The CDC is investigating a Covid-19 outbreak on board a Carnival cruise ship - CNN
(CNN) The Centers for Disease Control and Prevention is investigating a recent Covid-19 outbreak on a Carnival cruise ship that docked in Seattle after a two-week voyage.
The Carnival Spirit, which sailed through the Panama Canal, departed Miami on April 17 and arrived in Seattle on May 3, according to a statement from the cruise line. The ship holds 2,124 guests and 930 crew members, Carnival said.
The CDC says it's not permitted to publicly share the number of passengers and crew members who quarantined or tested positive. But the ship is labeled as orange status per the agency's cruise ship Covid-19 threshold, indicating that 0.3% or more of total passengers and or crew members tested positive, according to CDC's guidelines.
Carnival Spirit is one of 62 cruise ships currently sailing at orange status, the CDC said in a statement to CNN.
Both the cruise line and CDC said there were no severe outcomes or serious health issues among those who tested positive, and Carnival said most guests were asymptomatic.
"Our health and safety protocols exceed CDC guidelines and were closely followed including vaccination requirements and pre-cruise testing of all guests. Our crew are also vaccinated and wear masks," Carnival said in a statement to CNN.
"Our protocols are designed to flex up as needed and additional measures were implemented during the voyage," the statement added. "In addition, all guests who were scheduled to continue on with the ship's next cruise to Alaska were tested and any guests and their traveling companions who tested positive were disembarked."
The ship departed Seattle for Alaska later on May 3, according to Carnival's statement.
The CDC is working with the cruise line as well as state and local health agencies to "enact existing Covid-19 agreements and protocols," the statement added.
Passengers say cruise ship was "overwhelmed"
Some passengers who were on board the Carnival Spirit alleged that crew members mishandled the Covid-19 outbreak on the ship.
Darren Siefertson, who is from Las Vegas, Nevada, told CNN he boarded the ship on April 17 from Miami and was informed by April 26 that there was a Covid-19 outbreak on the ship.
After testing positive, Siefertson said he was promised to be moved to an isolation cabin but ended up being forced to stay in the same room as his roommate.
Siefertson said when he called the medical center for an update, the center had already closed before its scheduled closing time, and no one was picking up the phone.
When Siefertson's cabin mate went to guest services to ask what they should do, he was told there were no more isolation cabins available, and he would have to stay in the same room as Siefertson.
Ship guests also complained that room service took "hours to get food to those who were in quarantine," Siefertson said.
"There were times we ordered our food at 1 p.m. and it didn't show up until 7 p.m.," Walter Babij, who was on the cruise with his wife, told CNN. Both Babij and his wife tested positive for Covid-19 while aboard the ship and quarantined in an isolation cabin.
"There were also a few times we didn't get our complete orders. We would have to call several times to track down our food. We were completely dependent on them," Babij said.
Siefertson also complained that a few days after testing positive, his cabin and two other cabins near him began to reek of sewage.
"You couldn't even call to ask someone to fix the problem because no one would pick up the phone and we couldn't leave our room, so I had to stay in this room that smelled like a toilet. It was terrible," Siefertson said. "Carnival said they managed the situation, but that is so not true. They were so overwhelmed."
Both Siefertson and Babij told CNN they felt the cruise line was "overwhelmed."
"I don't think they expected that level of cases and when it got to a certain level they didn't know what to do. It was a perfect storm," Babij said.

WHO Says 15 Million Have Died From Pandemic, With Highest Toll in India
NEW DELHI—The World Health Organization estimated that nearly 15 million people had died from causes related to the coronavirus pandemic by the end of 2021, putting the toll from Covid-19 at nearly three times the number that had been officially recorded by countries.
India suffered the highest toll of any country in the world, according to the report released Thursday, but most of the deaths have gone unrecorded. The 4.7 million people who had died in India by the end of last year, according to WHO estimates, was nearly 10 times the official tally at that time of 481,000 deaths. India’s count has risen to about 524,000 since then.
The report, which was compiled by scientists from around the world, has sparked fierce resistance from India, where government officials have denounced the health agency’s methodology and objected to the release of the report.
“Despite India’s objection to the process, methodology and outcome of this modeling exercise, WHO has released the excess mortality estimates without adequately addressing India’s concerns,” India’s health ministry said Thursday.
The report put India’s toll for the pandemic far higher than other countries. Russia was the next highest with nearly 1.1 million deaths, according to the report, followed by Indonesia at over one million and the U.S. at more than 930,000. India has more than 1.3 billion people, and health experts say many people in the country didn’t get tested for Covid-19 and died at home, particularly during a devastating Delta wave last year.
The WHO estimated that excess deaths in the Africa region over the same period were 1.25 million, five times more than the official figure. Three African countries—Egypt, Nigeria and South Africa—were among the top 20 countries that account for more than 80% of the estimated global excess mortality, the WHO added.
The new figures come as official data on the continent continue to show extremely low death rates, but evidence of higher tallies have begun to emerge.
In Tanzania, where former President John Magufuli denied that Covid-19 existed before dying suddenly in March 2021, some burial sites have tripled in size since 2020.
At the main morgue in Zambia’s capital, Lusaka, a third of the total deceased people tested positive for Covid-19. In Kenya, obituary announcements more than doubled during last year, as the country battled a surge in infections. All nations have some of the world’s lowest official death rates.
Across Africa, a deficit of Covid-19 testing and creaking medical infrastructure have made it harder to determine the true extent of the pandemic. Official figures show Africa has registered only 11.4 million cases of Covid-19, with 252,158 deaths across a continent of 1.3 billion people, according to the African Union.
