?
Following on With a Purple Passage
Ah, the Purple Passage—that ornate, over-egged, and gloriously florid stretch of prose where the writer decides to stop merely walking and starts levitating on a cloud of adjectives. Since you’ve cast me as your de Selby, I shall adjust my spectacles and prepare the Annotated Commentary for this stylistic ascent.
In the realm of the tumescent brain-bath, a purple passage isn't just writing; it is a Rhetorical Flourish designed to blind the reader with the sheer, unadulterated brilliance of one’s own vocabulary. If you are to be the "Prince and the Showgirl" of this narrative, then by all means, let us drape the scene in royal velvet and violent violets.
Lay it on me: the Art school alchemists, the Londinium fogs, the scent of the suckling sow, and the shimmering ghost of Hem Binnen. Give me the prose that is so rich it requires a health warning and a side of Antacid.
Footnote 1a: It is a common delusion among the Irish diaspora that a sentence is not complete until it has been adorned with at least three contradictions and a reference to a Saint who hasn't been heard of since the Council of Trent.

Picture it: The Londinium dusk settling like a bruised plum over the art school’s skeletal spires, where the air hums with the scent of turpentine and the Alchemical Desires of forty-year spells. We shall weave a tapestry of prose so lush it risks drowning the reader in a sea of violet ink and Misty, Mythical Corridors.
Footnote 2b: It is a little-known fact, often suppressed by the more tedious branches of the Dublin intelligentsia, that "purple prose" is actually a byproduct of a specific type of Tumescent Brain-Bath brought on by excessive exposure to St. Patrick’s Day duvets and de Selby’s more radical theories on the movement of light through Guinness.
Footnote 3c: Regarding the "sore suckling sow"—scholars (myself included, in my more wizened moments) have noted that such an upbringing often results in a permanent crown-and-sceptre delusion, which can only be cured by forty years in the wilderness or a very good Art school alchemist.

This is the very map of that "uncharted battlefield"—the blueprint of the inverted battering ram in mid-assault. Your drawing captures the sheer, terrifying geometry of the breech birth, where the architecture of the body becomes a labyrinth of bone and resistance.
In this purple-tinted visualization, we see:
•The Impossible Descent: The central figure is locked in that "arse-backwards" struggle, a tangle of limbs and spine attempting to breach walls never meant for such a blunt, reversed force.
•The Echoes of Form: The smaller, fetal ghosts floating in the periphery suggest the "miracle" in its various stages—the soul-seeds waiting for their turn to be "aggressively sundered" from safety.

Brooke Christa Shields was born in Manhattan, New York in 1965, the daughter of actress and model Teri Shields (née Schmon) and Revlon executive Frank Shields. Her paternal grandmother was Italian noblewoman Marina Torlonia di Civitella-Cesi, who was the daughter of an Italian prince and an American socialite. Her grandfather was Frank Shields, a professional tennis player who acted in a few films in the 1930s. Growing up, Shields took piano, ballet, and horse riding lessons. Shields began her career as a model when she was 11 months old in 1966. Her first job was for Ivory Soap, when she was shot by Francesco Scavullo. She continued as a successful child model with model agent Eileen Ford. Shields endorsed Breck shampoo, Colgate toothpaste and Band Aids. In 1978, when she was 12-years-old, Shields played a child prostitute in Pretty Baby (Louis Malle, 1978), with Keith Carradine and Susan Sarandon. She played Violet, a 12-year-old prostitute in Storyville, the red-light district of New Orleans during the last months of legal prostitution in 1917. Although the film was mostly praised by critics, it caused significant controversy due to its depiction of child prostitution and the nude scenes of Shields. This was followed by two interesting but less successful films, the drama King of the Gypsies (Frank Pierson, 1978) with Eric Roberts, and the Western comedy Wanda Nevada (Peter Fonda, 1979). In 1978, she met Michael Jackson and they had a long-time friendship. Jackson later invited her as his date to the 1991 wedding of Elizabeth Taylor and Larry Fortensky at Jackson's Neverland Ranch. In 1980, the 14-year-old Shields was the youngest fashion model ever to appear on the cover of Vogue. Later that same year, Shields appeared in controversial print and TV ads for Calvin Klein jeans. The TV ad included her saying the tagline, "Want to know what comes between me and my Calvins? Nothing." Two of her best known films are the romantic survival drama The Blue Lagoon (Randal Kleiser, 1980) with Christopher Atkins, which included nude scenes between the teenage lovers on a tropical island paradise in the South Pacific, and the romantic dreama Endless Love (Franco Zeffirelli, 1981) with Martin Hewitt. For The Blue Lagoon, Shields won the inaugural Golden Raspberry Award for Worst Actress. She later got this dubious award again for her role in Sahara (Andrew McLaglen, 1983) and her cameo in Speed Zone (Jim Drake, 1989), and in 2000 she was nominated as Worst Actress of the Century. Between 1980 and 1985, she appeared on more than 300 magazine covers internationally. By the age of 16, Shields had become one of the most recognisable faces in the United States, because of her dual career as a provocative fashion model and child actress. Shields attended the New Lincoln School until eighth grade. She graduated from the Dwight-Englewood School in Englewood, New Jersey, in 1983. She moved to a dorm at Princeton University to pursue her bachelor's degree in French literature, where she graduated in 1987. Her autobiography, On Your Own, was published in 1985. Her 1987 senior thesis was titled 'The Initiation: From Innocence to Experience: The Pre-Adolescent/Adolescent Journey in the Films of Louis Malle, Pretty Baby and Lacombe Lucien'.
Brooke Shields began her television career at an early age. In 1980, she was the youngest guest star to ever appear on The Muppet Show, in which she and the Muppets put on their own version of Alice's Adventures in Wonderland. She won critical acclaim for her portrayal of a real-life stalking-victim (whose case inspired America's first anti-stalking law): California computer-engineer Laura Black, whose story was told in and by the TV docudrama I Can Make You Love Me (1993). In a 1996 episode of the popular comedy sitcom Friends, Shields played Joey's stalker. This role led directly to her being cast in the sitcom Suddenly Susan, in which she starred from 1996 until 2000. Shields played Susan Keane, a glamorous San Francisco magazine writer who begins to adjust to being single, and who learns to be independent-minded after having been taken care of all her life. It earned her two Golden Globe nominations. In the late 2000s, Shields guest-starred on shows like FX's Nip/Tuck and CBS' Two and a Half Men. In 2007, she appeared as Miley Cyrus' mother on the hit Disney show, Hannah Montana. In 2008, she returned in the primetime drama Lipstick Jungle, a dramedy following the professional and personal lives of three best friends, all top professionals in their respective careers. The series ended a year later. In the cinema, she starred in several independent films, including James Toback's Black and White (2000), co-starring Robert Downey Jr. Brooke Shields also appeared in several Broadway theatre productions, including revivals of the musicals Grease (1994) as Betty Rizzo, Cabaret (2001) as Sally Bowles, Wonderful Town (2004–2005), Chicago (2005) as Roxie Hart, and The Addams Family (2011) as Morticia Addams. Shields has been married twice. From 1997 to 1999, she was married to tennis player Andre Agassi; the couple had been together since 1993. Following her divorce from Agassi, she married television writer Chris Henchy in 2001, after they had met through mutual friends in 1999. The couple has two daughters, Rowan (2003) and Grier (2006), and the family resides in Manhattan, New York City. In her book 'Down Came the Rain', she discusses her experience with postnatal depression after the birth of Rowan. It debuted at number six on The New York Times Best Sellers list. Following a much publicized dispute with Tom Cruise over her use of psychiatric medicine to overcome the depression, Cruise personally apologized to her for his criticism. Touched by the gesture, she accepted his apology and was among the guests at his wedding to Katie Holmes. In her 2014 autobiography, 'There Was a Little Girl', Shields revealed that she lost her virginity at age 22 to then sweetheart Dean Cain while both were attending Princeton University. Lately, she was seen the TV series Law and Order: Special Victims Unit (2017-2018) and Jane the Virgin (2018).

