Opioids: Care Coordination, Transitions, and Engagement

This one-day conference will highlight innovative approaches to improving transitions of care for individuals with opioid use disorder. New Jersey has made fighting the opioid epidemic a priority. Yet thousands of citizens continue to die each year from opioid overdoses, and many more are at risk because they are unable to find care in the community when they are in crisis or are transitioning from one level of care to another. This conference will bring together policymakers, caregivers, and advocates from New Jersey and other states to discuss innovative programs that seek to stem the tide of opioid addiction and overdoses by implementing strategies to connect patients to care across the continuum. The morning panel will focus on strategies for improving care coordination and transitions for justice-involved populations, while the afternoon panel will focus on promising programs that employ care coordination to facilitate care transitions in the community.
