This is an RBOKL shot - Reverse Bottle On Kit Lens. I hand held it together - if you want to get sophisticated, use electrical tape! See here:

My camera had the standard kit lens (18-105mm) mounted, with my macro lens (front against the kit lens). The macro bottle DOF lever must be fully OPEN. Closing the aperture down will only narrow the field of view.
Control the DOF with the kit lens (f32 in this case). If you want really shallow DOF (depth of field) then leave the kit lens wider open.
Macro bottle is an old manual lens from an old film camera (55mm, f1.2).
Shoot with kit lens on tele (105mm) to minimise vignetting from the macro bottle. The bottle should also have a large aperture to minimise vignetting. To maximise magnification, set the macro bottle to infinity, kit lens to closest focus.