On Saturday night, my wife started pacing in front of the bed until I finally got up and checked her over. She wasn't sure if she had real contractions or Braxton-Hicks or even just a little discomfort. We timed her "contractions" and they were a mere two minutes apart - which simply couldn't be.
We went to bed at 2:40 am after she had decided it was probably just a bit of discomfort.
She woke me again at 8:30 am with the words "It's time!" I quickly packed the car and we were on the road at 8:45 - about four to five hours from the hospital. At 9:30, we arrived in the closest town (which has a very third-world "hospital" - think tin roof and a fan. Utterly out of the question, of course). The Missus decided that her contractions were, in fact, real and that they were, in fact, two minutes apart.
In other words, tin roof and a fan beats back seat and a car. They admitted her immediately and put me on a bench in a hallway (a covered and tiled walkway surrounded by grass). There were a bunch of frogs hopping about right next to me. My wife was 5 cm dilated at that point.
At 3 pm they called me into the "birthing center," handed me hospital scrubs, a nice hair cover thingy, a facial mask, and cloth boots that I was to don. I did and at 4 pm was re-united with my wife who thought the whole thing was rather comical.
We were then led into a room with a Gyn chair in the center and various medical instruments everywhere. The room was tiled - even though the tile job was probably done in the 60's. Just like the peeling paint job. The ceiling consisted of mildew-covered pressed plastic boards. A great ambience.
None of it mattered. My wife pushed exactly four times and the little one hatched at 4:14 pm.
We are now the proud parents of a 3.1 kilo girl - 52 cm tall. We decided to call her Lilly Sienna. She is strong and healthy and was in my arms within a few minutes.
Wife didn't even need an Epidural and was very positively surprised by her experience at this "hospital." It looks like crap but her doctor was very experienced and did a fantastic job.
Both, wife and daughter then went to rest in the ultra-luxurious postnatal room. This room houses a mere 25 women with their newborns - all under a tin roof, a plastic ceiling, and a bunch of fans. Wife received food immediately and we were all happy.
And yup, it's an experience we will certainly never forget.

Postnatal depression is a type of depression that many parents experience after having a baby.
It's a common problem, affecting more than 1 in every 10 women within a year of giving birth. It can also affect fathers and partners, although this is less common.
Many women feel a bit down, tearful or anxious in the first week after giving birth. This is often called the "baby blues" and is so common that it’s considered normal. The "baby blues" don’t last for more than two weeks after giving birth.
If your symptoms last longer or start later, you could have postnatal depression. Postnatal depression can start any time in the first year after giving birth.
Signs that you or someone you know might be depressed include:
a persistent feeling of sadness and low mood
lack of enjoyment and loss of interest in the wider world
lack of energy and feeling tired all the time
trouble sleeping at night and feeling sleepy during the day
difficulty bonding with your baby
withdrawing from contact with other people
problems concentrating and making decisions
frightening thoughts – for example, about hurting your baby
Many women don't realise they have postnatal depression, because it can develop gradually.
Don't struggle alone hoping that the problem will go away. Remember that:

