This is not computer generated, nor heavily photoshopped, image.

I don't know the popular name of the technique, but i prefer to call it stepped-zooming.

Can only be done using manual zoom lenses, with manual focusing...

Basically, this is a long exposure shot, intentionally misfocused to get the bokeh.

Using 4 sec. exposure with a 18-250 mm lens, gave me the chance to manually zooming forward step by step in total of 8 steps (so about half a second stop for each step).
