A wearable patch can painlessly deliver drugs through the skin
Using ultrasonic waves that propel drug molecules into the skin, the patch could be used to treat a variety of skin conditions.

The skin is an appealing route for drug delivery because it allows drugs to go directly to the site where they’re needed, which could be useful for wound healing, pain relief, or other medical and cosmetic applications. However, delivering drugs through the skin is difficult because the tough outer layer of the skin prevents most small molecules from passing through it.
In hopes of making it easier to deliver drugs through the skin, MIT researchers have developed a wearable patch that applies painless ultrasonic waves to the skin, creating tiny channels that drugs can pass through. This approach could lend itself to delivery of treatments for a variety of skin conditions, and could also be adapted to deliver hormones, muscle relaxants, and other drugs, the researchers say.
“The ease-of-use and high repeatability offered by this system provides a game-changing alternative to patients and consumers suffering from skin conditions and premature skin aging,” says Canan Dagdeviren, an associate professor in MIT’s Media Lab and the senior author of the study. “Delivering drugs this way could offer less systemic toxicity and is more local, comfortable, and controllable.”
MIT research assistants Chia-Chen Yu and Aastha Shah are the lead authors of the paper, which appears in Advanced Materials, as part of the journal’s “Rising Stars” series, which showcases the outstanding work of researchers in the early stages of their independent careers. Other MIT authors include Research Assistant Colin Marcus and postdoc Md Osman Goni Nayeem. Nikta Amiri, Amit Kumar Bhayadia, and Amin Karami of the University of Buffalo are also authors of the paper.
A boost from sound waves
The researchers began this project as an exploration of alternative ways to deliver drugs. Most drugs are delivered orally or intravenously, but the skin is a route that could offer much more targeted drug delivery for certain applications.
“The main benefit with skin is that you bypass the whole gastrointestinal tract. With oral delivery, you have to deliver a much larger dose in order to account for the loss that you would have in the gastric system,” Shah says. “This is a much more targeted, focused modality of drug delivery.”
Ultrasound exposure has been shown to enhance the skin’s permeability to small-molecule drugs, but most of the existing techniques for performing this kind of drug delivery require bulky equipment. The MIT team wanted to come up with a way to perform this kind of transdermal drug delivery with a lightweight, wearable patch, which could make it easier to use for a variety of applications.
The device that they designed consists of a patch embedded with several disc-shaped piezoelectric transducers, which can convert electric currents into mechanical energy. Each disc is embedded in a polymeric cavity that contains the drug molecules dissolved in a liquid solution. When an electric current is applied to the piezoelectric elements, they generate pressure waves in the fluid, creating bubbles that burst against the skin. These bursting bubbles produce microjets of fluid that can penetrate through the skin’s tough outer layer, the stratum corneum.
“This works open the door to using vibrations to enhance drug delivery. There are several parameters that result in the generation of different kinds of waveform patterns. Both mechanical and biological aspects of drug delivery can be improved by this new toolset,” Karami says.
The patch is made of PDMS, a silicone-based polymer that can adhere to the skin without tape. In this study, the researchers tested the device by delivering a B vitamin called niacinamide, an ingredient in many sunscreens and moisturizers.
In tests using pig skin, the researchers showed that when they delivered niacinamide using the ultrasound patch, the amount of drug that penetrated the skin was 26 times greater than the amount that could pass through the skin without ultrasonic assistance.
The researchers also compared the results from their new device to microneedling, a technique sometimes used for transdermal drug delivery, which involves puncturing the skin with miniature needles. The researchers found that their patch was able to deliver the same amount of niacinamide in 30 minutes that could be delivered with microneedles over a six-hour period.
Local delivery
With the current version of the device, drugs can penetrate a few millimeters into the skin, making this approach potentially useful for drugs that act locally within the skin. These could include niacinamide or vitamin C, which is used to treat age spots or other dark spots on the skin, or topical drugs used to heal burns.
With further modifications to increase the penetration depth, this technique could also be used for drugs that need to reach the bloodstream, such as caffeine, fentanyl, or lidocaine. Dagdeviren also envisions that this kind of patch could be useful for delivering hormones such as progesterone. In addition, the researchers are now exploring the possibility of implanting similar devices inside the body to deliver drugs to treat cancer or other diseases.
The researchers are also working on further optimizing the wearable patch, in hopes of testing it soon on human volunteers. They also plan to repeat the lab experiments they did in this study, with larger drug molecules.
“After we characterize the drug penetration profiles for much larger drugs, we would then see which candidates, like hormones or insulin, can be delivered using this technology, to provide a painless alternative for those who are currently bound to self-administer injections on a daily basis,” Shah says.
The research was funded by the National Science Foundation, a 3M Non-Tenured Faculty Award, the Sagol Weizmann-MIT Bridge Program, Texas Instruments, Inc., the MIT Media Lab Consortium, and a K. Lisa Yang Bionics Center Graduate Fellowship.

