The top photo shows water actually running out of the joint of the sink+bath pipe with the soil pipe after the plumber disturbed it. The slowly overflowing toilet caused it to leak enough to come through the ceiling.
The bottom photo shows the new joint, and the issue they had joining it to the non-standard bore of the bath+sink pipe.
Notice the "lath and plaster" ceiling construction and subsequent layer of artex and, finally, a smooth plaster skim. There's concern that one, or more, of these may contain asbestos.
