One of the most common of all plastic surgery procedures is rhinoplasty, also commonly known as a nose job. Rhinoplasty surgery can serve dual purposes, creating a more aesthetically pleasing look and also helping breathing conditions, such as a deviated septum.. Whether it's because of a genetic defect or some kind of injury, many people have trouble breathing through their nose. A rhinoplasty can counteract this, clearing the airway so you can breathe freely. It can even repair a deviated septum, straightening it and removing any blockages in the airway.
Perfecting surgery with this three-dimensional structure (the nose) takes years to master and continues to improve. Little did we know that rhinoplasty maneuvers that were used three years ago could cause disastrous results today. Rhinoplasty surgery is forever evolving! My fellowship director, J. Regan Thomas, MD, told me something that I’ll never forget – “you haven’t learned anything about rhinoplasty until you’ve performed at least a thousand procedures and followed them for many years”. This statement epitomizes why fellowships are so valuable. Some of the needed experience and potential pitfalls are circumvented by first hand observing and learning the analysis, judgment, techniques, complication management and most importantly, results from a seasoned rhinoplasty surgeon. This is why I super-specialized in rhinoplasty surgery during my fellowship in Facial Plastic & Reconstructive Surgery. The training catapults you years ahead of many other surgeons that aren’t fortunate to have post-graduate training. Many cosmetic surgeons are taught that aggressive cartilage removal is a procedure of the past. Today’s concept is “less is more”. Less cartilage excision, cartilage repositioning, camouflage techniques, structural grafting and suturing techniques are being taught in most rhinoplasty courses and at our national meetings.

The black loop hanging off the board supporting my right leg on the recliner is a shoestring.
For 3 months, walking consisted of someone coming over, picking up the shoestring, hoisting my leg, me grabbing my walker, and then the two of us moving forward in concert.
After 12 weeks of intensive knee rehab, I could hold the knee slightly bent with the weight of the fixator for short periods.
Tried my best to tear off that leg.

This shot is post 2 weeks MVA after first 4 surgeries (ORIF of patellar fractures with avulsion fx's of quad, patellar tendons = complete separation, ORIF of talar, calcaneal, tib/fib compound fractures with autograft bone from R tib under knee to talar neck/dome) to R knee and ankle at UMMC in Baltimore.
I wore a LOT of boxers that year.

last week i had the pleasure of attending GEL health, a conference designed solely around discussions of a patient's experience in the world of health care. it was organized by GEL ("good experience live") founder Mark Hurst, an old friend and former colleague from many, many moons ago.
mark and i met while working at yoyodyne, brainchild of permission marketing guru seth godin. mark and i were there when everyday people first crossed the so-called chasm and began emailing each other; he was the one who told me amazon.com was the next big thing; he helped me write my very first href hotlink. this was 1996.
fast forward thirteen years, and mark's become a guru in his own right. but he's not a marketing guru; he's not a sales guru. he's a people guru: his mission is improving the customer experience for people like you and me. in other words, marketing and sales (and health care) gurus turn to HIM for advice on how not to be assholes. and they listen.
after leaving yoyodyne, mark went on to start creativegood and goodexperience. he even wrote a book about it.
listening to him speak, all these years later, i marveled at the notion of someone pursuing a dream, and actually making it happen.