Here we see a less than thrilled Joni recovering from her recent thumb surgery, as she is wearing an arm sling with ice inside to combat swelling in the area of the surgery. Joni is no stranger to surgery. Her first surgery was the removal of her gall bladder after an acute gall bladder attack back when she was 34 years old. That surgery was performed the old fashioned way before that type of surgery was revolutionized. Joni spent 13 days in a hospital before and after the surgery; an unheard of amount of time these days. Thank God for medical insurance!!
Subsequently, Joni has undergone a pair of hip replacements, six years apart; a surgery to remove a bone spur on her right toe, which believe it or not was the most painful to recover from. She has also had surgery to repair a torn rotator cuff after a fall from a ladder and "minor surgery" to remove a huge MRSA infection on her face last year, which left her with a nasty scar which was the subject of possible plastic surgery, which was avoided through a process called microneedling. . . .and now surgery on her thumb. There is no bigger "big-wig" than Oprah... and if you doubt that, all you need to do is check the cover of the Sep '13 issue of "O" where you'll find Oprah wearing a 3.5lb wig she calls "Wild Thang." Timed perfectly to appear in Mid-August, which is the AAD's 13th Annual National Hair Loss Awareness Month, Oprah covers everything you need to know about hair... and hair loss. On p152, there's a discussion about the significant psychological impact of hair loss and how it makes one feel, "What we could once bend (or straighten or color) to our will feels suddenly, terrifyingly beyond our control."
During my interview with the folks at "O" a few months back, we discussed medical treatments, surgical options, cosmetic 'quick fixes' as well as the future of hair loss treatments.
In the final article, a comprehensive discussion of the modern medical treatments for the most common type of hair loss: chronic, genetic, hereditary female pattern hair loss was included. "Three Good Bets" recommended are topical minoxidil, anti-androgen pharmaceuticals, and hair transplantation. At Bauman Medical Group, we've been prescribing these therapies and more for many years.
As far as Minoxidil goes, we recommend the prescription "Formula 82M" as an alternative to the more common over-the-counter Rogaine and generic formulations. Hormone Boosting Thanksgiving Foods