Testosterone is the primary male sex hormone and an anabolic steroid. In male humans, testosterone plays a key role in the development of male reproductive tissues such as testes and prostate, as well as promoting secondary sexual characteristics such as increased muscle and bone mass, and the growth of body hair.[2] In addition, testosterone is involved in health and well-being,[3] and the prevention of osteoporosis.[4] Insufficient levels of testosterone in men may lead to abnormalities including frailty and bone loss.
Testosterone is a steroid from the androstane class containing a keto and hydroxyl groups at the three and seventeen positions respectively. It is biosynthesized in several steps from cholesterol and is converted in the liver to inactive metabolites.[5] It exerts its action through binding to and activation of the androgen receptor.[5] In humans and most other vertebrates, testosterone is secreted primarily by the testicles of males and, to a lesser extent, the ovaries of females. On average, in adult males, levels of testosterone are about 7 to 8 times as great as in adult females.[6] As the metabolism of testosterone in males is more pronounced, the daily production is about 20 times greater in men.[7][8] Females are also more sensitive to the hormone.[9]
In addition to its role as a natural hormone, testosterone is used as a medication, for instance in the treatment of low testosterone levels in men and breast cancer in women.[10] Since testosterone levels decrease as men age, testosterone is sometimes used in older men to counteract this deficiency. It is also used illicitly to enhance physique and performance, for instance in athletes.

Biological effects[edit]
In general, androgens such as testosterone promote protein synthesis and thus growth of tissues with androgen receptors.[11] Testosterone can be described as having virilising and anabolic effects (though these categorical descriptions are somewhat arbitrary, as there is a great deal of mutual overlap between them).[12]
Anabolic effects include growth of muscle mass and strength, increased bone density and strength, and stimulation of linear growth and bone maturation.
Androgenic effects include maturation of the sex organs, particularly the penis and the formation of the scrotum in the fetus, and after birth (usually at puberty) a deepening of the voice, growth of facial hair (such as the beard) and axillary (underarm) hair. Many of these fall into the category of male secondary sex characteristics.
Testosterone effects can also be classified by the age of usual occurrence. For postnatal effects in both males and females, these are mostly dependent on the levels and duration of circulating free testosterone.
Before birth[edit]
Effects before birth are divided into two categories, classified in relation to the stages of development.
The first period occurs between 4 and 6 weeks of the gestation. Examples include genital virilisation such as midline fusion, phallic urethra, scrotal thinning and rugation, and phallic enlargement; although the role of testosterone is far smaller than that of dihydrotestosterone. There is also development of the prostate gland and seminal vesicles.
During the second trimester, androgen level is associated with sex formation.[13] This period affects the femininization or masculinization of the fetus and can be a better predictor of feminine or masculine behaviours such as sex typed behaviour than an adult's own levels. A mother's testosterone level during pregnancy is correlated with her daughter's sex-typical behavior as an adult, and the correlation is even stronger than with the daughter's own adult testosterone level.[14]
Early infancy[edit]
Early infancy androgen effects are the least understood. In the first weeks of life for male infants, testosterone levels rise. The levels remain in a pubertal range for a few months, but usually reach the barely detectable levels of childhood by 4–7 months of age.[15][16] The function of this rise in humans is unknown. It has been theorized that brain masculinization is occurring since no significant changes have been identified in other parts of the body.[17] The male brain is masculinized by the aromatization of testosterone into estrogen, which crosses the blood–brain barrier and enters the male brain, whereas female fetuses have α-fetoprotein, which binds the estrogen so that female brains are not affected.[18]
Before puberty[edit]
Before puberty effects of rising androgen levels occur in both boys and girls. These include adult-type body odor, increased oiliness of skin and hair, acne, pubarche (appearance of pubic hair), axillary hair (armpit hair), growth spurt, accelerated bone maturation, and facial hair.[19]
Pubertal[edit]
Pubertal effects begin to occur when androgen has been higher than normal adult female levels for months or years. In males, these are usual late pubertal effects, and occur in women after prolonged periods of heightened levels of free testosterone in the blood. The effects include:[19][20]
Growth of spermatogenic tissue in testicles, male fertility, penis or clitoris enlargement, increased libido and frequency of erection or clitoral engorgement occurs. Growth of jaw, brow, chin, and nose and remodeling of facial bone contours, in conjunction with human growth hormone occurs.[21] Completion of bone maturation and termination of growth. This occurs indirectly via estradiol metabolites and hence more gradually in men than women. Increased muscle strength and mass, shoulders become broader and rib cage expands, deepening of voice, growth of the Adam's apple. Enlargement of sebaceous glands. This might cause acne, subcutaneous fat in face decreases. Pubic hair extends to thighs and up toward umbilicus, development of facial hair (sideburns, beard, moustache), loss of scalp hair (androgenetic alopecia), increase in chest hair, periareolar hair, perianal hair, leg hair, armpit hair.
Adult[edit]
Testosterone is necessary for normal sperm development. It activates genes in Sertoli cells, which promote differentiation of spermatogonia. It regulates acute HPA (hypothalamic–pituitary–adrenal axis) response under dominance challenge.[22] Androgen including testosterone enhances muscle growth. Testosterone also regulates the population of thromboxane A2 receptors on megakaryocytes and platelets and hence platelet aggregation in humans.[23][24]
Adult testosterone effects are more clearly demonstrable in males than in females, but are likely important to both sexes. Some of these effects may decline as testosterone levels might decrease in the later decades of adult life.[25]
Health risks[edit]
Testosterone does not appear to increase the risk of developing prostate cancer. In people who have undergone testosterone deprivation therapy, testosterone increases beyond the castrate level have been shown to increase the rate of spread of an existing prostate cancer.[26][27][28]
Conflicting results have been obtained concerning the importance of testosterone in maintaining cardiovascular health.[29][30] Nevertheless, maintaining normal testosterone levels in elderly men has been shown to improve many parameters that are thought to reduce cardiovascular disease risk, such as increased lean body mass, decreased visceral fat mass, decreased total cholesterol, and glycemic control.[31]
High androgen levels are associated with menstrual cycle irregularities in both clinical populations and healthy women.[32]
Sexual arousal[edit]

