I have used various blank books as journals for the past ten years - most recently moleskines, though for my next one I'll be trying an Exacompta sketch book. There's always the problem that they're hard to differentiate when you're done with them. I had some torn strips of very nice, heavy Rives BFK paper leftover from my letterpress project, so I made them into covers and lettered my own titles onto each one.
All it involved was some pretty straightforward folding; the covers hold themselves in place pretty well and you can write on the spines as well as on the front and back.
The titles were originally given as each journal was completed, not made up just for this project ;) I usually wait until the journal is nearly finished before titling it.
