6.How to avoid being trapped by poor quality VoIP
VoIP stands for “Voice Over Internet Protocol” … Not ‘Voice Over the Internet’.
I have witnessed inexperienced sales people promise ‘perfect voice quality’ using ADSL internet connections. The reality is ADSL is simply not reliable enough to use for business grade communications.
VoIP technology can work extremely well. It can deliver a secure, high quality and low cost phone service. To avoid being trapped by poor audio quality, discuss your options and strategies to construct a purpose built network for voice communications.

Finally! With the final piece of the puzzle provided to me by Cisco TAC, I've got a Nokia E-Series phone connecting directly to Cisco CallManager Express on our 2811 ISR (running IOS 12.4(9)T1). This is using SIP, not the (still coming?) software from Cisco/Nokia that makes the phone talk SCCP instead.
ie. (for friends who read all this and said "huh?") with one of these phones, you can have it connect via wi-fi to your office's phone system and make/receive normal local calls via the office phone lines rather than paying mobile rates. or that's the idea, anyway.
what doesn't (apparently) work so well is connecting to other SIP providers on the internet, since Nokia haven't implemented NAT traversal for getting through firewalls yet. You might be able to do it, though, depending on where you are.

If you're not interested in CallManager Express, you might want to make it work with Asterisk, which I've also done.
A Nokia E60 - I just had the firmware upgraded in the hopes of having an improved SIP client. It kind of works - that is, if you cheat and run Asterisk somewhere and make a SIP trunk back to your Cisco Router running CallManager Express. But it won't register directly with CallManager Express, goddammit.
So anyway. No GSM SIM in there, but you can still make SIP calls, of course, as evident by those two little icons on the right (wireless active and "IP telephony" active).
Post-upgrade, the phone was acting pretty confused - really long (as in, 5 minutes or so) bootup times, and such. *#7370# does a full reset, which helped.
5.Too many customers per account manager in most Telco’s
It takes several (5+) years, endless training sessions and raw experience to have any real hope of understanding the technology in modern Voice and Data Networks. This means that most customers have to put up with unsuitably qualified, inexperienced account managers who are forced into reactive modes due to the high customer to account manager ratios.
Would you trust your business with an inexperienced Accountant or perhaps a 1st year Solicitor?
I’ve lost count of the times I’ve heard “we have an account manager, but we never hear from them and we can never raise them on the phone”.

Typically the Telco sales person gives a great spiel and tells you everything you wanted to hear.
i.e. “It’s Easy…Everything is on easy to read bill…you will save up to 16%... one easy monthly payment.”
Here’s the real FACTS;
Telco’s are primarily interested in your monthly call spend!
Telco call rates have consistently dropped annually over the past 10 years
A tiny discount (up to 16%) on “qualifying calls” to lock you into fixed prices for 3 – 5 years. (Think about it this way…would you lock in a fixed Mortgage rate in a falling interest rate market?)
In almost all cases the tiny Telco discount falls a long way short of off setting the cost of paying for the their re-branded phone system.
I typically show people how to pay for a new phone system from savings made on their monthly phone bill (for customers spending more than $497 per month on calls)