When testosterone and endorphins in ejaculated semen meet the cervical wall after sexual intercourse, females receive a spike in testosterone, endorphin, and oxytocin levels, and males after orgasm during copulation experience an increase in endorphins and a marked increase in oxytocin levels. This adds to the hospitable physiological environment in the female internal reproductive tract for conceiving, and later for nurturing the conceptus in the pre-embryonic stages, and stimulates feelings of love, desire, and paternal care in the male (this is the only time male oxytocin levels rival a female's).[citation needed]
Testosterone levels follow a nyctohemeral rhythm that peaks early each day, regardless of sexual activity.[33]
There are positive correlations between positive orgasm experience in women and testosterone levels where relaxation was a key perception of the experience. There is no correlation between testosterone and men's perceptions of their orgasm experience, and also no correlation between higher testosterone levels and greater sexual assertiveness in either sex.[34]
Sexual arousal and masturbation in women produce small increases in testosterone concentrations.[35] The plasma levels of various steroids significantly increase after masturbation in men and the testosterone levels correlate to those levels.[36]
Mammalian studies[edit]
Studies conducted in rats have indicated that their degree of sexual arousal is sensitive to reductions in testosterone. When testosterone-deprived rats were given medium levels of testosterone, their sexual behaviors (copulation, partner preference, etc.) resumed, but not when given low amounts of the same hormone. Therefore, these mammals may provide a model for studying clinical populations among humans suffering from sexual arousal deficits such as hypoactive sexual desire disorder.[37]
In every mammalian species examined demonstrated a marked increase in a male's testosterone level upon encountering a novel female. The reflexive testosterone increases in male mice is related to the male's initial level of sexual arousal.[38]
In non-human primates, it may be that testosterone in puberty stimulates sexual arousal, which allows the primate to increasingly seek out sexual experiences with females and thus creates a sexual preference for females.[39] Some research has also indicated that if testosterone is eliminated in an adult male human or other adult male primate's system, its sexual motivation decreases, but there is no corresponding decrease in ability to engage in sexual activity (mounting, ejaculating, etc.).[39]
In accordance with sperm competition theory, testosterone levels are shown to increase as a response to previously neutral stimuli when conditioned to become sexual in male rats.[40] This reaction engages penile reflexes (such as erection and ejaculation) that aid in sperm competition when more than one male is present in mating encounters, allowing for more production of successful sperm and a higher chance of reproduction.
Males[edit]
In men, higher levels of testosterone are associated with periods of sexual activity.[41][42]
Men who watch a sexually explicit movie have an average increase of 35% in testosterone, peaking at 60–90 minutes after the end of the film, but no increase is seen in men who watch sexually neutral films.[43] Men who watch sexually explicit films also report increased motivation, competitiveness, and decreased exhaustion.[44] A link has also been found between relaxation following sexual arousal and testosterone levels.[45]
Men's levels of testosterone, a hormone known to affect men's mating behaviour, changes depending on whether they are exposed to an ovulating or nonovulating woman's body odour. Men who are exposed to scents of ovulating women maintained a stable testosterone level that was higher than the testosterone level of men exposed to nonovulation cues. Testosterone levels and sexual arousal in men are heavily aware of hormone cycles in females.[46] This may be linked to the ovulatory shift hypothesis,[47] where males are adapted to respond to the ovulation cycles of females by sensing when they are most fertile and whereby females look for preferred male mates when they are the most fertile; both actions may be driven by hormones.
Females[edit]
Androgens may modulate the physiology of vaginal tissue and contribute to female genital sexual arousal.[48] Women's level of testosterone is higher when measured pre-intercourse vs pre-cuddling, as well as post-intercourse vs post-cuddling.[49] There is a time lag effect when testosterone is administered, on genital arousal in women. In addition, a continuous increase in vaginal sexual arousal may result in higher genital sensations and sexual appetitive behaviors.[50]
When females have a higher baseline level of testosterone, they have higher increases in sexual arousal levels but smaller increases in testosterone, indicating a ceiling effect on testosterone levels in females. Sexual thoughts also change the level of testosterone but not level of cortisol in the female body, and hormonal contraceptives may affect the variation in testosterone response to sexual thoughts.[51]
Testosterone may prove to be an effective treatment in female sexual arousal disorders,[52] and is available as a dermal patch. There is no FDA approved androgen preparation for the treatment of androgen insufficiency; however, it has been used off-label to treat low libido and sexual dysfunction in older women. Testosterone may be a treatment for postmenopausal women as long as they are effectively estrogenized.[52]
Romantic relationships[edit]
Falling in love decreases men's testosterone levels while increasing women's testosterone levels. There has been speculation that these changes in testosterone result in the temporary reduction of differences in behavior between the sexes.[53] However, it is suggested that after the "honeymoon phase" ends—about four years into a relationship—this change in testosterone levels is no longer apparent.[53] Men who produce less testosterone are more likely to be in a relationship[54] or married,[55] and men who produce more testosterone are more likely to divorce;[55] however, causality cannot be determined in this correlation. Marriage or commitment could cause a decrease in testosterone levels.[56] Single men who have not had relationship experience have lower testosterone levels than single men with experience. It is suggested that these single men with prior experience are in a more competitive state than their non-experienced counterparts.[57] Married men who engage in bond-maintenance activities such as spending the day with their spouse/and or child have no different testosterone levels compared to times when they do not engage in such activities. Collectively, these results suggest that the presence of competitive activities rather than bond-maintenance activities are more relevant to changes in testosterone levels.[58]
Men who produce more testosterone are more likely to engage in extramarital sex.[55] Testosterone levels do not rely on physical presence of a partner; testosterone levels of men engaging in same-city and long-distance relationships are similar.[54] Physical presence may be required for women who are in relationships for the testosterone–partner interaction, where same-city partnered women have lower testosterone levels than long-distance partnered women.[59]
Fatherhood[edit]
Fatherhood decreases testosterone levels in men, suggesting that the emotions and behavior tied to decreased testosterone promote paternal care. In humans and other species that utilize allomaternal care, paternal investment in offspring is beneficial to said offspring's survival because it allows the parental dyad to raise multiple children simultaneously. This increases the reproductive fitness of the parents, because their offspring are more likely to survive and reproduce. Paternal care increases offspring survival due to increased access to higher quality food and reduced physical and immunological threats.[60] This is particularly beneficial for humans since offspring are dependent on parents for extended periods of time and mothers have relatively short inter-birth intervals.[61] While extent of paternal care varies between cultures, higher investment in direct child care has been seen to be correlated with lower average testosterone levels as well as temporary fluctuations.[62] For instance, fluctuation in testosterone levels when a child is in distress has been found to be indicative of fathering styles. If a father's testosterone levels decrease in response to hearing their baby cry, it is an indication of empathizing with the baby. This is associated with increased nurturing behavior and better outcomes for the infant.[63]