Paleo Green Bean Casserole
INGREDIENTS
1 yellow onion , sliced thinly
1 tablespoon coconut oil , or butter
2 cups parsnips , chopped
10 oz . mushrooms , chopped
3 cloves garlic , minced
1/4 cup nutritional yeast
1 1/2 cups water
1 1/2 teaspoons fine sea salt
1 lb . fresh green beans
INSTRUCTIONS
Melt a pat of coconut oil, or butter, in a large skillet and add the sliced onions. Saute gently over medium-low heat for about 45 minutes, stirring occasionally until caramelized. Use a splash of water, as needed, to prevent sticking. Once the onions are tender and golden in color, remove from heat and set aside in a bowl for later.
While the onions are cooking, you’ll have plenty of time to steam the parsnips. Fit a steamer basket into a saucepan, and fill with one inch of water. Add the 2 cups of chopped parsnips, and bring the water to boil over high heat. Cover and reduce the heat to low, steaming for about 8 minutes, or until the parsnips are easily pierced with a fork. Transfer the steamed parsnips to your blender container, and set it aside.
Using the same steamer basket and saucepan, break the pound of fresh green beans into one-inch pieces, and toss them into the steamer basket. Bring to a boil, then cover and steam for 6-8 minutes, until the green beans are bright green with a bit of crunch. Transfer the cooked green beans to an 8″ x 8″ glass baking dish.
*Note: At this point, cook the green beans to be as tender as you like– if you prefer them softer, steam them longer. The baking process later will not make them any more tender, so this is your chance to achieve the texture you like.
Once the onions have finished caramelizing, you can use the same pan to saute the mushrooms and garlic. Melt another pat of coconut oil or butter, and saute the garlic for about 3 minutes, then add the mushrooms. Cook for about 6 minutes, until liquid is released from the mushrooms.
Spread half of the mushroom mixture into the baking dish of green beans, and pour the other half of the mixture, along with any liquid, into the blender container with the steamed parsnips.
Into that blender container, add 1 1/2 cups water, 1 1/2 teaspoons sea salt and 1/4 cup of nutritional yeast to the cooked parsnips and mushrooms. Blend until smooth and creamy. (This mixture may be a bit salty to taste at this point, but keep in mind that it will be diluted when it’s poured over a pound of green beans!)
Pour the creamy sauce over the green beans and mushrooms in the glass baking dish, and stir to coat well. (You may very well have more sauce than you need for this recipe– I only used about 1 1/2 cups to coat the vegetables well. Save the rest for a future dish!)
Smooth the creamy vegetable mixture with a spatula, and top with the caramelized onions to finish.
*At this point, you could cover the dish and store it in the fridge until you’re ready to heat and serve.
When you’re ready to bake, preheat your oven to 350F and bake uncovered for about 30 minutes, until bubbly. Serve warm and enjoy!

Herb Roasted Turkey
INGREDIENTS
1 whole bone-in turkey breast, 6 1/2 to 7 pounds
1 tablespoon minced garlic (3 cloves)
2 teaspoons dry mustard
1 tablespoon chopped fresh rosemary leaves
1 tablespoon chopped fresh sage leaves
1 teaspoon chopped fresh thyme leaves
2 teaspoons kosher salt
1 teaspoon freshly ground black pepper
2 tablespoons good olive oil
2 tablespoons freshly squeezed lemon juice
1 cup dry white wine
INSTRUCTIONSRemove the turkey from the refrigerator 1 hour before roasting.
Combine the butter, sage, rosemary, thyme, and parsley in a food processor and process until smooth. Season with salt and pepper.
Preheat the oven to 450 degrees F.
Season the cavity of the turkey with salt and pepper and fill the cavity with half of the carrots, celery, and onion. Rub the entire turkey with herb butter and season liberally with salt and pepper.
Put 4 cups of the chicken stock in a medium saucepan and keep warm over low heat.
Place the remaining vegetables on the bottom of a large roasting pan. Put the turkey on top of the vegetables, put in the oven, and roast in the oven until lightly golden brown, 45 minutes. Reduce the heat to 350 degrees F and continue roasting, basting with the warm chicken stock every 15 minutes, until an instant-read thermometer inserted in the thigh registers 160 degrees F, about 2 to 2 1/4 hours longer. Remove the turkey from the oven, transfer to baking sheet and tent loosely with foil, and let rest 20 minutes before slicing.
For the sage gravy:
Strain the cooking liquid from the roasting pan into a medium saucepan (should be about 4 cups, if not, add more stock to make 4 cups). Add the neck, bring to a boil, add the sage leaves, remove from the heat, and let steep for 15 minutes. Remove the sage leaves.
Melt the butter in a medium saucepan over medium-high heat. Add the flour and cook for 1 minute. Add the wine and cook until reduced. Whisk in the stock and cook until thickened and smooth, about 5 minutes. Season with the salt and pepper and chopped sage

