(we have been given one, haven`t we?? I lost a point for that!! grrr) The document is well laid out and clearly divided into sub-sections.
You have demonstrated a clear understanding of the brief.
This is a very well designed questionnaire, you have though of your personal branding and the layout makes it easy to identify the various sections, you have used tick boxes for speed of answering and the colour coding makes the presentation very professional, you might consider printing on both sides to save paper.