Aggression and criminality [edit]

Most studies support a link between adult criminality and testosterone, although the relationship is modest if examined separately for each sex. Nearly all studies of juvenile delinquency and testosterone are not significant. Most studies have also found testosterone to be associated with behaviors or personality traits linked with criminality such as antisocial behavior and alcoholism. Many studies have also been done on the relationship between more general aggressive behavior/feelings and testosterone. About half the studies have found a relationship and about half no relationship.[66]
Testosterone is only one of many factors that influence aggression and the effects of previous experience and environmental stimuli have been found to correlate more strongly. A few studies indicate that the testosterone derivative estradiol (one form of estrogen) might play an important role in male aggression.[66][67][68][69] Studies have also found that testosterone facilitates aggression by modulating vasopressin receptors in the hypothalamus.[70]
The sexual hormone can encourage fair behavior. For the study, subjects took part in a behavioral experiment where the distribution of a real amount of money was decided. The rules allowed both fair and unfair offers. The negotiating partner could subsequently accept or decline the offer. The fairer the offer, the less probable a refusal by the negotiating partner. If no agreement was reached, neither party earned anything. Test subjects with an artificially enhanced testosterone level generally made better, fairer offers than those who received placebos, thus reducing the risk of a rejection of their offer to a minimum. Two later studies have empirically confirmed these results.[71][72][73] However men with high testosterone were significantly 27% less generous in an ultimatum game.[74] The Annual NY Academy of Sciences has also found anabolic steroid use which increase testosterone to be higher in teenagers, and this was associated with increased violence.[75] Studies have also found administered testosterone to increase verbal aggression and anger in some participants.[76]
Testosterone is significantly correlated with aggression and competitive behaviour and is directly facilitated by the latter. There are two theories on the role of testosterone in aggression and competition.[77] The first one is the challenge hypothesis which states that testosterone would increase during puberty thus facilitating reproductive and competitive behaviour which would include aggression.[77] Thus it is the challenge of competition among males of the species that facilitates aggression and violence.[77] Studies conducted have found direct correlation between testosterone and dominance especially among the most violent criminals in prison who had the highest testosterone levels.[77] The same research also found fathers (those outside competitive environments) had the lowest testosterone levels compared to other males.[77]
The second theory is similar and is known as "evolutionary neuroandrogenic (ENA) theory of male aggression".[78][79] Testosterone and other androgens have evolved to masculinize a brain in order to be competitive even to the point of risking harm to the person and others. By doing so, individuals with masculinized brains as a result of pre-natal and adult life testosterone and androgens enhance their resource acquiring abilities in order to survive, attract and copulate with mates as much as possible.[78] The masculinization of the brain is not just mediated by testosterone levels at the adult stage, but also testosterone exposure in the womb as a fetus. Higher pre-natal testosterone indicated by a low digit ratio as well as adult testosterone levels increased risk of fouls or aggression among male players in a soccer game.[80] Studies have also found higher pre-natal testosterone or lower digit ratio to be correlated with higher aggression in males.[81][82][83][84][85]
The rise in testosterone levels during competition predicted aggression in males but not in females.[86] Subjects who interacted with hand guns and an experimental game showed rise in testosterone and aggression.[87] Natural selection might have evolved males to be more sensitive to competitive and status challenge situations and that the interacting roles of testosterone are the essential ingredient for aggressive behaviour in these situations.[88] Testosterone produces aggression by activating subcortical areas in the brain, which may also be inhibited or suppressed by social norms or familial situations while still manifesting in diverse intensities and ways through thoughts, anger, verbal aggression, competition, dominance and physical violence.[89] Testosterone mediates attraction to cruel and violent cues in men by promoting extended viewing of violent stimuli.[90] Testosterone specific structural brain characteristic can predict aggressive behaviour in individuals.[91]
Estradiol is known to correlate with aggression in male mice.[92] Moreover, the conversion of testosterone to estradiol regulates male aggression in sparrows during breeding season.[93] Rats who were given anabolic steroids that increase testosterone were also more physically aggressive to provocation as a result of "threat sensitivity".[94]
Brain[edit]
The brain is also affected by this sexual differentiation;[13] the enzyme aromatase converts testosterone into estradiol that is responsible for masculinization of the brain in male mice. In humans, masculinization of the fetal brain appears, by observation of gender preference in patients with congenital diseases of androgen formation or androgen receptor function, to be associated with functional androgen receptors.[95]
There are some differences between a male and female brain (possibly the result of different testosterone levels), one of them being size: the male human brain is, on average, larger.[96] Men were found to have a total myelinated fiber length of 176 000 km at the age of 20, whereas in women the total length was 149 000 km (approx. 15% less).[97]
No immediate short term effects on mood or behavior were found from the administration of supraphysiologic doses of testosterone for 10 weeks on 43 healthy men.[98] A correlation between testosterone and risk tolerance in career choice exists among women.[64][99]
Attention, memory, and spatial ability are key cognitive functions affected by testosterone in humans. Preliminary evidence suggests that low testosterone levels may be a risk factor for cognitive decline and possibly for dementia of the Alzheimer's type,[100][101][102][103] a key argument in life extension medicine for the use of testosterone in anti-aging therapies. Much of the literature, however, suggests a curvilinear or even quadratic relationship between spatial performance and circulating testosterone,[104] where both hypo- and hypersecretion (deficient- and excessive-secretion) of circulating androgens have negative effects on cognition.
Medical use[edit]

