On this 11th day of September A.D. eighteen hundred and sixty-five, personally appeared before me Prothonotary of the District Court, in and for the said County and State, Phillip Berry aged 32 years, a resident of Philadelphia State of Penna., who, being duly sworn, according to law, declares that he is the identical Philip Berry who enlisted in the service of the United States at Buffalo N.Y., on the 26th day of August, in the year 1862, as a Sergeant in company "D", commanded by [blank], in the 164th regiment of N.Y. Vols., in the war of 1861, and was honorably discharged on the 7th day of September in the year 1865; and that while in the service aforesaid, and in the line of his duty, he received a gun shot wound ^in the right knee in action at Cold Harbor Va. June 3 1864 - by reason of which the leg is nearly useless. Sent to Satterlee U.S. Hospt. Phila. June 11th 1864 - Transferred to 105th Co. 2d Batt. V.R.C. Jany. 1865,

He hereby appoints W.N. Ashman, No. 1307 Chestnut Street, Philadelphia, Pa., to be his true and lawful Attorney, with full power to present and prosecute this claim and receive and receipt for any order, certificate, or money which may be payable under it.
