Verbal Abuse
I selected the lips from the original photo, deleted the background, smoothed the edges with Refine Edges and Eraser Tool, and created a new layer with the new selection, which I colored red with the Brush Tool. I then used the Filter "Poster Edges" to give it a harder look and used the Burn Tool to make the line between the lips as well as the lips themselves darker. I also added the Dissolve layering effect.
I selected the lines of the text bubble in the same way and colored them red. I added the Dissolve layering effect.
I used the Dissolve layering effect on the paragraph of text and 95% opacity to show how verbal abuse distorts the victim's viewpoint and mentality. The words looked more distorted on Photoshop, actually.
I used a linear gradient background of scale 1000%.

Rule of thirds: the word "abuse" and the sentence in red "it's your own fault" are at intersections.
This image is purposefully dark to match its subject matter.