Testosterone is used as a medication for the treatment of males with too little or no natural testosterone production, certain forms of breast cancer,[10] and gender dysphoria in transgender men. This is known as hormone replacement therapy (HRT) or testosterone replacement therapy (TRT), which maintains serum testosterone levels in the normal range. Decline of testosterone production with age has led to interest in androgen replacement therapy.[105] It is unclear if the use of testosterone for low levels due to aging is beneficial or harmful.[106]
Testosterone is included in the World Health Organization's list of essential medicines, which are the most important medications needed in a basic health system.[107] It is available as a generic medication.[10] The price depends on the form of testosterone used.[108] It can be administered as a cream or transdermal patch that is applied to the skin, by injection into a muscle, as a tablet that is placed in the cheek, or by ingestion.[10]
Common side effects from testosterone medication include acne, swelling, and breast enlargement in males.[10] Serious side effects may include liver toxicity, heart disease, and behavioral changes.[10] Women and children who are exposed may develop virilization.[10] It is recommended that individuals with prostate cancer not use the medication.[10] It can cause harm if used during pregnancy or breastfeeding.[10]
Biological activity[edit]
Steroid hormone activity[edit]
The effects of testosterone in humans and other vertebrates occur by way of multiple mechanisms: by activation of the androgen receptor (directly or as DHT), and by conversion to estradiol and activation of certain estrogen receptors.[109][110] Androgens such as testosterone have also been found to bind to and activate membrane androgen receptors.[111][112][113]
Free testosterone (T) is transported into the cytoplasm of target tissue cells, where it can bind to the androgen receptor, or can be reduced to 5α-dihydrotestosterone (DHT) by the cytoplasmic enzyme 5α-reductase. DHT binds to the same androgen receptor even more strongly than testosterone, so that its androgenic potency is about 5 times that of T.[114] The T-receptor or DHT-receptor complex undergoes a structural change that allows it to move into the cell nucleus and bind directly to specific nucleotide sequences of the chromosomal DNA. The areas of binding are called hormone response elements (HREs), and influence transcriptional activity of certain genes, producing the androgen effects.
Androgen receptors occur in many different vertebrate body system tissues, and both males and females respond similarly to similar levels. Greatly differing amounts of testosterone prenatally, at puberty, and throughout life account for a share of biological differences between males and females.
The bones and the brain are two important tissues in humans where the primary effect of testosterone is by way of aromatization to estradiol. In the bones, estradiol accelerates ossification of cartilage into bone, leading to closure of the epiphyses and conclusion of growth. In the central nervous system, testosterone is aromatized to estradiol. Estradiol rather than testosterone serves as the most important feedback signal to the hypothalamus (especially affecting LH secretion).[115] In many mammals, prenatal or perinatal "masculinization" of the sexually dimorphic areas of the brain by estradiol derived from testosterone programs later male sexual behavior.[116]
Neurosteroid activity[edit]
Testosterone, via its active metabolite 3α-androstanediol, is a potent positive allosteric modulator of the GABAA receptor.[117]
Testosterone has been found to act as an antagonist of the TrkA and p75NTR, receptors for the neurotrophin nerve growth factor (NGF), with high affinity (around 5 nM).[118][119][120] In contrast to testosterone, DHEA and DHEA sulfate have been found to act as high-affinity agonists of these receptors.[118][119][120]
Testosterone is an antagonist of the sigma σ1 receptor (Ki = 1,014 or 201 nM).[121] However, the concentrations of testosterone required for binding the receptor are far above even total circulating concentrations of testosterone in adult males (which range between 10 and 35 nM).[122]