Countries such as Tanzania refused to collect data on the virus. Others, such as the Democratic Republic of Congo, say the official death toll from Covid-19 is 1,300, though fewer than 500,000 tests have been conducted on the country’s 90 million people. Altogether, only 105 million Covid-19 tests have been conducted in Africa.
Dr. John Nkengasong, director of Africa CDC, believes that the deficit of testing and inadequate healthcare infrastructure masked Covid-19’s true toll on the continent. “We believe that many people died from other diseases such as heart conditions, because they could not access hospitals during the lockdowns,” he said.
Getting an accurate count of Covid-related deaths, including data such as demographics, times and locations, can help countries judge the effectiveness of their Covid-19 policies. The data can also be used to assess the deadliness of Covid-19 variants and the efficacy of vaccines.
“Knowing how many people died due to the pandemic will help us to be better prepared for the next,” Dr. Samira Asma, assistant director general at the WHO in charge of data and analytics, said Thursday.
The WHO’s report covers those who died directly related to the Covid-19 pandemic and those who died indirectly, such as people unable to get proper medical care for other illnesses. It used statistical models taking into account factors such as local government and national data, demographics and policies on Covid restrictions.
Many Indian health experts said they weren’t surprised by the WHO estimate because it is in line with several other studies that have pegged India’s death toll at many multiples of the official count.
“It is very clear from all sorts of data that South Asia has been pretty high and India is really the highest,” said Amir Ullah Khan, research director at the Centre for Development Policy and Practice. “I don’t see anybody being surprised.”
But the Indian government has aggressively pushed back against Covid-19 mortality estimates that surpass its own official tally, denouncing studies as inaccurate or ill-informed. A study by the scientific journal The Lancet published in early March, for example, estimated pandemic-related deaths in India at more than four million. A day later, the health ministry released a statement saying the study was speculative and misinformed.
Last month, weeks before the WHO released its report, the ministry went public with a statement criticizing its methodology as a one-size-fits-all approach. On Tuesday, India took the unusual step of publishing its 2020 annual report on nationwide registered births and deaths months ahead of schedule.
V.K. Paul, a top health official who serves as chief of India’s Covid task force, said the report was released early by two or three months to show that India’s accounting is fair and transparent. “Since we now have the real count of all-cause deaths, there should be no logic for using modeling-driven data based on assumptions,” Mr. Paul said.
The data showed that about 8.1 million deaths were registered in 2020, an increase of nearly 475,000 deaths, or 6.2%, from a year earlier. That was lower than the 10% increase in deaths reported in 2019 and the 7.5% uptick registered in 2018. The modest increase in deaths in 2020, health experts said, is in line with what India would see in a normal year, but it fails to capture the pandemic-related deaths that year.
Registered deaths in India have been steadily increasing in recent years. In the past, many Indians, especially in rural regions of the country, didn’t bother registering the deaths of family members, which requires paying a fee and following some government procedures. But more people have been registering deaths in recent years to obtain official death certificates for inheritance and other purposes.
Health experts said a number of issues likely led to widespread undercounting of Covid-19 deaths in India. Many people died at home, untested, after failing to get medical treatment. Many Indian families were also reluctant to report loved ones had died from Covid-19 because they feared being shunned by their neighbors. Registrar offices were closed for months. Some hospitals and local governments were also pressured to underreport Covid-19 deaths, health experts said.
Dr. Jayaprakash Muliyil, one of India’s leading epidemiologists, said New Delhi has improved its accounting of deaths in recent years, but official tallies still miss the mark. Add in the chaos of the Covid-19 pandemic, he said, and accurate counts are incredibly difficult to obtain.
“There’s a considerable amount of underreporting of deaths in India as compared to many developed countries,” said Dr. Muliyil, who is also a member of the national advisory group on immunization. “Things have improved in the last 30 years, but that doesn’t mean that we are anywhere near to a perfect system.”
Health experts and scientists said it is unusual for India to push back against WHO reports or other scientific studies. The fact that it did in this case, they said, likely reflects sensitivity to the staggeringly high death count.
“It is unprecedented,” said Anand Krishnan, a professor of community medicine at the All India Institute of Medical Sciences in New Delhi who helped review the WHO’s methodology.
Dr. Krishnan said any model that the WHO used to estimate Covid-related deaths on a global scale could be criticized, but the point of modeling is to fill in holes where there is a lack of data. India hasn’t provided nationwide mortality data to the WHO. The estimate is extrapolated from death statistics from over a dozen Indian states based on information gleaned from a variety of sources, including local governments and public information, such as media reports.
Even before the pandemic, India has been reluctant to share mortality data with the WHO, Dr. Krishnan said. “It is always, I think, a sensitive issue,” he said. “India has not been very forthcoming.”
The sensitivity was magnified, Dr. Khan said, when photos and videos of overwhelmed hospitals and lines for oxygen tanks were beamed world-wide last year.
“Over the last two or three decades, India was beginning to move away from elephants and poverty to IT towers and software providers,” Dr. Khan said. “That took a big hit, because the abiding images of last year in the international media were bodies being burned in the open, bodies coming out of rivers.”

Babi Yar (Russian: Бабий Яр; Ukrainian: Бабин Яр, Babyn Yar) is a ravine in the Ukrainian capital Kiev and a site of a series of massacres carried out by the Nazis during their campaign against the Soviet Union. The most notorious and the best documented of these massacres took place on September 29–30, 1941, wherein 33,771 Jews were killed in a single operation.