4 teaspoons poultry seasoning

2 cups almond flour
3 eggs whisked
INSTRUCTIONS
Preheat oven to 350º F.
Heat ghee or oil in a large cast-iron or oven-proof skillet over medium heat. Add onion, celery, apple, mushrooms, cranberries or dates, parsley, poultry seasoning, salt, and pepper. Sauté until very soft, about 7 minutes.
Remove from heat. Keep in oven-proof skillet or transfer to 9x9″ baking dish. Stir in almond flour and mix well. Add the whisked eggs and stir well. Bake for 45 minutes to 1 hour, or until browned on top.

INSTRUCTIONSPreheat oven to 350 degrees F (175 degrees C).
Place green beans and soup in a large microwave-safe bowl. Mix well and heat in the microwave on HIGH until warm (3 to 5 minutes). Stir in 1/2 cup of cheese and heat mixture for another 2 to 3 minutes. Transfer green bean mixture to a casserole dish and sprinkle with French fried onions and remaining cheese.

On p152, there's a discussion about the significant psychological impact of hair loss and how it makes one feel, "What we could once bend (or straighten or color) to our will feels suddenly, terrifyingly beyond our control."
During my interview with the folks at "O" a few months back, we discussed medical treatments, surgical options, cosmetic 'quick fixes' as well as the future of hair loss treatments.
In the final article, a comprehensive discussion of the modern medical treatments for the most common type of hair loss: chronic, genetic, hereditary female pattern hair loss was included. "Three Good Bets" recommended are topical minoxidil, anti-androgen pharmaceuticals, and hair transplantation. At Bauman Medical Group, we've been prescribing these therapies and more for many years.
As far as Minoxidil goes, we recommend the prescription "Formula 82M" as an alternative to the more common over-the-counter Rogaine and generic formulations. What is PRP - Platelet Rich Plasma?
PRP Platelet Rich Plasma is the use of a person’s own blood platelets to enhance hair growth as a stand-alone treatment or to improve the recovery and results of hair transplant surgery. In the field of tissue regeneration, research is continuing to progress regarding the use of PRP’s ability to stimulate stem cells, improve wound healing, and rejuvenate skin and hair follicles.
How is PRP performed as a stand-alone treatment for hair growth?
A small sample of blood is obtained from the patient. This sample is processed in an FDA-cleared device to separate the platelets from other components like red blood cells. Platelets are then chemically ‘activated,’ releasing powerful molecules which have profound biologic effects on hair follicles and skin. Importance of COVID-19 Antibody Testing
As Tedros Adhanom Ghebreyesus, head of the World Health Organization stated in a press conference last month, “The key message is to test, test, test.”
First of all, let’s discuss the 2 main types of COVID-19 testing that are currently being offered to the population and their differences.
The first is a nasopharyngeal swab, which detects the presence of active COVID-19 genetic material. As viruses replicate, their genetic material (i.e DNA or RNA) becomes exposed and we are able to use PCR (Polymerase Chain Reaction) technology to detect an active viral infection. Nasopharyngeal swab testing will NOT tell you if someone was previously infected and will NOT tell you if someone has antibodies in their system to COVID-19.
The second is antibody testing, which detects immune cells (i.e. Antibodies) produced against COVID-19 within the bloodstream. This can be achieved by performing a standard blood draw on a patient or finger stick. When our bodies become infected by a virus, our immune system builds specific antibodies to the virus to not only tag it for destruction, but also helps protect us from future infections as well.
Antibody finger prick testing uses a small sample of blood to look for specific biomarkers associated with Covid-19 — Immunoglobulin G (IgG) and Immunoglobulin M.
IgM – Your body makes IgM antibodies when you are first infected with a new virus or bacteria. When positive, this typically means there is an active and acute infection occurring within the body.
IgG – As your body responds to an infection it begins to convert IgM antibodies to IgG antibodies. When positive, this typically means a person has been vaccinated or has overcome an infection.
As countries across the world attempt to halt the spread of COVID-19, many of their efforts are being hampered by shortages of crucial testing kits. To combat this, the Food and Drug Administration (FDA) has granted an emergency use authorization (EUA) for coronavirus tests within the US, which allows several companies to gain regulatory approval for test kits more quickly.
Large population testing is crucial at not only localizing the prevalence within each county, but allows us to get a better idea of which patient populations (e.g., obesity, hypertension, diabetes, age) are most affected.
Nasopharyngeal testing also allows us to catch COVID-19 positive patients earlier at which point we can intervene with therapy and quarantine them if they have an active infection. Being able to test a person for antibodies to COVID-19 on the other hand, allows 2 main talking points:
COVID-19 Antibodies from previously recovered patients could help speed up the recovery of those who are in critical condition (e.g., on ventilator support, worsening oxygen levels, worsening chest x-ray) by helping to decrease the viral load quicker.
There are several studies currently underway across the U.S. on active patients in critical condition who have been transfused with COVID-19 positive plasma antibodies. People who test positive for the virus could help allow those who are positive to return to work and help restimulate the economy. It is thought that having positive antibodies may in fact protect those from reinfection.
We are not out of the woods just yet and still do not have a clear picture of what is to come in the near future, but if there is one thing we have learned from this is better preparedness.