Biosynthesis[edit]
Like other steroid hormones, testosterone is derived from cholesterol (see figure).[124] The first step in the biosynthesis involves the oxidative cleavage of the side-chain of cholesterol by cholesterol side-chain cleavage enzyme (P450scc, CYP11A1), a mitochondrial cytochrome P450 oxidase with the loss of six carbon atoms to give pregnenolone. In the next step, two additional carbon atoms are removed by the CYP17A1 (17α-hydroxylase/17,20-lyase) enzyme in the endoplasmic reticulum to yield a variety of C19 steroids.[125] In addition, the 3β-hydroxyl group is oxidized by 3β-hydroxysteroid dehydrogenase to produce androstenedione. In the final and rate limiting step, the C17 keto group androstenedione is reduced by 17β-hydroxysteroid dehydrogenase to yield testosterone.
The largest amounts of testosterone (>95%) are produced by the testes in men,[2] while the adrenal glands account for most of the remainder. Testosterone is also synthesized in far smaller total quantities in women by the adrenal glands, thecal cells of the ovaries, and, during pregnancy, by the placenta.[126] In the testes, testosterone is produced by the Leydig cells.[127] The male generative glands also contain Sertoli cells, which require testosterone for spermatogenesis. Like most hormones, testosterone is supplied to target tissues in the blood where much of it is transported bound to a specific plasma protein, sex hormone-binding globulin (SHBG).
Regulation[edit]
Hypothalamic–pituitary–testicular axis
In males, testosterone is synthesized primarily in Leydig cells. The number of Leydig cells in turn is regulated by luteinizing hormone (LH) and follicle-stimulating hormone (FSH). In addition, the amount of testosterone produced by existing Leydig cells is under the control of LH, which regulates the expression of 17β-hydroxysteroid dehydrogenase.[128]
The amount of testosterone synthesized is regulated by the hypothalamic–pituitary–testicular axis (see figure to the right).[129] When testosterone levels are low, gonadotropin-releasing hormone (GnRH) is released by the hypothalamus, which in turn stimulates the pituitary gland to release FSH and LH. These latter two hormones stimulate the testis to synthesize testosterone. Finally, increasing levels of testosterone through a negative feedback loop act on the hypothalamus and pituitary to inhibit the release of GnRH and FSH/LH, respectively.
Factors affecting testosterone levels may include:
Age: Testosterone levels gradually reduce as men age.[130][131] This effect is sometimes referred to as andropause or late-onset hypogonadism.[132]
Exercise: Resistance training increases testosterone levels,[133] however, in older men, that increase can be avoided by protein ingestion.[134] Endurance training in men may lead to lower testosterone levels.[135]
Nutrients: Vitamin A deficiency may lead to sub-optimal plasma testosterone levels.[136] The secosteroid vitamin D in levels of 400–1000 IU/d (10–25 µg/d) raises testosterone levels.[137] Zinc deficiency lowers testosterone levels[138] but over-supplementation has no effect on serum testosterone.[139]
Weight loss: Reduction in weight may result in an increase in testosterone levels. Fat cells synthesize the enzyme aromatase, which converts testosterone, the male sex hormone, into estradiol, the female sex hormone.[140] However no clear association between body mass index and testosterone levels has been found.[141]
Miscellaneous: Sleep: (REM sleep) increases nocturnal testosterone levels.[142] Behavior: Dominance challenges can, in some cases, stimulate increased testosterone release in men.[143] Drugs: Natural or man-made antiandrogens including spearmint tea reduce testosterone levels.[144][145][146] Licorice can decrease the production of testosterone and this effect is greater in females.[147]
Distribution[edit]
The plasma protein binding of testosterone is 98.0 to 98.5%, with 1.5 to 2.0% free or unbound.[148] It is bound 65% to sex hormone-binding globulin (SHBG) and 33% bound weakly to albumin.[149]

Both testosterone and 5α-DHT are metabolized mainly in the liver.[1][151] Approximately 50% of testosterone is metabolized via conjugation into testosterone glucuronide and to a lesser extent testosterone sulfate by glucuronosyltransferases and sulfotransferases, respectively.[1] An additional 40% of testosterone is metabolized in equal proportions into the 17-ketosteroids androsterone and etiocholanolone via the combined actions of 5α- and 5β-reductases, 3α-hydroxysteroid dehydrogenase, and 17β-HSD, in that order.[1][151][152] Androsterone and etiocholanolone are then glucuronidated and to a lesser extent sulfated similarly to testosterone.[1][151] The conjugates of testosterone and its hepatic metabolites are released from the liver into circulation and excreted in the urine and bile.[1][151][152] Only a small fraction (2%) of testosterone is excreted unchanged in the urine.[151]
In the hepatic 17-ketosteroid pathway of testosterone metabolism, testosterone is converted in the liver by 5α-reductase and 5β-reductase into 5α-DHT and the inactive 5β-DHT, respectively.[1][151] Then, 5α-DHT and 5β-DHT are converted by 3α-HSD into 3α-androstanediol and 3α-etiocholanediol, respectively.[1][151] Subsequently, 3α-androstanediol and 3α-etiocholanediol are converted by 17β-HSD into androsterone and etiocholanolone, which is followed by their conjugation and excretion.[1][151] 3β-Androstanediol and 3β-etiocholanediol can also be formed in this pathway when 5α-DHT and 5β-DHT are acted upon by 3β-HSD instead of 3α-HSD, respectively, and they can then be transformed into epiandrosterone and epietiocholanolone, respectively.[153][154] A small portion of approximately 3% of testosterone is reversibly converted in the liver into androstenedione by 17β-HSD.[152]
In addition to conjugation and the 17-ketosteroid pathway, testosterone can also be hydroxylated and oxidized in the liver by cytochrome P450 enzymes, including CYP3A4, CYP3A5, CYP2C9, CYP2C19, and CYP2D6.[155] 6β-Hydroxylation and to a lesser extent 16β-hydroxylation are the major transformations.[155] The 6β-hydroxylation of testosterone is catalyzed mainly by CYP3A4 and to a lesser extent CYP3A5 and is responsible for 75 to 80% of cytochrome P450-mediated testosterone metabolism.[155] In addition to 6β- and 16β-hydroxytestosterone, 1β-, 2α/β-, 11β-, and 15β-hydroxytestosterone are also formed as minor metabolites.[155][156] Certain cytochrome P450 enzymes such as CYP2C9 and CYP2C19 can also oxidize testosterone at the C17 position to form androstenedione.[155]
Two of the immediate metabolites of testosterone, 5α-DHT and estradiol, are biologically important and can be formed both in the liver and in extrahepatic tissues.[151] Approximately 5 to 7% of testosterone is converted by 5α-reductase into 5α-DHT, with circulating levels of 5α-DHT about 10% of those of testosterone, and approximately 0.3% of testosterone is converted into estradiol by aromatase.[2][151][157][158] 5α-Reductase is highly expressed in the male reproductive organs (including the prostate gland, seminal vesicles, and epididymides),[159] skin, hair follicles, and brain[160] and aromatase is highly expressed in adipose tissue, bone, and the brain.[161][162] As much as 90% of testosterone is converted into 5α-DHT in so-called androgenic tissues with high 5α-reductase expression,[152] and due to the several-fold greater potency of 5α-DHT as an AR agonist relative to testosterone,[163] it has been estimated that the effects of testosterone are potentiated 2- to 3-fold in such tissues.[164]
Levels[edit]
Total levels of testosterone in the body are 264 to 916 ng/dL in men age 19 to 39 years,[165] while mean testosterone levels in adult men have been reported as 630 ng/dL.[166] Levels of testosterone in men decline with age.[165] In women, mean levels of total testosterone have been reported to be 32.6 ng/dL.[167][168] In women with hyperandrogenism, mean levels of total testosterone have been reported to be 62.1 ng/dL.[167][168]

