The US lacks a universal medical coverage program. There is nothing like England's NHS (except for the military and veterans). The die-in in front of the Federal Building in San Francisco was part of a rally and march for Medicare for All, a movement to expand Medicare (coverage for the older or disabled folks). The whole demonstration, involving the blocking off of downtown streets in San Francisco, was planned and permitted and the police made sure that everybody kept to the agreed lines.