5 Tips to a Fit Summer
Let’s face it…summer has arrived, the kids are out of school and family trips to the beach and poolside are in full fledge. Summer is a very easy time to get out of your normal routine and fall off the wagon! Don’t get discouraged as I am going to give you a few tips to help keep you one step ahead of the rest!
Top 5 Tips to keep you in Tip Top Shape:
1. Plan Ahead
Remember, failing to plan is planning to fail
Create your healthy shopping list before your vacation
Write down your meals for the following day the night before you
Try utilizing online grocery shopping like HEB, which allows you to strategically pick the foods instead of rushing through the grocery store tossing items into the cart.
2. Stick to Fat Burning Foods
Skip the Breadline – Replace the burger or hot dog buns for lettuce wrap.
Avocados – Avocados contain mannoheptulose, a sugar that blunts insulin release and boosts calcium absorption, two functions key to fat loss.
Green Tea – Great drink to take iced to the pool and helps to curb cravings, improve mood, and is a natural fat-burner.

PRP Platelet Rich Plasma is the use of a person’s own blood platelets to enhance hair growth as a stand-alone treatment or to improve the recovery and results of hair transplant surgery. In the field of tissue regeneration, research is continuing to progress regarding the use of PRP’s ability to stimulate stem cells, improve wound healing, and rejuvenate skin and hair follicles.
How is PRP performed as a stand-alone treatment for hair growth?
A small sample of blood is obtained from the patient. This sample is processed in an FDA-cleared device to separate the platelets from other components like red blood cells. Platelets are then chemically ‘activated,’ releasing powerful molecules which have profound biologic effects on hair follicles and skin. Under comfortable local anesthesia, this powerful cocktail of growth factors, cytokines and other proteins is injected into the area of scalp where weak hair follicles exist. Microneedling of the skin is also performed. Mild soreness, if any, resolves in less than a day and improved hair growth can be measured in a matter of weeks.
Is all PRP the same?
Unfortunately not, and this is where it can get confusing for patients (and physicians!). At Bauman Medical Group, we use the FDA approved Autologel PRP process to separate and activate Platelet Rich Plasma for use as a stand-alone therapy or in conjunction with hair transplantation. Bauman Medical Group is proud to be the first complete hair transplant team to be fully certified by Cytomedix to produce Autologel. That means that every member of our team has passed certification for the preparation and application of Autologel PRP. At Bauman Medical Group, we feel that our PRP/ACell protocols are providing significant stimulation of hair growth and hair follicle rejuvenation.
