ttn_v_bw_o_n (UI Titan IIIC at LC-41)

An unidentified Titan III-C (poss. veh. no. 3C-12) on the pad at LC-41, August 1966.

 

Although this photograph has been fairly widely published, I've found no solid identification of this Titan III-C, although multiple searches have yielded an associated date of 26 August 1966, that being the launch date of vehicle no. 3C-12.

 

If correct, per Wikipedia (and others):

 

"The fifth Titan III-C (August 26, 1966) failed shortly after launch when pieces of the payload fairing started breaking off. Around 80 seconds, the remainder of the shroud disintegrated, causing the Titan to pitch down at a sharp angle that resulted in one Solid Rocket Motor separating. The automatic Inadvertent Separation Destruction System (ISDS) activated and destroyed the entire launch vehicle, sending its cargo of eight Initial Defense Satellite Communication System (IDSCS) communications satellites into the Atlantic Ocean."
