Sméagol (Gollum Transformation)

"Sméagol (or Trahald), later known as Gollum after The One Ring had come into his possession, was a Stoor, one of the first three early Hobbit-types. The Ring came to Sméagol one day, after murdering his cousin Déagol for it, who had first recovered it from the river they were fishing from, after which he underwent a corruption that transformed him completely into the creature that would come to be known as Gollum."

as seen in The Lord of the Rings: The Return of the King.