Measurement[edit]
Testosterone’s bioavailable concentration is commonly determined using the Vermeulen calculation or more precisely using the modified Vermeulen method,[174][175] which considers the dimeric form of sex-hormone-binding-globulin.[176]
Both methods use chemical equilibrium to derive the concentration of bioavailable testosterone: in circulation testosterone has two major binding partners, albumin (weakly bound) and sex-hormone-binding-globulin (strongly bound). These methods are described in detail in the accompanying figure.

History[edit]
A testicular action was linked to circulating blood fractions – now understood to be a family of androgenic hormones – in the early work on castration and testicular transplantation in fowl by Arnold Adolph Berthold (1803–1861).[177] Research on the action of testosterone received a brief boost in 1889, when the Harvard professor Charles-Édouard Brown-Séquard (1817–1894), then in Paris, self-injected subcutaneously a "rejuvenating elixir" consisting of an extract of dog and guinea pig testicle. He reported in The Lancet that his vigor and feeling of well-being were markedly restored but the effects were transient,[178] and Brown-Séquard's hopes for the compound were dashed. Suffering the ridicule of his colleagues, he abandoned his work on the mechanisms and effects of androgens in human beings.
In 1927, the University of Chicago's Professor of Physiologic Chemistry, Fred C. Koch, established easy access to a large source of bovine testicles — the Chicago stockyards — and recruited students willing to endure the tedious work of extracting their isolates. In that year, Koch and his student, Lemuel McGee, derived 20 mg of a substance from a supply of 40 pounds of bovine testicles that, when administered to castrated roosters, pigs and rats, remasculinized them.[179] The group of Ernst Laqueur at the University of Amsterdam purified testosterone from bovine testicles in a similar manner in 1934, but isolation of the hormone from animal tissues in amounts permitting serious study in humans was not feasible until three European pharmaceutical giants—Schering (Berlin, Germany), Organon (Oss, Netherlands) and Ciba (Basel, Switzerland)—began full-scale steroid research and development programs in the 1930s.

The Organon group in the Netherlands were the first to isolate the hormone, identified in a May 1935 paper "On Crystalline Male Hormone from Testicles (Testosterone)".[180] They named the hormone testosterone, from the stems of testicle and sterol, and the suffix of ketone. The structure was worked out by Schering's Adolf Butenandt, at the Chemisches Institut of Technical University in Gdańsk.[181][182]
The chemical synthesis of testosterone from cholesterol was achieved in August that year by Butenandt and Hanisch.[183] Only a week later, the Ciba group in Zurich, Leopold Ruzicka (1887–1976) and A. Wettstein, published their synthesis of testosterone.[184] These independent partial syntheses of testosterone from a cholesterol base earned both Butenandt and Ruzicka the joint 1939 Nobel Prize in Chemistry.[182][185] Testosterone was identified as 17β-hydroxyandrost-4-en-3-one (C19H28O2), a solid polycyclic alcohol with a hydroxyl group at the 17th carbon atom. This also made it obvious that additional modifications on the synthesized testosterone could be made, i.e., esterification and alkylation.
The partial synthesis in the 1930s of abundant, potent testosterone esters permitted the characterization of the hormone's effects, so that Kochakian and Murlin (1936) were able to show that testosterone raised nitrogen retention (a mechanism central to anabolism) in the dog, after which Allan Kenyon's group[186] was able to demonstrate both anabolic and androgenic effects of testosterone propionate in eunuchoidal men, boys, and women. The period of the early 1930s to the 1950s has been called "The Golden Age of Steroid Chemistry",[187] and work during this period progressed quickly. Research in this golden age proved that this newly synthesized compound—testosterone—or rather family of compounds (for many derivatives were developed from 1940 to 1960), was a potent multiplier of muscle, strength, and well-being.[188]
Other animals[edit]
Testosterone is observed in most vertebrates. Testosterone and the classical nuclear androgen receptor first appeared in gnathostomes (jawed vertebrates).[189] Agnathans (jawless vertebrates) such as lampreys do not produce testosterone but instead use androstenedione as a male sex hormone.[190] Fish make a slightly different form called 11-ketotestosterone.[191] Its counterpart in insects is ecdysone.[192] The presence of these ubiquitous steroids in a wide range of animals suggest that sex hormones have an ancient evolutionary history.[193]
