2 SB-600's, one in 38" umbrella camera right, one behind her aimed at the wall. Triggered with Nikon CLS. White wall camera left acted as a reflector to soften shadow side of her face.

 

Minimal photoshop: Adjusted saturation & white balance, spot healed eyes to remove commander pulse, dodged background (as per Zack Arias' workflow tutorial here)
